Statement of Work

Replace Roof on Building 49

PART 1 – GENERAL

1.01 STATEMENT OF WORK: This is a general overview of the project:

A. Location: Accomplish work at Robins AFB, Georgia

B. Bid Proposal - Construction Specification Institute (CSI) format shall be used for the purpose of this IDIQ contract. Required divisions at a minimum include engineering, materials, labor (prime and subcontractor), equipment, bond, G&A, overhead, profit. Contractor bid proposals should include accurate applicable line items, quantities, unit price, total price, and a written, flowable work plan authored in sequence that includes all elements (e.g. materials and labor) required to install a complete roofing system. It must meet all requirements listed in the Statement of Work (SOW) and contract specifications. All Task Orders (TO) shall be supported by necessary documentation to indicate that adequate engineering and planning to accomplish the requirements specified in the TO SOW and contract specifications. Examples of acceptable documentation include: drawings, calculations, catalog cut sheets, specifications, or architectural renderings.TO proposals will be evaluated on all submitted documentation. Proposal line items, quantities, documentation, contract specifications, and the TO SOW will become a part of the TO requirements upon award of each TO. All offered betterments will become part of the task order requirements. A "Betterment" is defined as any component, system or quantities that exceed the minimum requirements stated in the SOW. This includes all warranties that traditionally accompany an identified betterment. Evaluation of all work plan elements is on a go/no-go basis. The Government reserves the right to award without discussions.

C. Performance Period: This project is critical to the F-15 mission and is designed to be worked 6 days a week and 10 hours a day to expedite completion.

1. Buildings 49: The contractor shall have all submittals (except As-built drawings and O&M Manuals) submitted and approved within 45 days of Notice to Proceed

(NTP). Contractor shall begin work within 75 days of NTP and be completed with project within 216 days of NTP. This is based upon 120 calendar days of onsite work and 21 days for close-out.

D. Liquidated Damages will be applicable for this task order. If the contractor fails to complete the work within the time specified in the contract, the Contractor shall pay Liquidated Damages to the Government in the amount of $325.92 for the first day of delay, and $263.32 per day for each subsequent calendar day of delay until the work is completed or accepted.

E. Project Scope, Objectives and Criteria: Provide all labor, material, plant, equipment, supplies, testing and coordination and supervision as follows:

Contractor shall be responsible maintaining the building in waterproof state during construction and protection of all personnel and government assets at building 49 from any falling objects or debris. A full height stair tower shall be provided for access to the high bay area for construction access and inspections. Remove existing flashings, lightning protection, gutter, and downspouts. Install new flute filler insulation and vapor retarder. Install new retrofit framing in sequence identified in attached drawings. Time line for government evacuation, framing installation, and heavy metal cleaning in each section will be as follows: Dock 1 - 50 day’s evacuation to install all framing & heavy metal cleaning, 7 days turn around for access to Dock 2 - 50 day’s evacuation to install all framing and heavy metal cleaning. During the evacuation period in each dock initial heavy metal samples will be taken, the entire dock cleaned of heavy metals, clean samples taken and test results provided to the government prior to the dock being turned back over to the government. Potential heavy metals that could be identified in the hanger could include but not limited to Lead, Chromium VI, and Cadmium. Please see attached

WR-ALC SAFE-T-GRAM. The new retrofit framing shall be designed to remove the existing interior gutters. Section C on shall be raised to continue the same slope as section

D until it intersects the wall on sections A & B on building 49. Install new R-19 insulation and new 24 gauge industrial metal roof system with 3” minimum height trapezoidal mechanically seamed standing seam roof panels by an AC472 accredited manufacturer, 360° mechanical seam shall be seamed onsite (90°/180°/270° seams not allowed) with maximum panel width of 24 inches. Panels shall have intermediate low profile ribs, flat pans not allowed. Install all new flashings and gutters that cover the building to a minimum of the same elevation of the existing trims. Install new downspouts and new lightning protection. Roof warranty shall be a 20 year single source

NDL (no dollar limit) on material and labor warranty. Warranty shall cover weather-tightness, design wind speeds, and finish of the roofing and flashing components. Prior to acceptance, the manufacturer shall perform the necessary roof inspections required to determine that installation is per the manufacturer’s instructions and issue a letter to that effect. Roof shall not be accepted without this letter. All design shall be done by a professional engineer including verification that the existing structure will withstand the additional load of the new framing and roof panels. Engineer shall also design for required insulation and air flow required for retrofit framing. Design criteria: IBC 2015, ASCE 7-10, UFC 3-110-03, specification section 16670 and all other requirements in the basic roofing IDIQ. Roof manufacturer shall make a minimum of 2 visits during the project and provide a written report. Lightning protection system shall have a 3rd party inspection by either UL inspector or individual with AMMO 50 certification. All inspections require a 10 day written request to the COR and contraction officer. All other project requirements will be is accordance with requirements of the basic roofing IDIQ contract.

