ADDENDUM TO 52.212-1

To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein and meet solicitation requirements.

Failure to meet any solicitation requirement may result in an offer being ineligible for award.

This solicitation is a request for quote (RFQ). If anywhere used in this solicitation, the term "Request for Proposal (RFP)" shall mean "RFQ", the words "proposal" or "offer" shall mean "quote", the word "offeror" shall mean "quoter", and the word "contract" shall mean "purchase order".

All information returned shall be confined to the appropriate proposal part.

The quote shall be prepared clearly and concisely to demonstrate that the offeror has a complete understanding of the requirements. Offerors shall prepare their quotes on the presumption that the Government has no previous knowledge of their capabilities. Quotes will be evaluated only on the information specifically presented with the quote package.

Offerors must submit the following:

1. Cover letter with acknowledgement of compliance with all solicitation terms and conditions, acknowledgement of all solicitation amendments, firm/offeror's name, address, point of contact, offeror's System for Award Management (SAM) Unique Entity ID (UEID) and Nato Cage And Government Entity (NCAGE) number.

In providing acknowledgments as above, to all solicitation terms and conditions, the offeror accepts the contract terms and conditions as written in the solicitation sections "Solicitation/Contract Form" through "Representations, Certification, & Other Statements" as amended. These sections constitute the model contract.

2. Complete the necessary fill-ins and certifications.

3. Proof of active registration status in SAM on submittal of proposal as required by RFO provision 52.204-7

4. The offeror must submit a copy of a current Societa' Organismi di Attestazione (SOA) certification for categories OS18A, with a classification level sufficient to cover the total proposed project amount. This certification may be held by either the prime contractor or the subcontractor performing the work; however, if the work is subcontracted, the performing subcontractor must hold the required certification that will also constitute part of the proposal. No change to the proposed subcontractor is permitted during performance without prior approval from the Contracting Officer. Approval for a change is contingent upon proof that the new subcontractor possesses the same SOA certification and classification as the firm evaluated at the time of award.

5. Current Documento Unico di Regolarita' Contributiva (DURC) certifying the payment of host social security benefits. This document must not be older than 120 days from date of proposal.

6. Current Anti-Mafia Self-Certification including copy of the signatory's identity document. (see template form provided).

If, at any time, before or after contract award, the Government discovers any mafia-related activities or infiltration, the Government will consider it a contract breach and proceed by rejecting the offer or terminate the contract.

In addition to the requirements outlined above, proposals must consist of two additional separate parts:

PART I - PRICE PROPOSAL

The offeror must provide a price breakdown for the project by completing the Construction Cost Estimate Breakdown form provided.

Proposed amounts shall be limited to 2 digits after the decimal. To be considered for award all prices must be in Euro currency only. The Construction Cost Estimate Breakdown form must be complete, accurate, and sufficiently detailed to demonstrate a clear understanding of the requirement. The burden of proof for credibility of proposed pricing rests with the offeror.

Using the amount resulting from the completion of the Construction Cost Estimate breakdown form, fill the "Amount" block in the SF 1449's table at page 3. This amount will constitute the Total Evaluated Price (TEP).

The Construction Cost Estimate breakdown form must be returned in its entirety as well as the excel file used to develop the Construction Cost Estimate Breakdown.

FA568226Q00050001

PART II - TECHNICAL PROPOSAL - Limited to no more than 20 pages. Pages exceeding the page limitation will not be read or evaluated. The proposals must not be bigger than A4 size paper except for fold-outs used for charts, tables, or diagrams, which may not exceed A3 size paper. A page is defined as one face of a sheet of paper containing information. Typing shall not be less than 12 pitch. Elaborate formats, bindings or color presentations are not desired or required.

Subfactor 1 - Experience: Submit offeror's or subcontractor experience with the same work/trades and complexity as this requirement. The offeror must submit evidence of at least two (2) similar projects successfully performed within the last five (5) years from the date of proposal where the offeror or subcontractor was responsible for the entire project performance that involved substantially the same work and principal trades, magnitude and complexity as this requirement. Note to offerors: the subcontractor' experience presented with the proposal must be the same of which the SOA certification belongs to and that will be working on the construction site.

For each of the two projects, the offeror must submit a detailed narrative of the work performed, the client points of contact (to include phone number and email address), date of completion and award amount.

CURRENCY

All prices in response to this solicitation must be in Euro currency. The Contracting Officer has determined that submission of submission of offers in Euro currency is required for this solicitation to be entered into and performed outside of the United States. Therefore, submission of offers other than Euro currency are not accepted.
