About this file

The document is a Vendor Certification Form (Form PUR 7801) for the state of Florida, addressing vendor compliance with various state legal requirements. The form requires vendors to certify their status regarding multiple state statutes, including compliance with Israel boycott restrictions, scrutinized companies lists, vendor eligibility, and employment verification through E-Verify.

Key certifications include confirming the vendor is not on suspended or prohibited vendor lists, has not been involved in discriminatory practices, uses E-Verify for new employees, and complies with state disclosure requirements. The form also requires vendors to attest to not using coercive labor practices and provides specific compliance checkpoints for contracts involving personal identifying information, common carrier services, and contracts valued at $1 million or more. Vendors must sign the document, providing their Federal Employer Identification Number (FEIN) and authorized representative's details to validate their compliance with Florida state procurement regulations.
