About this file

This document is a detailed Asbestos Survey and Assessment report for Building 650 at Yokota Air Base in Japan, prepared for the Air Force Center for Environmental Excellence (AFCEE) in June 2003. The comprehensive survey identified and analyzed multiple asbestos-containing materials (ACMs) throughout the three-story, 69,335 square foot building, which serves as a Communications Squadron Systems Annex for the Defense Information Systems Agency.

The survey discovered sixteen non-friable miscellaneous materials and three thermal systems insulation materials containing asbestos, including vinyl baseboard mastics, pipe elbow insulations, duct gaskets, putty, and vinyl floor tiles. Most materials were found to be in good condition with low potential for damage, resulting in hazard rankings of 1-2. The report recommends incorporating these findings into an Operations and Maintenance (O&M) Plan and suggests that any disturbance of these materials could potentially release asbestos fibers, necessitating careful management and potential professional intervention during future renovations or maintenance activities.

Asbestos Survey and Assessment June 2003 Yokota Air Base, Japan Building 650 Page 1

1.0 INTRODUCTION

MWH was retained by the Air Force Center for Environmental Excellence (AFCEE) to conduct an inspection for asbestos-containing materials (ACM) throughout the industrial buildings located at the Yokota Air Base (AB) located in Japan. The inspections included an assessment of friable and non-friable suspect asbestos-containing building materials.

This report details the results of Building 650 – Communications Squadron Systems Annex

- Defense Information Systems Agency. The survey and assessment was performed on behalf of AFCEE in accordance with the scope of work dated 29 January 2002 and authorized under AFCEE Contract No. F41624-00-8022, Delivery Order Number 51 dated 25 March 2002.

The detailed survey and assessment followed criteria set forth by the Asbestos Hazard Emergency Response Act (AHERA), and included both an asbestos survey and the collection of bulk samples for the building. The building was inspected by accredited asbestos building inspector Angelina Symicek of Cedar Creek Consulting, Inc.

MWH performed an asbestos survey of Building 650 – Communications Squadron Systems Annex Defense Information Systems Agency. The building consists of three floors. The basement is approximately 20,990 SF, the 1st floor is approximately 28,700 SF and the 2nd floor is approximately 19,645 SF for a total of 69,335 SF. The interior finish consists of drywall and concrete walls, concrete floor with carpeting and floor tiling, and drywall, concrete ceiling and ceiling tiles. The exterior of the building is constructed of concrete.

Background information on asbestos is presented in Appendix A. Detailed information on the location of the ACM can be found in Section 3.0. The extent of materials for each floor are outlined in Drawings B30, B33, and B36.

Building 650 Page 2

2.0 DETAILED SURVEY METHODOLOGY

The main purpose of this survey and assessment was to establish the location of ACM and quantify them to the extent possible, throughout the facility. The approach taken to complete the survey follows the AHERA protocol. This approach involves identifying and categorizing potential ACM, identifying and categorizing homogeneous areas, establishing a hazard rank for each homogeneous area, and sampling the materials for microscopic analysis.

Following is a brief overview of the asbestos survey and assessment activities: A more detailed discussion of each activity then follows:

• A room-by-room tour of the building was conducted, and building layout drawings were verified using existing drawings provided by the Yokota AB Environmental Flight or new drawings were constructed on-site. The Building Layout Plan for each floor is presented as Drawings B28, B31, and B34.

• On a room-by-room basis, homogeneous sampling area determinations were made along with observations concerning the locations of suspect ACM materials.

• An assessment was made of each suspect ACM to assess the friability, potential for disturbance and material condition.

• Destructive bulk sampling was conducted for suspect materials (with the exception of assumed materials) in each homogeneous area. Samples were collected according to protocols included in Section 2.3.

• Photographs were taken of each homogeneous area.

• Bulk samples were submitted to a National Voluntary Accreditation Program-accredited laboratory for polarized light microscopy analysis (PLM) and transmission electron microscopy (TEM) analysis. The laboratory analyzed each sample submitted, with the following exception: when the first positive sample from a given sample set was identified, the remaining samples collected of that material were not analyzed. At that point, the sample material was considered an

ACM.

