STATEMENT OF WORK
FOR CONSTRUCTION
Construct CCTV Base Wide – Phases 3, 4 and 5
Revision 0: 5 January 2019, Original Issue

1. PROJECT INFORMATION:
A. PROJECT NUMBER: XQPZ150702L
B. PROJECT TITLE: Construct CCTV Base Wide – Phases 3, 4 and 5
C. PROJECT LOCATION: USAF Academy, Colorado
2. DESCRIPTION OF PROJECT:
A. PROJECT BACKGROUND: Install phases 3, 4 and 5 of a robust CCTV camera system in various locations throughout USAFA. The CCTV cameras require integration with the current Falcon-Eye camera system, replacement of existing Falcon-Eye Cameras along with placement of new security cameras. Phases 1A, 1B and 2 have been completed. This project will finish up all remaining requirements for a complete and usable system.
B. CONSTRUCTION SCOPE: The Contractor shall execute the project as shown in this SOW, design drawings, attachments, additional project documents and specifications.
3. GENERAL PROVISIONS:
A. The Contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting USAFA Specifications.
B. The Contractor must comply with USAFA Environmental Standards 2018 which outlines provisions to help ensure Contractor compliance with environmental law.
C. Include in the proposal a POP duration including on-site construction time.
D. PRE-CONSTRUCTION SITE SURVEY - Prior to the start of any on-site construction activities, the Contractor shall:
1. Conduct a project site condition survey with the 10 CES Construction Inspector.
2. The Contractor shall note the condition of trees, shrubs, grassed areas, concrete, and any structural damage that may exist immediately within or adjacent to work site.
3. The contractor shall field verify all dimensions or examine unseen connections before ordering materials or any performing work.
E. WORK CONFORMANCE - The work area will be confined to the applicable areas as shown in the design drawings and listed in this SOW. All work must conform to the current provisions of the USAFA Architectural and Engineering Services Guide available on CD from the Project Manager.
F. SUBMITTAL REGISTER - A submittal register is provided for this project, and is a supplemental document to the specifications. Provide submittals and maintain the Submittal Register throughout project construction period of performance, as required by the specifications Section 01 33 00, Submittal Procedures. This list of submittals is not all-inclusive; other submittals and forms may be required by the current Contract documents. Contractor shall submit the Master Submittal Register to Contracting within 14 days after the Notice to Proceed (NTP).
G. BASIS OF DESIGN - Any brand name items listed in this SOW and drawings are intended as a BOD only.
The Contractor may substitute a product, or products, for the brand name items as long as the substituted product(s) meet the minimum performance standards established by the listed salient characteristics. Refer to specification Section 01 00 00 GENERAL REQUIREMENTS. An ‘Or Equal’ product shall be submitted with the initial proposal and will be reviewed for technical acceptability.
The Contractor is responsible for how the substituted product(s) affect the design and schedule of the project. 10 CES will review the substituted products for General Conformance only. All substituted products will be reviewed for compliance with the USAFA Design Standards, any SHPO or historical considerations, Environmental regulation compliance and any other review processes that are applicable to this project.
As a part of the 10 CES review process for substituted products, and prior to the substituted product being used or installed, the Contractor must demonstrate in writing that the substitution will meet all project requirements listed above.
H. HAZARDOUS MATERIALS. Refer to Attached Hazmat Reports and Environmental Standards 2018 for requirements.
I. TRAFFIC CONTROL - Traffic control will be required and must conform to MUTCD. A traffic control plan must be submitted for approval prior to starting any demolition. The roadway and/or parking lot shall be left in a safe and usable condition at the end of each work day. All traffic control plans must be re-submitted by the construction contractor, with a time schedule for placement, at least two weeks prior to the requirement to insure it is clear when and where the traffic control devices will be placed.
4. SPECIFIC REQUIREMENTS:
A. CLIN 0001 – Provide cost to install those portions of the project falling within the area designated PHASE 3 construction.
B. CLIN 0002 – Provide cost to install those portions of the project falling within the designated PHASE 4 construction.
C. CLIN 0003 – Provide cost to install those portions of the project falling within the designated PHASE 5 construction. Contractor shall not bid options for the Santa Fe Trail design in Phase 5 to include any poles identified. These will be furnished and installed by the government.
D. Contractor shall incorporate as part of the bid all changes listed on Attachments 13 and 14.
E. Any new cameras, DVRs and equipment must integrate seamlessly with the existing Milestone XProtect, Expert Edition Video Management System.
F. No technology in hardware or software may come from China.
G. CAD & GIS files that comply with USAFA CAD/GIS standards are included as part of this Statement of Work. The construction contractor shall provide as-built drawings using the CAD & GIS files provided by the government. The construction contractor shall ensure their as-built CAD & GIS submittal is compliant with the latest USAFA CAD/GIS standards. USAFA CAD/GIS standard template files are available upon request.
