Re: Asbestos and Lead-Based Paint Roof Survey
Altus Air Force Base 205 South 1st Street; DECA Commissary Altus, Oklahoma 73523
Dear Mr. Ollinger:
Professional Service Industries, Inc. (PSI) is pleased to inform you of our findings for the above referenced project. The project encompassed the roof of DECA Commissary and included surveys for suspect asbestos-containing materials (ACM) and suspect lead-based paint (LBP), with the site visit being conducted on January 19, 2017. PSI’s Mr. James T. Russell, an U.S. Environmental Protection Agency (EPA) accredited and Oklahoma licensed inspector, performed the survey.
BUILDING DESCRIPTION
The building included in this renovation survey was the DECA commissary at Altus Air Force Base. The building encompassed approximately 64,650 square feet of total area. There is approximately 61,400 square feet of built-up tar and gravel roof over insulation and metal deck and approximately 3,250 square feet of standing seam metal roof over wood decking.
ASBESTOS SURVEY
The asbestos survey was conducted to assist the client in complying with requirements of 40 CFR Part 61, the National Emission Standards for Hazardous Air Pollutants (NESHAP) and the Oklahoma Department of Labor. The asbestos survey included both friable and non-friable suspect ACM. Friable ACM is defined by the EPA as any material that when dry, can be crumbled, pulverized, or reduced to powder by hand pressure. The EPA and the Oklahoma Department of Labor define ACM as any material that contains greater than one-percent asbestos. A total of nine (9) bulk material samples were collected and analyzed by the EPA recommended Polarized Light Microscopy (PLM) with dispersion staining method. The following materials were sampled as suspect ACM at the facility:

A material is considered by the EPA to be asbestos-containing if at least one sample collected from an area shows asbestos present in an amount greater than one percent (1%). ACMs were identified on the roof at the subject property.
1. Roof tar flashing at penetrations, expansion joints and perimeter parapet walls.
LEAD PAINT SURVEY
The lead paint survey was conducted in general accordance with the U. S. Department of Housing and Urban Developments (HUD) Publication: Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing, June 1995. The paint was tested using paint chip sampling. A total of four
(4) paint chip samples were collected for analysis by the recommended Flame Atomic Absorption Spectrometry analytical method (SW846 Method 7420). The following painted components were sampled:
Component and Sample Location
Color/Substrate Paint Chip (%) Paint Condition
LB-01 North roof hatch Brown/Metal <0.015% Poor
LB-02 North roof ladder Brown/Metal <0.014% Poor
LB-03 Middle Roof HVAC Framing Grey/Metal <0.015% Poor
LB-04 North Roof HVAC Framing Grey/Metal <0.024% Good
Laboratory test results of 0.5% by weight or greater are considered positive under the HUD guidelines and the EPA. None of the analytical results for the paint chip samples were at or above this definition for lead-based paint.
WARRANTY
PSI warrants that the findings contained herein have been prepared in general accordance with the standard of care exercised within the asbestos and/or LBP testing and inspection industries. PSI recognizes that raw laboratory test data are not usually sufficient to make all abatement and management decisions.
The survey included inspection of reasonably accessible materials such as above or behind suspended ceilings or other non-permanent structures. PSI did not however inspect or sample inaccessible areas such as behind walls or within ductwork and PSI did not dismantle any part of the structure to survey inaccessible areas. Inaccessible is defined as areas of the building that could not be tested (sampled) without destruction of or damage to the structure or a portion of the structure.
The information contained in this report is based upon the data furnished by the Client and observations and test results provided by PSI. These observations and results are time dependent, are subject to changing site conditions, and revisions to Federal, state and local regulations.
PSI did not provide any service that was designed or intended to prevent or lower the risk of the

Report Notes: (PT) Point Count Results
Quantitation is based on a visual estimation of the relative area of bulk sample components, unless otherwise noted in the "Comments" section of this report. The results are valid only for the item tested. This report may not be used to claim product endorsement by NVLAP or any agency of the U.S. Government. Method used: E.P.A.
Method for the Determination of Asbestos in Bulk Building Materials (EPA / 600/R-93/116 July 1993). Polarized Light Microscopy is not consistently reliable in detecting asbestos in floor coverings and similar non-friable organically bound materials. Quantitative Transmission Electron Microscopy is currently the only method that can be used to determine if the material can be considered or treated as non-asbestos containing. Samples will be disposed of within 30 days unless notified in writing by the client. No part of this report may reproduced, except in full, without written permission of the laboratory. The reporting limit is 1% by weight. NVLAP Lab Code 101350-0.
