2. Contractors are requested to submit a written technical and price quotation in response to the attached Performance Work Statement (PWS) (Attachment 1) and Quality Assurance Surveillance Plan (QASP) (Attachment 2).

In accordance with DISA Acquisition Regulation Supplement (DARS) 52.209-9000, each contractor shall specifically identify in its quotation, whether or not any potential or actual Organizational and Consultant Conflicts of Interest (OCCI) as described in Federal Acquisition Regulation (FAR) Subpart 9.5 exists for this procurement. If the contractor believes that no OCCI exists, the OCCI response shall set forth sufficient details to support such a position. If a contractor believes that an actual or perceived OCCI does exist on this procurement, the contractor shall submit an OCCI plan with the quotation, explaining in detail how the OCCI will be mitigated and/or avoided.

3. Evaluation Factors and Basis for Award Determination:

a. Award will be made to the contractor whose quotation is determined to represent the overall best value to the Government using an LPTA evaluation process. Quotations will be evaluated with regard to how they respond to the requirements of the PWS, on records of past performance, and on price submissions. The technical portion of the quotation should include all information the contractor wants the Government to consider and evaluate regarding its company’s ability to perform all required tasks and conform to all required terms and conditions. In order to be eligible for award, the contractor’s quotation must sufficiently address all aspects of the PWS. With regard to the best value analysis for the award decision, the below evaluation factors and subfactors will be used in the determination of technical acceptability. Section 508 Accessibility Standards do apply to this requirement. Section 508 requirements are listed in the attached PWS. Compliance with Section 508 standards will be considered prior to evaluating any of the selection criteria listed below. Any quotation failing to meet the Section 508 Accessibility Standards, as listed in the PWS, shall be eliminated from consideration for award.

b. After receipt of quotations, the Government will first evaluate the lowest price quotation. If the lowest price quotation is determined to be technically acceptable and otherwise properly awardable, no further evaluations will be conducted, and award will be made. If, however, the lowest price quotation is determined to be technically unacceptable and/or otherwise not properly awardable, further evaluations will be conducted.

c. The standards for determining technical acceptability are below, as well as information concerning price quotations. Page limitations are identified below under each evaluation factor and will be treated as maximums. If exceeded, excess pages will not be read or considered in evaluation of the quotation. The following items will not be considered when calculating page limits: cover page, table of contents, glossary, executive summary, generic resumes, past performances, and OCCI mitigation plan. In the event any evaluation notices are issued, page limitations may also be placed on those. The technical portion of the quotation is to be submitted as a Microsoft (MS) Word 2007 or PDF document. The page format shall have a 1 inch margin using no smaller than an 11 point Ariel, Times New Roman, or Courier New font for all text to include any text contained in a table. The price quotation spreadsheet shall be a separate document from the technical quotation and shall be submitted using the attached Excel Bid Format Spreadsheet (Attachment 5). All formulas shall remain present in the spreadsheet for verification. The file size of each document shall not exceed 2MB. Page limitations shall not be circumvented by including inserted text boxes/pop-ups or internet links to additional information; such inclusions are not acceptable and will not be considered part of a quotation. The Government reserves the right not to consider for award any quotation that does not adhere to the administrative requirements of this RFQ.

(1) Technical/Management Approach Factor: The Technical/Management Subfactors will be evaluated and rated either Acceptable or Unacceptable. In order for a quotation to be rated overall Technically Acceptable, each of the following subfactors must be rated Acceptable. Technical/ management quotations should not exceed 15 pages.

1. Subfactor 1: Preventative Maintenance (PWS Paragraph 6.1) In order to be rated acceptable the contractor shall address and/or demonstrate in the proposal how the performance of the semi-annual preventative maintenance of the security system is performed, what is included in the preventative maintenance, how the semiannual database clean, backup and storage will be conducted, all in accordance with (IAW) the PWS.

(b) Subfactor 2: Certified Technician (PWS 6 & 6.1.8.3) In order to be rated acceptable the contractor shall address and/or demonstrate in the proposal how contractor personnel assigned under this contract are/will be certified by the security system software manufacturer.

(c) Subfactor 3: Software/Firmware Updates (PWS 6.1.8) In order to be rated acceptable the contractor shall address and/or demonstrate in the proposal how performance of the Software/Firmware Updates and implementation of the Security Technical Implementation Guide will be conducted IAW the PWS.

(d) Subfactor 4: Unscheduled (Emergency) Maintenance & Repair (24 x 7) (PWS 6.2) In order to be rated acceptable the proposal shall address and/or demonstrate how the Emergency Maintenance Services will be conducted IAW the PWS.

(2) Past Performance Factor: Past performance is based on acceptable/unacceptable and will be considered only after the technical proposal/quotation is determined acceptable.

Offerors will be evaluated on performance for recentcy and relevancy. Past performance efforts shall be within the past 3 years. No more than 3 efforts shall be submitted (Attachment 3). Recent efforts are those which have been performed during any portion of the last three years, including those efforts currently being performed. References other than those identified by offerors may be contacted by the Government, and the information received may be used in the past performance evaluation.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.

RELEVANCY DEFINITIONS

Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.

Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.

Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.

Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.

