Customer respondent, please complete the following questionnaire and email to Ronald Jean-Baptiste Ronald.Jean-Baptiste@hq.dhs.gov and Donald Nusbaum at Donald.Nusbaum@hq.dhs.gov, no later than return it to the attention of the individual named below, no later than the date specified in the notification letter.