2.1 HOMOGENEOUS AREA DESIGNATIONS

Suspect ACMs were grouped into homogeneous sampling areas. AHERA defines a homogeneous area as an area of surfacing material, thermal system insulation material, or miscellaneous material that is uniform in texture and color, date of material application, and appears identical in every other respect. Materials installed at different times are

Building 650 Page 3 considered different sampling areas. If there is any reason to suspect that particular materials might be different (even though they appeared uniform), they are assigned to separate sampling areas. For example, material in different wings of a building, or in special areas such as locker rooms are assigned to separate sampling areas, unless there was reason to believe that the material was identical throughout each area. Although floor tile and mastic are sampled together, they are analyzed and results were reported separately (per AHERA). Drywall and drywall joint compound are sampled, analyzed, and reported together (per AHERA).

2.2 DELINEATION OF HOMOGENEOUS AREAS

During the detailed survey, the location of each homogeneous material, a material description, the quantity of material, and the condition of the material were documented.

An assessment of known or assumed ACMs was performed and the materials were classified using priority levels defined by AHERA. A log of the photographs is presented in Appendix B. Photographs of each of the homogeneous areas are presented in Appendix C

2.3 SAMPLING STRATEGIES

The asbestos inspection was conducted by an accredited asbestos building inspector (copies of personnel certifications are presented in Appendix D). The inspection consisted of a detailed visual survey of surfacing materials, thermal systems insulation, and miscellaneous materials throughout the building.

The AHERA regulations categorize ACM into three types: thermal system insulation (TSI), surfacing material, and miscellaneous material. TSI typically includes materials applied to pipe fittings (valves, tee’s, etc.); tanks, boilers, breeching, flues and stacks, turbine jackets, ducts, or similar applications; and mechanical or structural components to prevent heat transmission or condensation. Surfacing materials typically include spray or trowel applied fireproofing and acoustical finishes. Miscellaneous materials typically include items such as gasket materials, vinyl asbestos floor tile, ceiling tiles, adhesives, mastics, and small amounts of packing or caulking materials.

During the inspection, ACMs were grouped into homogeneous materials and sampling plans were developed. Components of the inspection included the identification of homogeneous suspect materials on a room-by-room basis. A drawing of the facility identifying each of the rooms surveyed is presented as Drawings B28, B31, and B34. Areas from which samples were to be obtained were also identified during this task. Sample locations for each floor are identified in Drawings B29, B32, and B35. Collection and analysis of bulk samples was subsequently performed to confirm whether or not the suspect materials contained asbestos.

Building 650 Page 4

During the inspection, the location of the homogeneous materials, a material description, the quantity of material, and the condition of the material was documented. The bulk sample log (presented in Appendix E) lists the sample number, location, description of the sample, and the asbestos content.

An assessment of known or assumed ACM was performed and the materials were classified using priority levels defined by AHERA. Homogeneous suspect ACM were, for the purposes of this survey and assessment and as outlined in the AHERA sampling protocol, placed into the four materials type categories, as described below. AHERA sampling protocol specifies sampling procedures for each material type:

Friable Surfacing Material

Friable surfacing material was sampled as follows:

1. At least three bulk samples from each homogeneous material that is 1,000 square feet or less.

2. At least five bulk samples from each homogeneous material that is greater than 1,000 square feet but less than or equal to 5,000 square feet.

3. At least seven bulk samples from each homogeneous materials that is greater than 5,000 square feet.

Thermal System Insulation

Thermal system insulation was sampled as follows:

1. In a randomly distributed manner, at least three bulk samples from each homogeneous materials of thermal system insulation that was not assumed to contain asbestos.

2. At least one bulk sample from each homogeneous material of patched thermal system insulation that was not assumed to contain asbestos.

3. In a manner sufficient to determine whether the material was asbestos-containing or not, a minimum of three (3) bulk samples were collected from each insulated mechanical system, that was not assumed to be asbestos-containing, where cement or plaster was used on fittings such as tees, elbows or valves.

4. Bulk samples were not collected from any homogeneous material where the inspector determined that the thermal system insulation was fiberglass without fiberglass tar paper wrap, foam glass, rubber or other non-ACM.

Building 650 Page 5

Miscellaneous Materials

In a manner sufficient to determine whether a material was or was not asbestos-containing, three (3) bulk samples were collected from each area of homogeneous friable miscellaneous material that was not assumed to be asbestos-containing.

Non-Friable Suspected ACM

Bulk samples were collected from any non-friable homogeneous material in a manner sufficient to determine whether the material was asbestos-containing or not and samples were collected in discreet locations.

At the direction of Yokota AB Environmental Flight, inaccessible areas of the building, roof and roofing materials, fire door interior insulating materials, and interior ductwork lining insulation were not sampled in order to protect the integrity of the materials.

