PAST PERFORMANCE SURVEY for:

Simplified Acquisition of Base Engineering Requirements (SABER)

Offerors: Complete Sections 1 and 2 of this survey, and then forward the survey to references for completion and submission to the Government. Only provide surveys for those contracts defined as RELEVANT or MORE RELEVANT per the solicitation. In your proposal submission (Part I), include copies of Sections 1 or 2 that were sent to references so the Government is aware of what organizations were sent surveys.

References: You have been provided as a reference in regards to the Simplified Acquisition of Base Engineering Requirements (SABER) source selection at the Air Force Research Laboratory, Rome Research Site, Rome, NY. GGREEN/SATISFACTORY - Performance met/meets contractual requirements. The performance of the element being assessed contained/contains some minor problems for which corrective actions taken by the contractor were satisfactory.
YYELLOW/MARGINAL - Performance did not/does not meet some contractual requirements. The performance of the element being assessed reflects a serious problem for which the contractor has not/did not identify corrective actions.
RRED/UNSATISFACTORY – Performance did not/does not meet some contractual requirements. The performance of the element being assessed reflects a serious problem(s) for which the contractor’s actions were ineffective.