H. Specific GIS Submittal Requirements:
Features outside the facility/building footprint, as well as the outline of the new facility/building, shall be located using survey grade GPS equipment with an accuracy of +/- 0.03ft (survey grade accuracy). Features may include, but not limited to walls, utility lines and all their components (pipes, valves, manholes, utility poles, vaults, etc.), culverts, roads, curb & gutters, concrete pads, fences, boundaries, sidewalks, monuments, gates, posts, etc.
Features captured with survey grade GPS equipment shall be delivered in ArcGIS format (described below) and their attributes populated accordingly.
Features shall be geo-referenced (have true location on surface of the earth) based on Colorado State Plane Central zone 0502 coordinate system in feet (NAD_1983_StatePlane_Colorado_Central_FIPS_0502_Feet).
GIS data shall be submitted in ESRI ArcGIS 10x or newer format in a File Geodatabase (FGDB) file format. Submittals shall be ready to be loaded and viewed without conversion or editing. GIS data shall comply with Spatial Data Standards for Facilities, Infrastructure, and Environment (SDSFIE v3.1 or latest version). SDSFIE compliant geodatabase template in ArcGIS format is available from USAFA 10 CES CAD/GIS section upon request.
I. UTILITY COORDINATION AND CSU INSPECTOR NOTIFICATION – The Construction Contractor shall provide utility coordination and notify the CSU.
See Attachment 03 Utility Locate Maps. Inspectors prior to starting excavation work or related utilities work associated with this project, per the following:
1. Academy Dig Permits and UNCC - The Contractor shall submit the standard USAFA Form 144 Dig Permit required prior to starting excavation work or related utilities work for the project (Dig Permits, Bldg 8116, 7:30- 8:30am and 3:00-4:00pm Monday-Friday). The Academy will provide utility locates for water and sanitary sewer. The UNCC will provide utility locates for gas and electric, but does not notify CSU regarding planned work in the vicinity of CSU utility infrastructure.
2. CSU Inspector Notification – The Contractor shall provide a minimum of 24-hour notice to CSU Inspectors prior to starting work in the vicinity of CSU gas and electric infrastructure. The current CSU POCs (subject to change) are:
(a) GAS – Wayne Simshauser, Office 719-668-3667, Cell 719-491-8142, and
(b) ELECTRIC – Daniel Gonzales, Office 719-668-5638, Cell 719-491-6886.
3. HTHW Infrastructure – Where the project includes work directly involving HTHW infrastructure or adjacent to HTHW lines, Contractor shall ensure adequate distance separation between the HTHW infrastructure and any nearby gas and electric infrastructure, such that the heat from the HTHW does not damage the PVC gas mains or PVC electric lines and create a safety risk.
4. Utility Outages – For the duration of the project, Contractor shall provide information to the CE Inspector for coordination of outages thirty-five (35) calendar days in advance.
J. SECURITY AND ACCREDITATION
1. The contractor shall keep the Communications PM and Security Advisor (SA) (Cyber Office, USAFA/A6C, 333-9880) apprised on any security issues discovered on the hardware or software. Software utilized for the system must be tested and approved by the Air Force. The contractor and any subs shall not openly disclose USAFA vulnerabilities to anyone outside the government.
The overall system, servers and data in this system shall be secured and have permission based control access points with a use 2- Factor Authentication solution (e.g. PIV cert on CAC, or Token), with unique user name assigned to each administrator IAW AFI 33-129, Chapter12-Countering the Threat.
2. All system components must be security configured in accordance with the latest Defense Information Systems Agency, Security Technical Implementation Guide (DISA STIG) by performing the STIG and mitigating all identified vulnerabilities or notating the inability to mitigate the threats in a plan of actions and milestones (POA&M) and provide this documentation to the USAFA Cyber Security office with a clear way forward. STIGS can be located at http://iase.disa.mil/stigs.
USAFA networks have controlled network traffic. The Contractor shall work closely with USAFA's IT personnel to ensure service is accessible by authorized government personnel. This may require white listing, domain named server modifications, updates, scanning and/or detailed discussion with security personnel to ensure access that works for our military institution and the external visitors/schools that participate in this event. The Contractor shall demonstrate this capability at the Critical Design Review (CDR).

3. The Government will designate certain solution users who are able to access all government data on the solution. The Government will designate other users as basic administrators who do not have unrestricted access. The Contractor shall create administrative capacity in the solution such that administrators can be changed throughout the duration of the contract. The solution shall accommodate administrators with individual logins and passwords for all functions. All logins including but not limited to administrator logins must be encrypted and complaint with the DISA Web Server STIG and meet a minimum complexity requirements as listed in AFMAN17-1301, para.
