About this file

This document is a Terms and Conditions agreement for a Blanket Purchase Agreement (BPA) establishing a Law Enforcement Officers Safety Act (LEOSA) Credentialing Program. The contractor shall provide all necessary personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items required to perform verification and issuance of LEOSA credentials to qualified active duty United States Air Force (USAF) and United States Space Force (USSF) personnel as detailed in the accompanying Performance Work Statement (PWS). The BPA has a maximum value of $1,000,000.00, with individual calls limited to $25,000.00 per authorized caller, to be paid via Government Purchase Card (GPC).

Prior to the annual anniversary of the BPA, the contractor must furnish an approved price list to the Contracting Officer specifying prices, discount terms, and effective periods; no calls will be accepted against the BPA until this price list receives approval. Authorized callers are identified in a Contracting Officer-signed authorization letter that specifies individuals by title, name, office symbol, and dollar limitation per purchase. Delivery tickets must include the BPA number, date of call/purchase, invoice number, quantity, unit price, and name of the individual placing the call. Either party may cancel the entire agreement with thirty (30) days written notice. The agreement shall be reviewed annually before its anniversary date and revised as necessary to conform to statutory, Executive Order, Federal Acquisition Regulation (FAR), and supplemental requirements.
