Metal Plating, Section L 21 March 2018
1.0 PROGRAM STRUCTURE AND OBJECTIVES
1.1 Introduction / Purpose
The purpose of this acquisition is to create and implement a Multiple Award
Indefinite Delivery Indefinite Quantity Contract (MAC IDIQ) that provides metal plating to remanufacture existing (known) and future (unkown) assets in support of
309th Missile Maintenance Group (309 MMXG) and 309th Commodities
Maintenance Group (309th CMXG) programs. The resulting Metal Plating Basic
IDIQ contract(s) are specifically established to support the 309 MMXG and 309
CMXG.
1.2 Number of Contracts to be Awarded
The Government intends to award basic IDIQ contract(s) to “all qualified” offerors for the Metal Plating requirement.
1.3 MAC IDIQ Ordering and Performance Period
The basic IDIQ contract(s) will have an eight (8) year and six (6) months ordering period and a nine (9) year performance period in accordance with (IAW) Federal
Acquisition Regulation (FAR) 52.216-22. Individual delivery orders (DOs) will be competed under the ordering procedures established in the Basic IDIQ SOW.
1.4 Budget/Funding Information
Funding will be obligated for each individual DO awarded under the basic IDIQ contract(s). The Government reserves the right to cancel this solicitation, either before or after the Request for Proposal (RFP) closing date. In the event the
Government cancels this solicitation or determines not to award a contract, the
Government has no obligation to reimburse an offeror for any bid and proposal costs and/or any other costs incurred.
1.5 Principle Purpose of the Contract / Applicable Labor Laws
This solicitation and resulting contract(s), IAW FAR 22.1003-6, Repair Distinquished from Remanufacturing of Equipment, shall be subject to 41 U.S.C. Chapter 65, Contracts for Materials, Supplies, Articles, and Equipment Exceeding $15,000, rather than to the Service Contract Labor Standards statute.
1.6 Source Selection / Basis for Contract Award
This acquisition will award basic IDIQ contract(s) to “all qualified” offerors IAW
FAR 15 source selection procedures. See Section M of this solicitation for specific information on source selection procedures that will be conducted in support of this acquisition.
1.7 Discussions
It is the Government’s intent to award without discussions; therefore, it is imperative that offerors submit their best terms initially. However, if during the evaluation period, it is determined to be in the best interest of the Government to hold discussions, offeror responses to Evaluation Notices (ENs) and the Final Proposal
Revision (FPR) will be considered in making the source selection decision. If the offeror’s proposal has been evaluated as acceptable at the time discussions are closed, any changes or exceptions in the FPR are subject to evaluation and may introduce risk that the offeror’s proposal be determined unacceptable and ineligible for award.
2.0 GENERAL INSTRUCTIONS
2.1 Instructions to Offerors (ITO)
This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal. The offeror's proposal shall include all data and information requested by the ITO and shall be submitted in accordance with these instructions. The offeror shall be compliant with the requirments as stated in the basic IDIQ contract(s) Statement of Work (SOW), Contract Data Requirements Lists (CDRLs), and Model Contract/Solicitation. Non-conformance with the instructions provided in this ITO may result in an unfavorable proposal evaluation.
2.2 Include Sufficient Detail
The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal shall not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements.
Offerors shall assume that the Government has no prior knowledge of their facilities, capabilities and experience, and will base its evaluation on the information presented in the offeror's proposal.
2.3 Embellishments Not Desired
Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.
2.4 Proposal Acceptance and Validity Dates
The proposal acceptance period is specified in Section A (front page) of the model contract/solicitation as 240 calendar days. The offeror shall make a clear statement in
Section A of the Contract Documentation proposal volume (Volume III) that the proposal is valid until this date.
2.5 Disposition of Proposals
IAW FAR 4.803(a)(10), Contents of Contract Files, the Government will retain one
(1) copy of each offerors’ proposal. Unless the offeror requests otherwise, the
Government will destroy extra copies of such proposals.
3.0 GENERAL INFORMATION

