About this file
This document contains a past performance rating form for the OASIS Multiple Award Indefinite Delivery Indefinite Quantity contract. The form includes six performance rating criteria of quality of service, schedule, cost control, business relations, management of key personnel, and utilization of small business. Raters are to choose an adjectival rating of exceptional, very good, satisfactory, marginal, or unsatisfactory for each criterion and provide additional comments if a rating of marginal or unsatisfactory is selected. The form also includes definitions for each adjectival rating. It appears to be an attachment to the OASIS solicitation for evaluating offerors' past performance on relevant contracts.