8.5.4.5 - 8.5.4.5. Case sensitive, at least 15-character mix of two upper case letters, two lower case letters, two numbers, and two special characters. For password changes, change at least four characters when a new password is created.
4. When requested, the Contractor shall provide all required documentation to accredit the system. This documentation at a minimum will include a continuity of operation procedures, incident response plan, basic network diagram, and basic vulnerability management plan detailing scan frequency and patch frequency including metrics. Failure to provide this documentation will delay accreditation of the system.
5. The CCTV system & cameras should be Open Network Video Interface Forum (ONVIF) conformant and comply with ONVIF Profile S for IP-based video streaming to ensures cameras will work with other vendor's video management software, Profile G for edge storage and retrieval (e.g. video management software to ensure the cameras & Virtual Management System (VMS) will work with other vendor's video storage devices and Profiles C & A for integration between access control and IP-based video management systems to ensure the VMS system will work with other vendor's access control systems.
K. POST CONSTRUCTION STORM WATER MANAGEMENT - Refer to Environmental Standards 2019 for requirements.
L. Coordinate work schedules with the 10 CES CM at least seven (7) calendar days in advance.
M. Any exceptions to this statement of work must be detailed in the contractor proposal.
N. An acceptable period of performance (POP) for this project shall be no more than 287 calendar days.
O. Contractor to provide as-built GIS infrastructure underground location drawings per USAFA GIS Standards at the end of the project.
P. No new cameras shall be placed on the State Historical Society protected colored Murano tile located in the stairwells and exterior of the Cadet Area buildings. Any camera work done near these areas will require prior approval of the on-site inspector before being started or installed.
Q. Contractor shall include labor hours needed for doing work after regular
0700- 1700 normal working hours due to special events, cadet test finals and any noise related work near classrooms.
R. Within 15 days after award, the contractor shall submit detailed schedule for each phase, detailing the critical path for each phase for Government review and approval.
S. AS-BUILT DRAWINGS - Maintain redlined as-built drawings in the project construction office (or trailer) during the duration of the construction. The redlined drawings must be available for inspection during normal working hours.
T. During the duration of the project, the contractor shall coordinate outages to the CE Inspector 35 calendar days in advance.
U. Every conduit and camera served shall be numbered from the starting point at the COM rooms to each endpoint at the camera. Each camera shall be numbered in relationship to the conduit that serves a particular camera that it supports. CAD drawings shall be required showing location of every conduit to the camera showing markings as mentioned previously.
5. PROJECT MEETINGS:
A. PRE-CONSTRUCTION MEETING - A pre-construction meeting will be held at the Contracting conference rooms within two (2) weeks after the contract award.
B. WEEKLY MEETINGS - Weekly planning meetings will be held at the contractor’s jobsite trailer. The date and time will be the same each week, and will be set by the construction manager at the pre-construction meeting.
6. SUBMITTALS:
A. Provide submittals for all equipment and material as outlined in the project submittal register.
B. 10 CES CM must review all submittals before commencement of relevant work.
7. PROJECT CLOSEOUT:
A. Supply as-built CADD drawings at the conclusion of the project on CD, four (4) copies and four (4) hard copies, in the format in adherence to the latest USAFA CADD standards.
B. Supply four (4) hardcopies of Operation and Maintenance (O&M) manuals, and two (2) on CD.
C. PROJECT CLOSEOUT DELIVERABLES – Before project closeout, ensure that all Environmental reports have been submitted and approved by the 10 CES Environmental group (CEIEC) in USAFA Civil Engineering.
8. TYPICAL USAFA YEARLY CALENDAR EVENTS: These USAFA events are typical each year. The Contractor is required to include all events within the overall project construction schedule. (Note: events may occur more than once in a given project schedule).

B. Spring Break 22 Mar 2019 to 30 Mar 2019
C. Graduation 22 May 2109
D. In-processing 27 June 2019
E. Parents Weekend 29 Aug 2019 – 2 Sep 2019
F. CORONA* 27 Oct 2019 – 2 Nov 2019
G. Thanksgiving Break 26 Nov 2019 to 30 Nov 2019
H. Winter Break 16 Dec 2019 to 2 Jan 2020
*Indicates USAFA Institutional Events. Three (3) days prior to each of these Institutional dates, the Contractor shall remove all barriers around newly poured concrete that has cured a minimum of seven (7) days. The barriers may also be removed a minimum of two (2) days after new concrete sealer has been applied. The barriers shall be moved to a location approved by the 10 CES Construction Inspector. The Contractor shall coordinate these dates in their proposed construction schedule.