3.2 Debriefing
The PCO, IAW FAR 15.503, will promptly notify offerors in writing of any decision to exclude them from the competition or they are an unsuccessful offeror, whereupon they may request and receive a debriefing. Offerors desiring debriefing shall make their request IAW the requirements of FAR 15.505 or 15.506, as applicable.
3.3 Discrepancies
If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the PCO in writing with supporting rationale as well as the remedies the offeror is asking the
PCO to consider as related to the omission or error. The offeror is reminded that the
Government reserves the right to award this effort based on the initial proposal, as received, without discussion. This reservation includes matters of additional or substitute pages of the initial proposal.
3.4 Electronic Reference Documents:
All referenced documents for this solicitation are available on the Federal Business
Opportunities (FedBizOpps) website at https://www.fbo.gov/ or through the PCO.
Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation.
3.5 Oral Presentation
Oral presentations WILL NOT be allowed.
3.6 Communications
Exchanges of source selection information (i.e. questions and answers) between
Government and offerors will be controlled by the PCO. Email may be used to transmit such information to and from offerors only if the email can be sent and received encrypted and includes “Source Selection Information – See FAR 2.101 &
3.104” in the Subject line. Otherwise, source selection information will be transmitted https://www.fbo.gov/ via direct mail. In order to facilitate the sending and receiving of encrypted emails, offerors must use MS Outlook email configured to support encryption or a different email product that is S/MIME compatible and configured to support encryption. If you intend to submit your source selection information via encrypted email, you will need to contact the PCO prior to that first submittal in order to exchange certificates used for encryption. To ensure the process is working correctly, send a test encrypted message first (without including any source selection information). The PCO may determine to conduct telephone exchanges when deemed appropriate by the
Government. See section 3.10 of this ITO for submittal/distribution of hard and electronic copies of proposals.
3.7 Amendments to Solicitation
If this Request for Proposal (RFP) is amended, all terms and conditions that are not amended remain unchanged and in full force and effect. Offerors shall acknowledge receipt of any amendment to this request by the date and time specified in the amendment(s) by signing and returning the front page of each amendment with its proposal.
3.8 Submission, Modification, Revision, and Withdrawal of Proposals
Offerors are responsible for submitting proposals, and any revisions, and modifications, or withdrawing proposals IAW FAR 15.208 and this solicitation’s requirements. Late proposals will only be considered IAW FAR 15.208.
3.9 Organization/Number of Copies/Page Limits
The offeror shall prepare the proposal as set forth in the Table 1 - Proposal
Organization. The titles and contents of the volumes shall be as defined in this table, all of which shall be within the required page limits and with the number of copies as specified. Each volume identified in the table shall be separately bound in a three-ring, loose-leaf binder. The contents of each proposal volume are described in the
Section L, ITO paragraph, as noted in the table below:
3.9.1 Page Limitation
Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and (for paper copies) will be either returned to the offeror or disposed of as soon as practicable.