Bulk sampling procedures are as follows:

• A small area is wetted where the sample will be collected

• Small pieces of the material were collected in a discreet location

• The sample is placed into a sample container and labeled with a unique ID number

• Duct tape is securely placed over the sample locations on mechanical insulation

• The sampled areas are wiped clean

• Continue sampling and documenting samples

After completion of the field sampling activities, the bulk samples were transported to MACS Lab, Inc (MACS Lab). MACS Lab participated in the analysis of standard bulk samples through the National Voluntary Laboratory Accreditation Program (NVLAP) accreditation #101948-0. Copies of the laboratory certification are presented in Appendix F.

The examination for the presence and identification of asbestos fibers in bulk samples was performed using crossed PLM and a dispersion staining particle identification technique using an Olympus BH-2 polarizing microscope at magnifications of 100X in accordance with USEPA, Method for the Determination of Asbestos in Bulk Building Materials, EPA 600/R-93/116 (July 1993). Bulk 12” x 12” vinyl floor tile samples were analyzed using the TEM method with a Hitachi H 600 TEM microscope at a magnification of 20,000X.

Building 650 Page 6

When analyzing single component asbestos samples, the laboratory may stop when the first positive sample is found within a particular homogeneous area sample set. The remaining samples, which were collected within the same homogeneous area, are assumed to also be asbestos-containing.

All sample results of zero percent (0%) indicate an asbestos-free material. All sample results between zero and one percent (0-1%) indicate non-regulated ACMs. All sample results greater than one percent (>1%) are regulated ACMs. Copies of the laboratory reports and laboratory analysis data sheets are presented in Appendix F.

2.4 CHAIN-OF-CUSTODY PROCEDURES

Chain-of-Custody forms (presented in Appendix F) were completed for samples involving analysis. The Chain-of-Custody form established a record of possession for all samples.

Anyone handling samples, prior to and including laboratory reception, signed the Chain-of- Custody form. All blanks on the form are completed. If a blank is not applicable to a project, it is crossed out. This indicates that a blank was considered and not overlooked.

Information on the Chain-of-Custody forms includes:

• Client address, contact and telephone number

• Authorizer address, contact and telephone number, if different from the client

• To whom preliminary results should be provided

• To whom the report should be addressed and provided

• Client references, such as project location, client project number, or client purchase order number

• The type of analysis required

• The turnaround time required

• All appropriate sampling information

• Condition of sample package and seals

• Date, time and mode of receipt

• Signature of person relinquishing samples

• Signature of employee receiving samples

Building 650 Page 7

The Chain-of-Custody form data is reviewed for completeness at the laboratory. The sampling data is also compared to the sample container labels for completeness.

2.5 ASSESSMENT LOGIC

A physical assessment of each identified homogeneous area of suspect ACM was conducted to determine what, if any, damage the area has sustained, and to assess whether the material should be classified as either friable or non-friable. The homogeneous areas were observed and touched to determine friability, physical condition, potential for disturbance, and Hazard Rank. A decision tree and explanation of hazard ranks is included in Appendix G.

Friable is defined by the US EPA as any material than can be crumbled, pulverized, or reduced to powder by hand pressure, when dry. This may also include previously non-friable material that becomes broken or damaged. Based on this definition of friability, the inspector ascertained if a material was friable by physically touching each material surface.

Suspect ACMs are also characterized as being in good condition, damaged, or significantly damaged. A material that is in good condition is a material with no visible damage or deterioration or showing only very limited (less than one percent) damage or deterioration.

Damaged materials exhibiting water stains, gouges, punctures, or mars on less than ten percent (<10%) overall, or less than 25 percent (<25%) localized of the surface area.

Significantly damaged material exhibits deterioration, water stains, gouges, punctures, or mars on greater then ten percent (>10%) overall or greater than 25 percent (>25%) of the localized surface area.

Following this field procedure, the suspect ACMs were classified according to the Classification for Hazard Potential. Rankings of potential hazards range from seven (most hazardous) to zero (least hazardous). Each suspect ACM was assigned a hazard rank based upon the material’s conditions and the disturbance potential as presented in the following table:

HAZARD RANKING CATEGORIES

Hazard Ranking Material Condition Disturbance Potential 7 Significantly Damaged Any 6 Damaged Potential for Significant Damage 5 Damaged Potential for Damage 4 Damaged Low Potential for Damage 3 Good Potential for Significant Damage 2 Good Potential for Damage 1 Good Low Potential for Damage 0 Non-ACMs N/A

Building 650 Page 8

The hazard rankings for this detailed survey have been conducted on a “room-by-room” or “area” basis. Therefore, hazard rankings are specific to the material condition and potential for disturbance in that particular area. The response actions typically associated with each hazard rank are presented in the table below.

