STATEMENT OF WORK

FOR CONSTRUCTION

RPR/CONST PLANETARIUM RENOVATION

1. PROJECT INFORMATION:

A. PROJECT NUMBER: XQPZ 15-0112

B. PROJECT TITLE: Rpr/Const Planetarium Renovation

C. PROJECT LOCATION: Building 2120, Planetarium, USAF Academy, Colorado

2. DESCRIPTION OF PROJECT:

A. PROJECT BACKGROUND: The USAFA Planetarium is an original feature of the Academy and is situated in the Cadet Area. The Planetarium has been out of service for more than a decade and is in need of renovation. The Planetarium is comprised of both interior and exterior restrooms, mechanical rooms, projection room and planetarium. The main floor area is approximately 5000SF.

B. CONSTRUCTION SCOPE: The Contractor shall execute the project as shown in this Statement of Work (SOW), additional project drawings and Specifications.

3. GENERAL PROVISIONS:

A. The Contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting USAFA Specifications.

B. The Contractor must comply with specification sections 01 35 00 USAFA Environmental Standards and 01 35 10 USAFA Erosion Control Revegetation and Tree Care Standards which outlines provisions to help ensure Contractor compliance with environmental law. Include a Spill Prevention and Response Plan with every project.

C. Pre-Construction Site Survey: Prior to the start of any on-site construction activities, the Contractor shall conduct a condition survey with the 10 CES Construction Inspector, after which the Contractor shall note the condition of trees, shrubs, grassed areas, concrete, and any structural damage that may exist immediately within or adjacent to work site.

D. The work area will be confined to the applicable areas as shown in the design drawings and listed in this SOW. All work must conform to the current provisions of the USAFA Architectural and Engineering Services Guide available on CD from the Project Manager.

E. Submittal Register: A submittal register is provided for this project, and is a supplemental document to the specifications. Provide submittals and maintain the Submittal Register throughout project construction period of performance.

Contractor shall submit the Master Submittal Register to Contracting within 14 days after the Notice to Proceed (NTP).

F. Any brand name items listed in this SOW and drawings are intended as a Basis of Design (BOD) only. These brand names were selected because they address the specific issues that are unique to this project (i.e. complying with USAFA Design Standards, SHPO Review, User requirements, maintenance considerations, Environmental compliance, etc.). Because of these unique conditions and requirements, the brand name products have been researched and shown to have a proven record of working when the manufacturer’s recommendations and requirements have been met.

The Contractor may substitute a product, or products, for the brand name items as long as the substituted product(s) meet the minimum performance standards established by the brand name product(s).

The Contractor is responsible for how the substituted product(s) affect the design and schedule of the project. 10 CES will review the substituted products for General Conformance only. All substituted products will be reviewed for compliance with the USAFA Design Standards, any SHPO or historical considerations, Environmental regulation compliance and any other review processes that are applicable to this project.

As a part of the 10 CES review process for substituted products, and prior to the substituted product being used or installed, the Contractor must demonstrate in writing that the substitution will meet all project requirements listed above.

G. HAZARDOUS MATERIALS. All hazardous materials (HAZMAT) brought on site are subject to pre-approval by the USAFA Hazardous Materials Management Program (HMMP) Team (CE, LG, SE & Bio Env). AFI 32-7086, Hazardous Material Management provides approval requirements for contractor used HAZMAT. USAFA may prohibit the use of any HAZMAT it deems to be especially hazardous to human health or the environment or may result in excessive quantities of hazardous waste. The Prime Contractor will submit an Alternative AF Form 3952, Chemical/Hazardous Material Authorization Request (provided by the HAZMAT Tracking Activity (HTA)) for all HAZMAT required for each Project.

The Prime Contractor will also submit HAZMAT requirements for any subcontractors on the Contract. The Alternative AF Form 3952, must fully describe the Project requiring the use of HAZMATs. The Process Name in EESOH-MIS will include the Project Number and Name of the Project. The approval process requires approximately 10 working days. The Contractor will report authorized HAZMAT usage at any time during the execution of the contract or at least monthly to the HTA (email is the preferred method). The report shall include a listing of the assigned barcode label numbers, and date used or disposed. This report may be emailed (LG_Hazmat@us.af.mil) or hand delivered to the HTA.

H. For all projects that have a Period of Performance that is 90-days or longer in length, the Contractor shall prepare and submit a DRAFT DD Form 1354, Transfer and Acceptance of Military Real Property, for all installed property (by building number if multiple buildings are involved in the project/task order) to the

CO. The DRAFT 1354 shall be submitted no later than 30-days after the Notice to Proceed. This DRAFT 1354 will be reviewed by Real Property to resolve any issues and provide clarity to the Contractor for inclusion in the FINAL DD form 1354. Please request a sample document if needed.