Offerors will be notified of any page overage. Page limitations for Evaluation
Notice (EN) responses will be identified in the letters forwarding the ENs to the offerors. When both sides of a sheet display printed material, it shall be counted as 2 pages. Each page shall be counted except the following: any cover sheets, table of contents, tabs, and glossary of abbreviations and acronyms. Page 1 of volume is defined as the first page after the table of contents, tabs and glossaries of terms/abbreviations/acronyms. All pages shall be numbered. Sheets printed on both sides shall be numbered on both sides.
3.9.2 Page Size and Format
Page size shall be 8.5 x 11 inches, not including foldouts. Pages shall be typed with 1.5 line spacing. Except for the reproduced sections of the solicitation document, the text shall be Times New Roman and size shall be no less than 12 point. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least 1 inch margins on the top, bottom, and sides. Pages shall be numbered sequentially by volume. These page size and format restrictions shall apply to responses to ENs.
Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc.
These displays shall be uncomplicated, legible and shall not exceed 11 x 17 inches in size. Foldout pages shall fold entirely within the volume. Foldout pages shall only be used for large tables, charts, graphs, diagrams and schematics; not for pages of text. For tables, charts, graphs and figures, the text shall be Times
New Roman and size shall be no less than 10 point.
3.9.3 Cost or Pricing Related Data
FAR 15.304(c)(1) requires "price or cost to the Government shall be evaluated in every source selection (10 U.S.C. 2305(a)(3)(A) (ii) and 41 U.S.C.
3306(c)(1)(B))"; however, approval to deviate from FAR 15.304(c)(1) and waive price/cost as a source selection evaluation factor for the basic Metal Plating MAC
IDIQ contract(s) has been authorized. The qualifying offeror(s) is not required to disclose price/cost in response to the solicitation. However, qualifying offerors will still need to provide cost or price at the time of competition on the individual delivery orders.
3.9.4 Classified Information
This effort is expected to be UNCLASSIFIED; therefore, your response SHALL
NOT include any classified information.
3.9.5 Cross Referencing
Each volume shall be written on a stand-alone basis so that its contents may be evaluated without cross-referencing to other volumes of the proposal. Information required for proposal evaluation that is not found in its designated volume will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is not permitted.
3.9.6 Indexing
Each volume shall contain a detailed table of contents which provides page numbers and delineates the subparagraphs within that volume. Tab indexing shall be used to identify sections.
3.9.7 Glossary of Abbreviations and Acronyms
Each volume shall contain a glossary of all abreviations and acronyms used, with an explanation for each.
3.9.8 Binding and Labeling
Each volume of the proposal should be separately bound in a three-ring loose-leaf binder, which shall permit the volume to lie flat when open. Staples shall not be used. The outside of each binder shall include a cover sheet, clearly marked as to volume number, title, original or copy number, solicitation identification and the offeror's name. The same identifying data should be placed on the spine of each binder. All unclassified document binders shall have a color other than red or other applicable security designation colors. Apply all appropriate markings including those prescribed IAW FAR 52.215-1(e), Restriction on Disclosure and
Use of Data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor
Bid or Proposal Information and Source Selection Information.
3.9.9 Electronic Copies
The content and page size of electronic copies shall be identical to the hard copies. In the event there is a discrepancy, the hard copies will be used for evaluation. For electronic copies, indicate on each CD the solicitation #, copy #, and the offeror's name. Each volume shall be in a different directory on the CD.
Use separate files within each volume directory to permit rapid location of all portions, including subfactors, exhibits, annexes, and attachments, if any. The offeror shall submit Volumes I – III in electronic format, using read only CDs (i.e.
CD-R). Re-writable CDs (i.e. CD-RW) shall NOT be used. If files are compressed, the necessary decompression program must be included. The electronic copies of the proposal, as applicable, shall be submitted in a format readable by Microsoft (MS) Word 2013, MS Excel 2013, or in a “word searchable” PDF format. ONLY PDF formats shall be used for drawings, cut sheets, and related types of documents. Do not use PDF formats for Excel material. Word and Excel documents shall be editable and NOT password protected.
3.9.10 Distribution/Submission/Proposal Volumes
The "original" proposal shall be identified. Proposals shall be addressed to the

4.0 VOLUME I – FACTOR 1: TECHNICAL
4.1 General Instructions
The Techncial Proposal Volume should be clear, specific and complete. Legibility, clarity and coherence are very important. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirments specified in the RFP (including the basic
IDIQ SOW). It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation. Provide as specifically as possible the actual methodology you would use for accomplishing/satisfying these subfactors.
Exception: Sections 3.1.1 through 3.1.18 of the basic IDIQ SOW list many potential processes that might be included in any future delivery order. Contractors are not expected to have all capabilities but must show core capability as described in paragraph 4.1.1.
The offeror’s Technical proposal will be evaluated for award purposes by evaluating the following subfactors. Your responses to the factor will be evaluated in accordance with Section M of the solicitation.

4.1.1 Subfactor 1: Core Metal Plating Processes
The offeror’s proposal shall demonstrate/describe their technical approach and capability to satisfy Metal Plating Basic IDIQ SOW core sections/requirements
3.1.1 thru 3.1.4, 3.1.6, 3.1.7, 3.1.10, and 3.1.12. Offerors are not required to submit capabilities pertaining to the plating processes described in 3.1.5, but should be aware these processes may be required on individual Delivery
Orders.shall demonstrate the process for completing each core metal plating process.
4.1.2 Subfactor 2: Professional Certifications and/or Licenses
The offeror’s proposal shall demonstrate/describe their technical approach and capability to satisfy Metal Plating Basic IDIQ SOW section/requirement 3.2.2.
4.1.3 Subfactor 3: Transportation
The offeror’s proposal shall demonstrate/describe their technical approach and capability to satisfy Metal Plating Basic IDIQ SOW section/requirement 4.3.2.

