About this file
This is a Quality Assurance Surveillance Plan (QASP) for the Special Operations Medical Training Services contract. The QASP establishes the Government's framework for monitoring contractor performance across three contract line items: CLIN X00101 (Prolonged Field Care Training), CLIN X00102 (Austere Medical Training for Non-Medical Personnel), and CLIN X00103 (Custom Medical Training). The document does not create requirements beyond those stated in the contract or Performance Work Statement (PWS), and in case of conflicts, the contract and PWS control.
The QASP identifies eight performance objectives with corresponding acceptable quality levels and surveillance methods. Performance Objective 1 requires 100 percent on-time submission of all planning materials, course materials, schedules, rosters, training records, assessments, certificates, and After Action Reports. Performance Objective 2 mandates verification that 100 percent of required personnel meet qualifications before performing assigned duties. Performance Objective 3 requires 100 percent availability of facilities, equipment, supplies, and safety measures before training begins. Performance Objectives 4 through 8 address specific training execution requirements: CLIN X00101 must achieve 100 percent compliance with ordered capacity tier and staffing; optional cadaveric or real-tissue training add-ons must maintain required student-to-specimen ratios; CLIN X00102 must reserve all Government-ordered MARSOC student seats and maintain a minimum 90 percent first-time National Registry of Emergency Medical Technicians (NREMT) EMT examination pass rate for each cohort; and CLIN X00103 must execute each task order in 100 percent compliance with defined scope and requirements. Surveillance methods include direct observation, document review, facility inspection, personnel qualification verification, pre-event readiness reviews, and student feedback review. The Contracting Officer's Representative (COR) maintains detailed surveillance records and may issue Contract Discrepancy Reports (CDRs) when deficiencies are identified. Contractors must submit Corrective Action Plans within ten business days of receiving a CDR, identifying root causes, immediate and preventive actions, responsible officials, and completion dates.

QUALITY ASSURANCE SURVEILLANCE PLAN
SPECIAL OPERATIONS MEDICAL TRAINING SERVICES
1.0 PURPOSE
This Quality Assurance Surveillance Plan (QASP) establishes the Government’s approach for monitoring contractor performance under the Special Operations Medical Training Services contract.
This QASP provides a framework for assessing contractor compliance with the Performance Work Statement (PWS), applicable task orders, and other contract requirements. It identifies key performance objectives, performance standards, acceptable quality levels, surveillance methods, surveillance frequency, and surveillance records.
This QASP does not create requirements beyond those stated in the contract, PWS, applicable task orders, or other contract documents. In the event of a conflict between this QASP and the contract or PWS, the contract and PWS control.
2.0 SCOPE
The Government will use this QASP to surveil contractor performance under:
1. CLIN X00101 – Prolonged Field Care Training;
2. CLIN X00102 – Austere Medical Training for Non-Medical Personnel;
3. CLIN X00103 – Custom Medical Training; and
4. Applicable contract-wide requirements.
Government surveillance may include direct observation, document review, facility inspection, verification of personnel qualifications, review of contractor quality-control records, review of student feedback, review of training records and deliverables, and other reasonable methods necessary to assess contractor compliance with the PWS and applicable task orders.
Government surveillance does not relieve the contractor of responsibility for internal quality control, safety, personnel qualifications, deliverable timeliness, or successful performance.
3.0 ROLES AND RESPONSIBILITIES
3.1 Contracting Officer
The Contracting Officer is responsible for overall contract administration and is the only Government official authorized to modify contract terms, task-order scope, price, period of performance, or other contractual requirements.
The Contracting Officer may issue written notices of deficiency, Contract Discrepancy Reports (CDRs), cure notices, or other contractual communications, as appropriate.
3.2 Contracting Officer’s Representative
The Contracting Officer’s Representative (COR) is responsible for technical surveillance of contractor performance within the scope of the COR appointment.
The COR may monitor contractor performance; review deliverables; verify personnel qualifications; conduct or coordinate facility and training-area inspections; observe training execution; review contractor quality-control records; document deficiencies;
recommend issuance of CDRs or corrective action; and coordinate with Government subject matter experts, as appropriate.
The COR shall not direct work that changes contract scope, price, period of performance, or other contractual terms.
3.3 Government Subject Matter Experts
Government subject matter experts may assist the COR by observing training, reviewing curriculum and assessment materials, evaluating technical performance, validating scenario realism, and providing technical feedback.
Government subject matter experts do not have authority to modify the contract or issue direction that changes contractual requirements.
3.4 Contractor
The contractor is responsible for maintaining an internal quality-control process sufficient to ensure that personnel, facilities, equipment, supplies, logistics, curriculum, training materials, assessments, and deliverables comply with the PWS and applicable task orders.
The contractor shall correct deficiencies identified through its own quality-control process or Government surveillance in accordance with the PWS, applicable task order, or written direction from the Contracting Officer.
4.0 SURVEILLANCE METHODS
The Government may use one or more of the following surveillance methods:
4.1 One Hundred Percent Inspection
One hundred percent inspection consists of reviewing every required item, record, deliverable, qualification package, or result associated with a defined performance requirement.
4.2 Periodic Inspection
Periodic inspection consists of inspecting selected aspects of performance at intervals determined by the COR, based on risk, performance history, training schedule, and Government requirements.
4.3 Direct Observation
Direct observation consists of observing classroom instruction, practical exercises, scenario-based training, facilities, logistics, role-player support, medical simulation, and other contractor performance.
4.4 Document Review
Document review consists of reviewing plans, rosters, résumés, certifications, licenses, course materials, schedules, assessments, training records, reports, and other documentation necessary to verify compliance.
4.5 Pre-Event Readiness Review
A pre-event readiness review consists of reviewing and validating personnel, facilities, equipment, supplies, safety measures, logistics, student support, course materials, and other resources before a scheduled training event begins.
4.6 Student and Stakeholder Feedback Review
The Government may review student, instructor, role-player, and other stakeholder feedback to identify performance trends, strengths, deficiencies, or areas requiring additional surveillance.