RESPONSE ACTIONS BASED ON HAZARD RANK

Hazard Rank Response Action 7 Evacuate or isolate area if needed. Abate or repair the ACM. O&M Program required.

6 Evacuate or isolate the area if needed. Abate or repair the ACM. O&M Program required.

0 Inventory for future use.

2.6 OPERATIONS AND MAINTENANCE

As discussed in Section 2.5, materials classified with a hazard rank of 1 through 3 can be managed in place through proper operation and maintenance. The AHERA program specifies specific criteria for creation of an Operation and Maintenance (O&M) Program that could be applied to the Yokota AB. In general, the AHERA Program specifies:

• Notification and labeling procedures

• General awareness and worker training

• Medical surveillance of workers

• Maintenance procedures and systems

• Renovation and remodeling procedures

• Emergency response procedures

• Periodic re-inspection to look for damage or deterioration

• Recordkeeping

During our inspection, ACM identified was generally found to been in good condition.

Significant data was also present, demonstrating that current systems in place by Yokota personnel were effectively managing the base ACM. It is recommended that base personnel review the AHERA regulations found in 40 CFR 763, Subpart E to make sure the Yokota O&M program is complete. The information in this survey should be incorporated into the base program.

• Mastic adhering 5” gray vinyl baseboard

• Mastic adhering 5” tan vinyl baseboard

• Mastic adhering 5” older black vinyl baseboard

• Mastic adhering 5” blue vinyl baseboard

• Yellow mastic adhering 5” gray vinyl baseboard (painted brown)

• Mastic adhering 5” light brown vinyl baseboard

• Mastic adhering 3” dark gray plastic baseboard

• Mastic adhering 5” gray/brown vinyl baseboard

• Mastic adhering 5” newer black vinyl baseboard

• 2”-4” pipe elbow insulation

• 4”-8” pipe elbow insulation

• 8”-12” pipe elbow insulation

• Duck gasket

• Gray putty

• Beige pipe seal

• Black putty

• 12” x 12” beige vinyl floor tile with blue markings

• 12” x 12” white vinyl floor tile with brown and purple markings

• 12” x 12” gray vinyl floor tile with blue markings (replacement tiles)

Building 650 Page 13

At the time of the inspection, all of the vinyl baseboard mastic’s (listed above), the gray and black putty, and the beige pipe seal appeared to be in good condition and had a low potential for damage resulting in a Hazard Ranking of 1 for these materials. In this condition, these materials do not constitute a hazard. However, any disturbance of the materials such as cutting, drilling, or abrading may release fibers into the air. In order to facilitate the proper management and maintenance of these materials, MWH recommends that they be included in an O&M Plan.

The 12” x 12” beige vinyl floor tile with blue markings, the 12” x 12” white vinyl floor tile with brown and purple markings, and the 12” x 12” gray vinyl floor tile with blue markings (replacement tiles) appeared to be in good condition at the time of the inspection and had a potential for damage resulting in a Hazard Ranking of 2 for these materials. The floor tiles that were underneath carpeting had a low potential for damage and resulted in a Hazard Ranking of 1. In this condition, these materials do not constitute a hazard. However, any disturbance of the materials such as cutting, drilling, abrading or damaging the floor tiles may release fibers into the air. In order to facilitate the proper management and maintenance of these materials, MWH recommends that they be included in an O&M Plan.

At the time of the inspection, the 2”- 4”, 4”- 8” and 8”- 12” pipe elbow insulation were observed to be in good condition and had a potential for damage resulting in a Hazard Ranking of 2 for these materials. In this condition, these materials do not constitute a hazard. However, any disturbance of the materials such as cutting, drilling, abrading or damaging the pipe elbow insulation may release fibers into the air. In order to facilitate the proper management and maintenance of these materials, MWH recommends that they be included in an O&M Plan.

At the time of the inspection, the duct gaskets (next to unit EEF-1 in Room 209) were observed to be in good condition and had a high potential for damage resulting in a Hazard Ranking of 3 for this material. In this condition, this material does not constitute a hazard.

However, any disturbance of the material such as damaging the gaskets during replacement may release fibers into the air. In order to facilitate the proper management and maintenance of these materials, MWH recommends that they be included in an O&M Plan.

It is important to note that the roof, roofing materials and miscellaneous materials such as fire door interior insulation and interior ductwork lining insulation, if present in the building, were not sampled as discussed in Section 2.3. These items should be tested if major roof work, remodeling, or building demolition is planned.