5.0 VOLUME II – FACTOR 2: PAST PERFORMANCE
5.1 General Instructions
Offerors must use the Past Performance Information (PPI) Tool when developing their Past Performance Volume. See section 5.1.5 and Appendix I of this document for PPI Tool instructions.
Submit past performance information for your organization on a minimum of three
(3) and maximum of five (5) recent and relevant contracts with U.S. Government
Agencies and/or commercial sources.
If a joint venture is contemplated, provide complete information as to the joint venture. Include any relevant and recent past performance information on previous joint ventures with the same partner. If this is a first time joint venture, each party to the joint venture shall provide a list of past relevant contracts.
The Government reserves the right to use both the information provided in the offeror’s Past Performance proposal volume and information obtained from other sources available to the Government, to include, but not limited to: the Past
Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS); Electronic Subcontracting Reporting
System (eSRS), or other databases; and interviews/questionnaires with Program
Managers, Contracting Officers and Fee Determining Officials, the Defense Contract
Management Agency (DCMA), and commercial sources.
Each past performance example will be evaluated for award purposes by evaluating recency, relevancy and performance quality. Your responses to the factor will be evaluated in accordance with Section M of the solicitation.
5.1.1 Recency
To be recent, the effort must be ongoing or must have been performed during the past five (5) years from the date of issuance of this solicitation. ast performance information that fails this condition will not be evaluated.
5.1.2 Relevancy
An in-depth assessment of all recent performance information obtained will be completed to determine how closely the efforts performed under the past performance examples relate to experience, competence and technical capability required under this RFP. Defined as work that involves similar scope and magnitude of effort and complexities this RFP requires.
5.1.3 Past Performance Discrepancy
The offerors may address any performance problems/issues that the Government may identify and explain corrective action taken and current status. The
Government will consider this information in determining the offeror’s past performance rating.
5.1.4 Joint Venture
If a joint venture is contemplated, provide complete information as to the joint venture.
If this is an existing joint venture, include any recent and relevant past performance information of this joint venture (up to three).
If this is a first time joint venture, each party to the joint venture must provide a list of past relevant contracts (up to three each).
5.1.5 PPI Tool
The offerors shall use the PPI Tool (Use latest version from FBO.gov) in order to electronically submit the Past Performance Volume in accordance with the RFP.
See Appendix I for instructions.
6.0 VOLUME III – CONTRACT DOCUMENTATION
6.1 General Instructions
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to comply with the terms and conditions of the solicitation may result in the offeror being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete supporting rationale. The Government reserves the right to determine any such exceptions unacceptable.
6.1.1 Model Contract and Solicitation Representations and Certifications:
The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The offeror’s proposal shall include a signed copy of the Model Contract/Solicitation and completed Sections
A through K. This includes:
Section A – Solicitation/Model Contract Form: Complete Blocks 14, 15, 16, 19 and 20 of SF 1442 and include signed amendments to the solicitation
(if any). Signature by the offeror on the SF 1442 constitutes an offer, which the Government may accept. The “original” copy should be clearly marked under separate cover and should be provided without any punched holes.
Section B – Supplies or Services and Cost/Prices: Do not input pricing information in Section B of the Model Contract and Solicitation. All required pricing shall be included in the Price Volume.
Section I – Contract Clauses: Offerors shall complete all required clause fill-ins.
Section K - Representations, Certifications, and other Statements of
Offerors: Complete representations, certifications, acknowledgments and statements. Offeror must have filled out an online FAR provision 52.204-08
Annual Representations and Certifications (May 2014), Online Reps and Certs at http://www.sam.gov. Offeror shall ensure certification as a small business under North American Industry Classification System (NAICS) 332813, Electroplating, plating, polishing, anodizing and coloring.
6.1.2 Ability to obtain resources
Submit acceptable evidence that your company possesses adequate financial resources to perform the contract. An example of acceptable evidence is a commitment or explicit arrangement that will be in existence at the time of contract award, to acquire the needed materials, equipment, personnel and other resources necessary to sustain operations. This information will be used as part of the CO’s determination of responsibility IAW FAR 9.104.
6.1.3 Limitations on Subcontracting
IAW FAR Clause 52.219-14 Limitations on Subcontracting, at least 50 percent of the cost of contract performance. A deviation was approved for this effort to allow the 50 percent requirement to be satisified by subcontracting with like entities (i.e, small businesses) see SOW section 5.4.3.
Submit documentation that demonstrates how the prime contractor will meet this requirement.
6.1.4 Exceptions to Terms and Conditions
Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors shall clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost, and specific requirements of the solicitation. This information shall be provided in the format and content of
Table 6.1.3.