F. Project Requirements:

1. The contractor shall provide the technical knowledge and skill necessary to perform services to meet standards set forth in this Statement of Work for base facilities, equipment and systems and at a minimum meet manufacturer’s requirements.

2. All stored equipment shall be stored and protected in accordance with manufacturer’s recommendations.

3. This project will require daily reports to be filled out daily and submitted to the project manager on a weekly basis. Daily reports at a minimum shall contain the following information, Amount and type of work accomplished

Materials delivered to the site

Weather conditions and impact to work and schedule

Labor workforces on site

Construction equipment on site

Safety inspection performed and violations noted

Any outstanding issues, i.e. delays, RFIs, etc.

4. All items shall be installed in accordance with the manufacturer’s installation instructions unless the Base Facility Standards, specifications, drawings, contract clauses, schedule, attachments, manufacturer’s requirements, and other contract documents are more stringent. The most stringent requirement shall be met at no additional cost to the Government.

If meeting the more stringent requirement will void the manufacturer’s warranty the contractor shall notify the Contracting Officer and the inspector prior to voiding the warranty.

5. Coordination shall be required between different disciplines.

G. Evacuation: Before fencing off the construction area, notify the Contracting Officer in writing 30 days before ready to start so that Contracting Officer’s Representative and Project

Manager can notify appropriate personnel.

H. Physical Security:

1. The contractor shall be responsible for safeguarding all government personnel and property being modified or adjacent to the new work. The contractor shall identify any existing problems in writing prior to start work.

2. At the close of each work day, all government facilities, equipment, and materials shall be secured and back in operation unless agreed on prior to the work starting. The contractor shall be responsible for controlling access into an area that has been turn over to them for construction.

I. Contractor Operations: Comply with Occupational Safety and Health Administration

(OSHA), Corps of Engineers Safety Manual (EM 385-1-1), the contract safety provisions, Air

Force Occupational Safety and Health (AFOSH) Standards, and National Fire Protection

Association (NFPA) standards.

1. Contractor Employees:

a. Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is the obligation of the contractor.

b. The contractor shall furnish to each of his/her employees a place of employment that is free from recognized hazards.

c. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks.

d. This contract shall in no way require persons to work in surroundings or under working conditions that are unsafe or dangerous to their health.

e. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property.

f. This requires providing personnel with protective equipment and associated safety equipment as may be necessary.

g. The contractor must also protect government personnel from hazards generated by the work.

2. Contractor Badges: All employees to enter Robins AFB shall have government provided badges. Contact the Project Manager/Badge Agent for the current badge application form. Badge request shall be submitted a minimum of 10 days prior to the employee’s arrival.

3. Fire Reporting:

a. Brief all workers and subcontractors as to the location of telephone and fire alarm pull stations.

b. Report all fires as soon as discovered. The fire reporting number for on Base phones is 911. The fire reporting number for off Base phones is 478-222-2900. The caller should give his or her name and location of what is on fire. Also, give any other information that may be requested by the Fire Department dispatcher. Stay on the telephone until the dispatcher has obtained all necessary information.

4. Work shall be performed to, but not be limited to, the requirements set forth by the following:

a. Robins AFB – Base Facility Standards (last edition)

b. Occupational Safety and Health Administration (OSHA)

c. All other applicable federal, state and local codes to ensure the system/s are installed in accordance with manufacturer’s specifications.

d. Air Force Occupational Safety and Health (AFOSH)

e. National Fire Protection Association

f. UFC 3-600-01 – Fire Protection Engineering for Facilities

g. The contractor shall perform all work to conform to the standards of the government listed in paragraph above and per manufacturer specifications/ requirements.

h. Equipment and materials provided by the contractor shall match/meet or exceed the specified equipment and material.

i. The contractor shall correct work not meeting these specifications at no additional expense to the government.

j. The contractor shall coordinate work between the different disciplines.