4. SPECIFIC REQUIREMENTS:

A. CLIN Schedule:

1. CLIN0001: Provide all work for XQPZ15-0112 Rpr/Const Planetarium Renovation described in this SOW, and on the plans and specifications not listed in CLIN0002 and CLIN0003. This work is considered funding code 52200.

2. CLIN0002: Provide all external work related to the new chiller and chiller enclosure not included in CLIN0001. This is a breakout of the minor construction portion of the project, not a bid option. This work is considered funding code 52904.

3. CLIN0003: Provide all work as described on plans and specifications for Bid Option 1. Mainly, this is some (not all) work related to the Lower Level and exterior bathroom remodel. This work is considered funding code 52400.

B. Non-standard working hours may be required in order to perform some of this work according to the specifications and drawings.

C. All RFIs, contract modifications, sample boards, decisions, etc. must be coordinated with USAFA/A7, and 10 CES. Ultimate refusal or acceptance shall rest with A7.

D. Construction Staging:

1. 10 parking spaces for Contractor vehicles will be reserved within 225 feet of the Planetarium

2. A staging area 25 feet by 50 feet will be reserved within 125 feet of the Planetarium. This space will likely be on a grassy area and the Contractor will need to repair to the original condition upon completion.

Other temporary accommodations may be possible with a minimum of one week’s notice.

3. An additional staging area of 25 feet by 75 feet will be made available within 5 miles of the Planetarium.

E. Utility Coordination and CSU Inspector Notification – The Construction Contractor shall provide utility coordination and notify the Colorado Springs Utilities (CSU) Inspectors prior to starting excavation work or related utilities work associated with this project, per the following:

a. Academy Dig Permits and UNCC (Utility Notification Center of Colorado) - The Contractor shall submit the standard USAFA Form 144 Dig Permit required prior to starting excavation work or related utilities work for the project (Dig Permits, Bldg. 8116, 7:30-8:30am and 3:00-4:00pm Monday- Friday). The Academy shops will provide utility locates for water and sanitary sewer. The UNCC will provide utility locates for gas and electric, but does not notify CSU regarding planned work in the vicinity of CSU utility infrastructure.

b. CSU Inspector Notification – The Contractor shall provide a minimum of 24-hour notice to CSU Inspectors prior to starting work in the vicinity of CSU gas and electric infrastructure. The current CSU POCs (subject to change) are:

(a) GAS – Wayne Simshauser, Office 719-668-3667, Cell 719-491-8142, and

(b) ELECTRIC – Daniel Gonzales, Office 719-668-5638, Cell 719-491-6886.

c. HTHW (High Temperature Hot Water) Infrastructure – Where the project includes work directly involving HTHW infrastructure or adjacent to HTHW lines, Contractor shall ensure adequate distance separation between the HTHW infrastructure and any nearby gas and electric infrastructure, such that the heat from the HTHW does not damage the PVC gas mains or PVC electric lines and create a safety risk.

d. Utility Outages – For the duration of the project, Contractor shall provide information to the CE Inspector for coordination of outages fifteen (15) calendar days in advance.

e. The Contractor shall confirm with CSU if additional load is going to require CSU to modify their existing gas meter/supply service. The contractor will need to complete and submit Load Data form.

F. Any exceptions to this statement of work must be detailed in the Contractor proposal.

5. PROJECT CLOSEOUT:

A. Supply as-built CAD drawings at the conclusion of the project on CD, four (4) copies and four (4) hard copies. The CAD drawings must be in the format in adherence to USAFA CAD standards. All CADD Standards requirements and associated files, templates, blocks, border sheets etc. are available upon request from 10 CES.

B. Supply four (4) hardcopies of Operation and Maintenance (O&M) manuals, and two (2) on CD.

C. Project Closeout Deliverables – 30 Days prior to construction completion, notify the Construction Manager through the Contracting Officer of the construction closeout via the Contractor completed Final DD Form 1354 form (Note: Beneficial Occupancy Determination (BOD) must be completed prior to final signature of Final DD Form 1354). All as-built drawings, O&M Manuals, and CDs shall be delivered to the Construction Representative prior to BOD.

6. USAFA YEARLY CALENDAR EVENTS:

A. Winter Break 17 Dec 2016 – 3 Jan 2017

B. Spring Break 24 Mar – 1 Apr 2017

C. Graduation 19 May – 24 May 2017

D. Internal Processing 29 Jun 2017

E. Basic Cadet Training (BCT) 29 Jun – 5 Aug 2017

F. Parent’s Weekend 1 Sept – 2 Sept 2017

G. CORONA 25 Sept – 30 Sept 2017

The Contractor shall coordinate these dates in their proposed construction schedule.