Recentcy Definition – Performance during the Last 3 years

3) Price Factor:

(a) Price quotations will be evaluated to determine if they are reasonable and complete. The Government reserves the right, but is not obligated, to review prices to determine whether they are so low that they reflect a lack of technical understanding, and to reject a quotation for offering unrealistically low prices. The contractor shall submit pricing for the base period and all option periods, including option pricing for an additional 6-month period that may be authorized IAW FAR 52.217-8. See FAR 52.217-8 which authorizes the Government to require continued performance of any services within the limits and at the rates specified in the contract. The pricing quotation should include a separate line item for the additional 6-month period IAW FAR 52.217-8. These prices shall be identical to the quoted pricing in the 6 months prior to expiration of the base period, or the final option period, if option periods are present. The total evaluated price will consist of the contractor’s quoted price for the base period, all option periods, and the option pricing for the additional 6-month period.

(b) Contractors should ensure price quotations include detailed information regarding the resources required to accomplish the task (e.g., labor categories, labor hours, rates, travel, incidental equipment, contract access fees, etc.). CLIN 0002 Emergency Maintenance - emergency services will always be at the FFP rate negotiated at time of award regardless of what trade is used Note: The SubCLINs awarded under the CLIN 0003 for Service Calls shall be priced as firm fixed. Each SubCLIN will provide for service calls within the level and each service call will be paid the awarded FFP, no matter what the actual proposed cost are. Attached to this RFQ is historical data for service calls placed on this system (Attachment 6). The levels have been determined based on this data and are as follows:

Level I - $0- $300 - $195 is the FFP paid with an estimated 4 calls per year Level II $301 - $600 - $490 is the FFP paid with an estimated 4 calls per year Level III $601 - $900 - $790 is the FFP paid with an estimated 4 calls per year

As the need occurs during the contract period, the Contracting Officer Representative (COR) will relay to the contractor work required that falls under the scope of this service call CLIN. The contractor will submit a proposal, covering all costs, which the COR will evaluate and if accepted, determine which level covers the call. The COR shall not request a service call nor shall the contractor accept a service call that exceeds the contracted/funded amount per SubCLIN. However, if at the end of the contract period the full quantity of calls has not actually been placed, a modification will be issued to reduce the quantity and pay for only the calls performed.

c) Each CLIN (for the base year and each option years) will be evaluated as follows; the following method is for EVALUATION PURPOSES ONLY:

• CLIN0001: the contractor shall provide a monthly preventative maintenance cost which will be multiplied by 12 to establish an annual cost for the base year and each option year. This CLIN will be evaluated at 12 months (the annual total) for the base year and each option year. (FFP)

• CLIN0002: the contractor shall provide a FFP hourly rate(s) for each labor rate category identified in Attachment 4. This CLIN will be evaluated at one hour for each of the listed labor rate(s) categories for the base year and each option year. (TM CLIN)

• CLIN0003: this CLIN will not be evaluated

• CLIN0004: A scenario has been provided to evaluate CLIN 0004 (Attachment 7). The scenario provides detailed work effort and will be evaluated by the fully burdened hourly rates prepared by the contractor in response to the scenario for the base year and each option year. (FFP) Although only the fully burdened labor rate will be evaluated as part of the cost evaluation, the contractor shall prepare a technical and cost proposal in response to the technical requirement scenario provided at attachment 7.

Again, the price quotation spreadsheet (Attachment 4) shall be a separate document from the technical/management quotation.

4. The Government intends to evaluate quotations and award a contract without exchanges with contractors. However, the Government reserves the right to conduct exchanges or seek clarifications if the contracting officer (KO) determines they are necessary. The Government further reserves the right to reduce the number of quotations being competitively considered to the greatest number that will permit efficient competition among the most highly rated quotations. If a contractor’s quotation is removed from further consideration for award, written notice of the removal will be provided to the contractor. In the event issues pertaining to a proposed task cannot be resolved to the KO's satisfaction, the Government reserves the right to withdraw and cancel the proposed order. In such event, contractors will be notified in writing.

5. In response to this RFQ, no terms, conditions, or assumptions will be entertained. If terms, conditions, or assumptions are included in a quotation, the quotation may be excluded on grounds that it failed to comply with the RFQ’s instructions.

6. All quotations received in response to this RFQ shall remain valid for 90 calendar days from the date of receipt of quotations. Further, should the contractor be awarded this contract, the contractor will furnish the items and/or services identified in its quotation at the price quoted to the designated point(s) within the time specified in the schedule.

7. A Site Visit will be held on Tuesday, August 5, 2014, at 9:00 AM (Hawaii-Aleutian Time Zone). Offerors are urged to attend the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable.

CONTRACT/ORDER CLOSEOUT—FIXED-PRICE, TIME-AND-MATERIALS, OR

LABOR-HOURS (JAN 2007)

Timely contract closeout is a priority under this contract/order. The Contractor shall submit a final invoice within ninety (90) calendar days after the expiration of this contract/order, unless the Contractor requests and is granted an extension by the Contracting Officer, in writing. In addition, and concurrent with the submission of the final invoice, the Contractor shall notify the Contracting Officer of the amount of excess funds that can be deobligated from this contract/order so the closeout process can begin as soon as possible upon expiration of this contract/order. A Contractor’s failure to respond and/or sign the bilateral closeout modification within thirty (30) calendar days of receipt will constitute approval of the terms of the modification and the modification will subsequently be processed unilaterally by the Contracting Officer to deobligate excess funds and close this contract/order. If this contract/order contains option periods, the Contractor is required to submit an invoice within ninety (90) calendar days after expiration of the base period of performance and the expiration of each exercised option period of performance to allow for deobligation of excess funds that were obligated in those respective periods of performance.