5. Permits & Utility Outages:

a. It shall be the contractor’s responsibility to attend permit and outage request meetings.

b. Before any “hot work” is performed on the job, an AF Form 592, USAF

Welding, Cutting, or Brazing Permit, must be obtained and displayed at the work site for the duration of the permit.

c. Permits can be obtained by contacting the Civil Engineer Construction

Inspector/Army Corp Project Engineer assigned to the job. The Fire Alarm Dispatch Center can be reached at 478-926-3487 or 478-926-3488.

d. Permits will be returned to the fire department, Bldg. 377 Fire Station Number

1, on the day they expire.

e. Adequate number and type of portable fire extinguishers, supplied by the contractor, will be as close to the work as possible. At least a 10 lb ABC type fire extinguisher shall be located at close proximity to the work being performed. In certain circumstances, one or two 20 lb ABC type fire extinguishers will be required for hazardous locations. The fire inspector issuing the welding/burning permit will determine the required level of protection at the time of permit issuance.

f. A digging permit will be required before any digging, driving of stakes or post starts.

g. Digging Permits are obtained on Monday at 0800 hours of each week at building 1555 break area. If Monday falls on a holiday, the permits will be issued on Tuesday following that Monday.

h. Utility Outages will be signed off on during the weekly outage meeting held on Wednesdays at 1000 hours in building 1555. Request utility outages in writing to the Base

Civil Engineer, with a copy to the Contracting Officer a minimum of 21 days before the proposed outage.

i. Outages shall be scheduled at the Government’s convenience and may be at times other than normal working hours. For example, the times may be on weekends or during the second and third shifts (including holidays, holiday weekends, and work curtailment days).

These are at no additional cost to the Government.

j. These hours are subject to change.

6. Superintendent/ Project Manager: A full time Superintendent/ Project Manager is not required for this delivery order. A Foreman/Superintendent who is a certified installer of the roofing materials manufacturer and works for the general contractor shall be onsite and on the roof at all times when roofing is being installed.

a. The Foreman/Superintendent can be a working Foreman/Superintendent. If the Foreman/Superintendent is not a working superintendent he must at all times be available to the Government and be totally aware all aspects of the project.

b. The contractor shall provide the Contracting Officer a letter designating a competent person as site Superintendent/ Project Manager and Certified

Foreman/Superintendent. The letter shall state that the individuals has full authority to act for the Contractor and will be the primary Point of Contact for the Government. The Government reserves the right to disapprove the proposed one or both individuals submitted.

c. The Government through the Contracting Officer or Technical Representative of the Contracting Officer (CEA Project Manager) reserves the right to shut down all Contractor site activities if the certified superintendent and or certified foreman is not on site on the roof at such times of a visit. Site work shall not resume until the certified superintendent or certified foreman has returned to the site. Several violations of this requirement are cause for termination for default.

7. Site, Maintenance, Clean-up and Restoration:

a. Maintain the work site in a neat, orderly and safe manner.

b. Remove scrap, waste and excess materials promptly. Provide signs, barricades and lights as required to protect base personnel.

c. Do not allow trash and debris to accumulate and become unsightly. Sweep up and collect in contractor-maintained disposal containers daily. Dispose of collected debris weekly as a minimum.

d. Restore the project site to its final condition as required by the contract as soon as possible.

8. Rejected Work: If work performed by the contractor or a subcontractor does not meet the plans, specifications, national standards, Base Standards, applicable codes, industry standards, or acceptable workmanship the contractor shall accomplish the rework at no additional cost to the government. Rework will be rescheduled for the next normal workday.

9. Equipment Damaged by Contractor: The contractor shall take before and after photographs of the work site. Prior to starting work the contractor shall photograph, annotate, and bring to the attention of the inspector any noticeable damage to equipment, ceilings, walls, doors and frames, furniture, floors etc. The location of the annotated and photographs needs to be well defined. If the damage is not documented then the contractor shall be responsible for the repairs. The contractor shall jointly verify the status of the fire alarm panel with the inspector prior to work starting. The contractor shall be responsible for the repair of any of the above damaged in the process of performing their work. The repairs shall utilize original manufacturer’s approved parts and comply with manufacturer’s specifications. When the contractor finishes the repair, the equipment shall function within acceptable manufacturer’s specifications. This shall be accomplished at no cost to the Government.