6.1.5 Authorized Offeror Personnel
Provide the name, title, and telephone number of the company/division point of contact regarding decisions made with respect to your proposal and who can obligate and bind your company contractually. Also, identify those individuals authorized to negotiate with the Government.
6.1.6 Company/Division Address, Identifying Codes, and Applicable Designations
Provide company/division's street address, county and facility code; CAGE code;
DUNS code; and size of business (large or small). This same information must be provided if the work will be performed at any other locations(s). List all locations where work is to be performed and indicate whether such a facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location. Small businesses who intend to enter into a joint venture must still meet the 50 percent requirement per FAR 52.219-14 and must include evidence of a formal/informal joint venture, to include setting up a SAM account
(http://www.sam.gov) that includes a CAGE code and a DUNS Number.
6.2 Volume Organization
The Contract Documentation volume shall consist of the following sections:

Section 3 – Exceptions to Terms and Conditions

APPENDIX I: DOWNLOADING THE PPI TOOL
The PPI Tool can be downloaded by performing the following steps (if you are unable to download the
Tool, contact the contracting officer for assistance):
1. Access the FedBizOps (https://www.fbo.gov/) website.
2. Find the solicitation posting.
3. Locate the “ppi tool” link from the “All Files” column on the solicitation’s
“Notice Details” tab .
4. Select the link and save the “ppi tool” to your computer. Name the file as the prime contractor +
RFP number + file extension (e.g. XYZCompanyFA8224-15-R-0027.mdb).
Note: PPI Tools saved in Microsoft version 2007 and greater will be saved with “.accdb” file extension.
Entering information in the PPI Tool
After selecting and saving the tool, enter information by performing the following steps:
1. Open the saved PPI Tool.
2. Select the “Options” button from the “Security Warning” banner, if applicable
. The “Security Alert” pop-up screen displays.

Note:
Files saved using Microsoft Office 2010 will have the “Enable Content” button on the “Security
Warning” banner and therefore will not get a Security Alert pop-up.

If a "read only" file is opened, in order to populate data in the file, click "Save As" in the "Read-
Only" message bar. Enter the filename as the prime contractor + RFP number + file extension
(e.g. XYZCompanyFA8224-15-R-0027.mdb).
3. Select the radio button “Enable this content” and then click “OK.” A setup pop-up screen displays.

4. Select the “Start” button. The “Application Setup” screen displays.

5. Enter the Offeror’s Name and RFP Number and then click the “Next” button. The application setup continues.

6. Choose the appropriate option by selecting the corresponding radio button and then click the “Next” button. The “Contractor’s Menu” displays.

7. Click the “Step 1: Set up Business Relationships” button to create a business relationship, if applicable, for each contributing contracting organization before proceeding throughout the PPI Tool. Identify all prime and sub-prime organizations and categorize them according to the appropriate role in the proposed acquisition. The “Business Relationships” screen displays.

8. Click the “New” button to create a business relationship for the proposed acquisition. An additional
“Business Relationships” screen displays.

9. Complete the fields as follows (fields marked on the screen with an asterisk ‘*’ are required):
Contractor’s Name: Self-explanatory
Role in Proposed Acquisition: Choose one of the four options – Prime, Sub, Joint Venture, or
Other (Explain). An explanation comment box will display when you select “Other.”
Place of Work: Location where contractor will perform work.
Percentage of Work: Identify percentage of work contributed by specified contracting organization.
Responsibilities: Detail responsibilities of specified contracting organization.
10. Select one of the buttons at the bottom of the screen.
Add – Saves the current business relationship and allows for the addition of a new one.
Close – Cancels the current business relationship without saving.
Note: In order to edit or delete an existing business relationship in the list, double-click on it.
11. Select the “Close” button on the “Business Relationships” screen after all of the business relationships has been added.
12. Click the “Step 2: Enter Past Performance Information (PPI)” button to enter the Past Performance Information. The “Past
Performance” screen displays.

13. Click the “New” button to enter Past Performance Information for the proposed acquisition. An additional “Past Performance” screen displays.

