QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1. Contract or Task Order Title: Security System Maintenance at DECC PAC

2. Work Requirements:

6.1. Task 1: Preventative Maintenance Services

6.2. Task 2: Emergency Maintenance Services (24x7)

6.3 Task 3: Unscheduled Maintenance and Repair Service Calls

6.4. Task 4: Realignment, Additions and Modification to Existing Equipment

3. Primary Method of Surveillance:

The method of surveillance varies by task with 100 percent inspection for critical emergency repairs, service reports, and AQ forms; random sampling for routine on-going maintenance, and periodic inspection for infrequent tasks, to be recorded at frequencies defined by the deliverables section of the PWS.

· 100 Percent Inspection. This is usually only the most appropriate method for infrequent tasks or tasks with stringent performance requirements, e.g., where safety or health is a concern. With this method, performance is inspected/evaluated at each occurrence. One hundred percent inspection is too expensive to be used in most cases.

· Random Sampling. This is usually the most appropriate method for recurring tasks. With random sampling, services are sampled to determine if the level of performance is acceptable. Random sampling works best when the number of instances of the services being performed is very large and a statistically valid sample can be obtained. Computer programs may be available to assist in establishing sampling procedures.

· Periodic Inspection: These services are monitored weekly, monthly, quarterly, semiannually, annually, etc. Periodic types of activities are perfect for periodic inspection because not only are they infrequent, but there is normally a predetermined, specified time frame within which the tasks must be accomplished.

The facility manager or alternate facility representative should know when Contractor personnel are performing services in support of the facility, whether routine, emergency, or in support of a Technical Requirement (TR). The Contractor personnel shall sign in to the supported facility in accordance with established security and visitor protocols, and shall physically notify the facility manager or alternate facility representative of their arrival and departure. As a QA representative for the Government, the facility manager may, for any service visit, request the Contractor personnel to demonstrate what work was accomplished during the visit, discuss the condition of the serviced equipment, and/or inspect with the Contractor personnel the services rendered for compliance with guidance and instructions for the specific services. All demonstration/inspections shall be documented with a signed QA (CD521-2178) form. (Security escort personnel are not authorized and may not be delegated to inspect or sign as complete work accomplished under this contract.)

4. Scope of Performance:

An overview of the work to be provided under this contract follows. The Contractor shall:

· Provide scheduled preventative maintenance services to the integrated access control system (ACS), intrusion detection system (IDS), and closed circuit television (CCTV) system.

· Provide emergency repair/sustainment services, as necessary, for the integrated security system.

· Provide response to unscheduled service calls of minor maintenance and repair (non-emergency) which may occur between scheduled preventative maintenance visits.

· Provide services to realignment, additions, and modification, of components to the security system as required during the lifetime of the contract.

· Document the system layout and connectivity, and provide complete Configuration Management of the system(s) through the life of the contract.

· Ensure that the system meets all security and safety guidance and requirements of section 12 and 14 of this contract.

Analog layout plans of the facilities indicating the layout of existing security equipment are available solely to bidding vendors through the contracting office. These informational layout diagrams are restricted for security reasons and will not be disseminated through other channels. Digital (CAD) layout plans of the facilities, generally without existing security equipment and communications layout information, will be provided at time of award to the successful bidder.

Associated equipment and hardware such as electronic door strikes, magnetic locks, panic/crash bars, door hardware (including hinges, handle sets, lock sets, power openers, & latch guard plates), X-09 (and similar) high-security locksets, strobe lights & switches, legacy emergency backup batteries, controller units and enclosures, and control & power wiring for the system is also present and included in the scope of this agreement. The Contractor will be responsible for maintenance and repair of these additional components. Doors in the facility that do not have security devices on them are excluded from the scope (although they may be added to the scope by the addition of security system equipment).

The Contractor is responsible for surveying the existing building and security systems to confirm the number, location, and type of installed equipment and the support requirements for each. All new conductors (power and data) are required to be installed into conduit (not free-run). Where existing (legacy) equipment is used, retrofit of existing conductors into conduit is not required until that legacy equipment is replaced. Proposals and designs shall explicitly indicate where legacy free-run conductors (power and data) are located. Where legacy equipment is replaced, conductors shall be replaced and installed into conduit (not free-run).