10. Contract Qualifications: The contractor shall ensure that all employees performing work under this contract have all applicable licenses and certification to work on the item/s in question. At a minimum the contractor shall hold all applicable license/s required by the state for the type of work being accomplished. Supporting documentations shall be provided if requested. At any time, work performed by a non-qualified person/s may be rejected as noncompliance and may not be acceptable.

1.02 CONTINUED OCCUPANCY:

A. Access: Building 49 shall remain in full operation during the entire duration of the project. The contractor will be given access to the project area.

B. The work area is not in a restricted area.

If in a restricted area add: The contractor can expect to have to receive special badges or be escorted to work in the restricted. Up to 30 minutes of delays at the beginning and end of the work day may occur.

C. Beneficial Occupancy: The Government reserves the right to take beneficial occupancy of parts of the project area before the total project completion date. This is not final acceptance, and identified deficiencies must still be corrected.

1.03 HOURS OF OPERATION:

A. Standard Work Hours: The contractor shall perform the services required under this contract during the following hours: MON – FRI, 0700 –1630, except federal holidays and as noted herein. The contractor may be allowed to work, with prior approval of the contracting officer representative, extended hours to ensure timely completion of work at no additional cost to the government. Permission to work extended/alternate hours shall be requested in writing a minimum of 5 days prior to when the alternate work hours are desired. The Government reserves the right to deny the contractor’s request.

B. Holidays: The contractor is not required to work on the following days without written approval: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence

Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. If the holiday falls on a Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.

Permission to work on holidays shall be requested in writing a minimum of 5 days prior to when the alternate work hours are desired. The Government reserves the right to deny the contractor’s request.

1.04 SUBMITTALS:

A. All materials listed below are required for each building listed in this delivery order:

1.05 GOVERNMENT FURNISHED PROPERTY: Will not be provided in this project.

1.06 GOVERNMENT REPRESENTATIVES:

A. The contractor shall not make any changes to or deviate from the requirements listed in the

SOW unless approved by the Contracting Officer.

B. The government project manager and inspector is (PM) for the contract is Terry Ross Stinson, 78 CEG/CENME, 478-327-2942.

SUBJECT: Update 2 Guidance for Cleaning Work Areas

ISSUE DATE: 3 January 14

In accordance with OSHA 29 CFR 1910.1025 (h)(2)(ii), 1910.1026(j)(2)(ii), and

1910.1027 (k)(5), industrial areas of any shop in buildings 44, 45, 46, 47, 48, 49 , 50, 54, 59, 81, 82, 83, 89, 91, 110, 125, 137, 140, 142, 149, 150, 158, 165, 169, 180, 189, 255, 321, 323, 350, 635, 640, 645, 670, 2316, 20031, 20036, and 20180 that have processes that are potential sources of Lead, Chromium VI, and/or Cadmium dust, or that handles parts/equipment previously exposed to this dust, are designated No Dry Sweeping areas.

In addition, adjacent areas such as break rooms, shop floor offices, PSC’s, WSSCs, restrooms, and locker rooms that may potentially be contaminated from dust that has migrated from the industrial area are also designated No Dry Sweeping areas. Any abrasive operation (sanding, grinding, buffing, cutting, trimming, grit blasting, filing, hand sanding, etc.) that may generate Lead, Chromium VI, or Cadmium dust are also designated No Dry Sweeping areas.

Compressed air and dry sweeping may be used in building 59D, the grit blast booth in building 169, building 137 bay 3 & 4, and the two grit blast booths in building 142 as long as the ventilation system is in operation and the proper PPE is used while cleaning.

Dust free cleaning methods: For gross contamination use a HEPA-filtered vacuum.

Other dust free methods include wet methods such as wet wipes, mops, and electrostatic

“swiffer-type” sweepers and are approved for cleaning contaminated surfaces.

Commercial floor scrubbers that use wet methods and do not visibly cause dust to become airborne are also approved for large floor areas.

These procedures are not applicable in administrative areas that are located outside of the production floor areas.

The WR-ALC Safety Office will conduct random visits to these areas to ensure compliance with this guidance and to assist with any questions or concerns expressed.

Additional facilities may be identified through future swipe sampling and inspections.
