About this file

This document is a Past Performance Questionnaire (PPQ) for Architect-Engineer (A-E) Services, solicitation number FA8201-25-R-0003, dated 01 July 2025. The PPQ is designed to evaluate a contractor's performance across four key areas: Quality of Service, Cost Control, Schedule, and Customer Satisfaction. The form provides a structured method for customers to rate contractor performance using a scale ranging from Unsatisfactory to Exceptional, with specific sub-criteria within each rating category.

The PPQ is part of a multi-award Indefinite Delivery/Indefinite Quantity (ID/IQ) contract for A-E services supporting the 75th Civil Engineering Group at Hill Air Force Base. The contract will select up to ten contractors, with the first two slots reserved for Small Businesses and the remaining slots open to full and open competition. The place of performance includes Hill AFB and associated locations such as Little Mountain Test Annex, Utah Test and Training Range, and Boulder Seismic Station. Completed questionnaires are to be returned by 18 July 2025 to specified Air Force email addresses.
