About this file

This is a Past Performance Questionnaire template for solicitation FA524026Q0017, issued by the 36th Contracting Squadron on Guam for corrosion control repair and paint services on AGE (Aerospace Ground Equipment) and MUNS (Munitions) trailers. Contractors submitting quotes are required to complete and return the questionnaire via email to the listed contracting personnel (Gary Scullin, Elena Gifford, and Thomas Houle) no later than 21 August 2026 at 11:00 AM Chamorro Standard Time.

The questionnaire collects general contractor information including company name, address, point of contact details, project description, contract number, overall contract value, and performance period. Contractors must indicate whether they performed as prime or sub-contractor. The evaluation component requires raters to assess contractor performance across six criteria: Quality of Service, Schedule, Cost Control, Management of Key Personnel, Small Business Subcontracting, and Regulatory Compliance. Each criterion uses a five-point adjectival scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory), with mandatory additional comments required for any Marginal or Unsatisfactory ratings. The Cost Control and Small Business Subcontracting categories include a "Not Applicable" option.
