VII. Standard Operating Procedures
A. The COBRATF Lead Paramedic or, in his/her absence, the most senior paramedic on duty, is responsible for assuring all personnel have and wear level “C” while treating patients from the toxic agent area.
B. Paramedics have been approved by the MSS contract physician to wear two (2) pair of medical examination gloves or one (1) pair of butyl rubber gloves while treating patients from the toxic agent area.
C. After the patient(s) has been delivered to the training building medical room from inside the toxic agent area, the medical room is monitored by the MINICAMS for a minimum of ten (10) minutes IAW CDP COBRATF 8.6.
D. The paramedics immediately monitor the patient with an ICAM IAW CDP COBRATF
1.21, or LCD (if available), and M8 detector paper IAW CDP COBRATF 1.23, to verify that the patient is decontaminated or clean.
E. If air monitoring technicians notify the paramedics via Safety Control that the medical room is clean and the patient is verified clean by use of an ICAM or LCD, and M8 detector paper, the paramedics may remove their PPE. The patient may then be transported, if needed, to the hospital via ambulances or air evacuation at the discretion of the Lead Paramedic and/or senior paramedic, or contract physician, if on site.
F. If air monitoring technicians notify the paramedics via Safety Control that the medical room is contaminated, perform the following procedures:
1. Retrieve modified garden hose, two (2) buckets and two (2) sponges from beneath the sink. Tighten the bracket around the hose onto the faucet adapter over the sink. Fill buckets with soap and cold water and place a sponge in each bucket.
2. While the patient is laying on the treatment/containment bed, sponge them with soap and water from head to toe, including both sides of the body, to ensure decontamination of all parts of the body. Rinse with cold water from the hose.
3. Decontamination includes the APR. Exercise caution to not get soap and water around/in the inlet valve.
4. Roll patient onto one side, preferably the side opposite the canister side of the APR, and sponge the entire posterior side of the body with soap and water. Rinse the posterior body with cold water from the hose.
5. Capture all soap and rinse water in the decontamination capture bin for disposal.
6. When the decontamination process is complete, notify safety control and request air monitoring to begin sampling the medical room again.
7. The paramedics monitor the patient with an ICAM or LCD (if available), and M8 detector paper to verify that the patient has been successfully decontaminated.
8. If the medical room or the patient is determined to be contaminated, repeat procedures
VII.F.1-7.
FOR OFFICIAL USE ONLY 3
9. If the patient and medical room are determined to be clean, the paramedics may remove their PPE, as well as the APR from the patient.
G. If the decontamination procedures are utilized in the training building medical room, perform the following procedures to remove the decontamination run-off capture bin from the medical room:
1. When Safety Control notifies the paramedics that air monitoring technicians have cleared the training building medical room, the Lead Paramedic, or most senior paramedic on site, advises Safety Control that he/she requires two (2) support team members to remain in the anteroom to receive the capture bin.
2. Once the patient has left the training building medical room, a minimum of one (1) paramedic, two (2) if required for training support, remains in PPE in the medical room, replaces the capture bin, opens the door to the anteroom and passes the capture bin to the support team staff for decontamination.
3. Once the anteroom door is closed, the paramedic(s) notifies Safety Control to have air monitoring technicians sample the training building medical room IAW CDP
COBRATF 8.6. Paramedics remain in PPE until air monitoring technicians advise that the medical room is clear.
4. After the capture bin is decontaminated, support team members place it in hot laundry for monitoring as “Non-Expendable Equipment” IAW CDP COBRATF 1.18.
5. Once considered cleared for removal from hot laundry, the capture bin is returned to the paramedics for use in the training building medical room.
