About this file
This document contains a subcontractor consent form and details of a federal contract opportunity for underground storage tank replacement services. The subcontractor consent form requests that any major subcontractors or teaming partners identified in a proposal consent to allowing their past and present performance information to be disclosed to the prime contractor during the source selection process.
The federal contract opportunity is a small business set-aside between $500,000 to $1 million issued by the Department of the Air Force Headquarters District Washington. It is for a firm-fixed price contract to replace underground storage tanks and related components at a Defense Army and Air Force Exchange Services gas station at Joint Base Anacostia-Bolling. The scope of work includes replacing underground tanks and sumps, excavating and backfilling, testing and commissioning, and coordinating with facility personnel. Certified underground storage tank technicians are required. Funds are not presently available and no award will be made until funding is secured.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Evaluation Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The consent form must be signed by an individual who had the authority to sign for and legally bind the company. The completed consent forms should be submitted as part of your Past Performance volume. The following consent form requires editing by the prime contractor prior to forwarding to their respective subcontractors (see bolded text).