14. Complete the fields on each of the tabs as follows (fields on the screen marked with an asterisk ‘*’ are required):
Contract Information Tab
Contractor: Select from the dropdown the appropriate contractor.
Cage Code: Self-explanatory.
Contract Number: If you don’t have a contract number, enter “N/A.”
Program Title: Enter full name of program.
Contracting Agency/Customer: Enter servicing contracting agency and customer (office symbols suffice).
DUNS Number: Self-explanatory
Delivery Task/Order: If the order is provided as a stand-alone reference, enter the task/call/delivery/purchase order number.
Contract Type: Enter Firm-Fixed-Price (FFP), Cost Plus Fixed-Fee (CPFF), Indefinite
Delivery/Indefinite Quantity (ID/IQ), LH, Blanket Purchase Agreement (BPA), Cost Plus
Incentive-Fee (CPIF), Cost Plus Award Fee (CPAF), etc. For additional clarification, click the question mark button.
Short Program Title (i.e. Acronym): Enter abbreviated title for the program or enter
“N/A.”
Contract Dollar Value:
o Original: Input total contract dollar value, with all options if applicable, in the amount originally awarded on the referenced contract.
If ID/IQ or BPA, provide total ceiling.
If stand-alone task/call/delivery/purchase, provide amount of the individual contract.
o Current: Input total contract dollar value, with all options if applicable, as the contract stands at time of PPI submission.
If ID/IQ or BPA, provide total ceiling.
If stand-alone task/call/delivery/purchase, provide amount of the individual contract.
Period of Performance (mm/dd/yy) o Start Date: Input start date of contract.
o Original End Date: Input original end date based on award.
o Current End Date: Input end date, as the contract stands at time of PPI submission.
o Explain the differences in Period of Performance, if applicable: Enter an explanation of the difference between “Original End Date” and “Current End Date.”
Program Details Tab
Brief Description of Effort as:
o Select Prime, Sub, Joint Venture, or Other (Explain). An explanation comment box will display when you select “Other.”
o Provide a brief description of the actual work performed under this contract reference.
o ****If applicable: Provide information on problems encountered on the identified contracts. At a minimum, briefly describe the problem experienced, actions taken to alleviate the problem, and whether or not the problem was satisfactorily overcome.****
Explain/describe the work provided and an explination on what aspects of the contract are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate (i.e. scope, magnitude of effort and complexities).
Additionally, include comments regarding compliance with contract terms and conditions. Include comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions. For problems encountered, include corrective action implemented, effectiveness of actions, and customer involvement required.
POC & Key Individuals Tab
Customer Points of Contact: Click the “Program/Site Manager,” “Contracting
Officer” or “Admin POC” button for the point of contact that you would like to add, edit, or delete.
Note: For government contracts provide current information (i.e. name, office, phone and email) on Program Manager, Contracting Officer, and Admin POC, if available.
15. Select from the buttons at the bottom of the Past Performance screen:
Save – Saves the Past Performance Information and displays the “Contract Information” tab on the Past Performance screen.
Close – Closes the Past Performance screen. If there were any updates, a pop-up window displays asking to save before closing.
Delete – Deletes the current PPI record. A pop-up window displays, select “Yes” to delete the record or “No” to close the window without deleting the record.
Note: In order to edit or delete an existing PPI record in the list, double-click on it.
16. Select the “Close” button on the “Past Performance Information” screen after all of the PPI records have been added.
Printing Reports and Submitting PPI Tool
In order to print the Business Relationships and PPI reports, perform the following steps:
1. Click the “Step 3: Print Report” button . A pop-up displays asking which report to print.

2. Select separately each of the two options, “Business Relationships” or “Past Performance.” The
Business Relationships and Past Performance documents will print separately; combine the two to create your Past Performance Volume.
In order to submit the Business Relationships and PPI, perform the following steps:
3. Click the “Step 4: Submit” button . The “Submission Instructions” screen displays.

4. Follow the submission instructions.
Hard copies of the pages generated from this tool shall be used in the hard copy of the Past
Performance Volume subject to the limitations outlined in this RFP and should be included in the Past Performance Volume.
Submit an electronic copy (e.g. CD) of the saved PPI database file with your proposal. Enter the filename as the prime contractor + RFP number + file extension (e.g.
XYZCompanyFA8224-15-R-0027.mdb).
Once the file is saved to a CD or any location that is marked as “Read-only,” it must first be saved to the desktop in order to read/edit the file.
