SUMMARY OF CHANGES

The purpose of this amendment is to: Block 14 (Description of Amendment/Modification):

1. Remove Section L(D)(3) from Section L. This part details the requirement for joint venture agreements to be received and approved by the SBA prior to the proposal due date. This information is no longer correct and this part of Section L has been amended to reflect this.

All other terms and conditions remain unchanged.

Section L

PROPOSAL PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts; Part I - Summary and Miscellaneous Data, Part II - Price Proposal and Part III - Past Performance Proposal.

B. Specific Instructions:

1. Submit one electronic proposal by emailing john.jeffrey.1@us.af.mil and tyler.norris.1@us.af.mil requesting a drop-off request via DoD SAFE (Secure Access File Exchange) Service. All proposals shall be submitted using DoD SAFE. Instructions on how to use DoD SAFE can be found at https://safe.apps.mil/. All questions are due by 1400 AST on 16 February 2021.

2. PART I - SUMMARY AND MISCELLANEOUS DATA.

(a) The offeror shall provide a concise summary and master table of contents of the entire proposal.

(b) Complete blocks 13, 15, 16, 17 and 18 of the RFP Section A, SF1442. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. These sections constitute the model contract.

Note: Attachments J-11 through J-14 are part of the solicitation only and will not be applicable to resulting contract.

(c) Complete the necessary fill-ins and certifications in Sections C through J. Section K shall be returned in its entirety. For Sections C through J, the offeror shall submit only those pages that require a fill-in.

(d) Provide a copy of the letter from the Small Business Administration (SBA) showing proof of Section 8(a) status.

(e) If none proposed, submit an Affirmation of no Exceptions, Deviations, or Waivers.

3. PART II-PRICE PROPOSAL.

(a) This part shall contain a completed Section B (Supplies or Services and Prices/Costs) and a Task Order

Request for Proposal for the first project to be awarded under this IDIQ. The Task Order Request for Proposal will not be used in evaluation for the base IDIQ but used to meet the requirements of AFFARS 5316.504(a)(2) and award the first Task Order under resultant contract.

(b) Insert proposed unit prices and extended amounts in Section B for each Contract Line Item Number (CLIN), including all seven year periods. The extended amount must equal the unit price multiplied by the number of units. It is imperative that mathematical calculations are correct; particular care should be taken to ensure the amounts match when using a spreadsheet program, such as Microsoft EXCEL. The unit prices and extended amounts are limited to two decimal places. The line item schedule proposed prices shall be submitted in electronic format. Offerors shall fill in the unit prices on the Government provided worksheet (see Attachment J- 9). The worksheet will calculate totals. Offerors are responsible for validating all totals. The Government bears no responsibility if there are calculation errors in the final proposal submission.

(c) Extreme care should be exercised to ensure that no price data of any kind are included in any other part of the proposal.

(d) Solicitation Attachment 9 Price Schedule is included as excel spread sheet containing base year through year seven for Roadway (Tab 1) and Airfield (Tab 2) CLINs. Insert contractor bid unit price onto each CLIN and Attachment 9 spread sheet should calculate a total amount bid price.

(e) After completion of Tab 1 and Tab 2, complete Tab 3, Seed Project by filling in necessary line items to complete Project FXSB 96-2103, Repair Pavement at Building 5257 (See Attachment 9, Tab 3). A site visit for this project will be held on 05 February 2021 at 1000.

(f) Solicitation Attachment 8 Economic Price Adjustment Schedule (EPA), is included containing EPA CLIN for asphalt and fuel adjustment. Insert contractor information for each column onto each CLIN.

4. PART III- PAST PERFORMANCE PROPOSAL.

(a) Quality and Satisfaction Rating of Contracts: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) that demonstrates customer satisfaction with overall job performance and/or quality of completed and/or ongoing contracts. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems, such as cost overruns, extended performance periods, numerous warranty calls, etc. (See Sec M-B)

(b) Performance Surveys/Questionnaires: Past Performance Questionnaires (PPQs) shall be used by the offeror in obtaining and providing past performance information. The government requires the offeror send out a PPQ to each of the Points-of-Contact (POCs) identified in the Past Performance Proposal (paragraph (c) below).

The offeror shall send out the PPQ (Attachment J-12), using the Past Performance Evaluation Cover Letter (Attachment J-11), to each of its references.

THE RESPONSIBILITY TO SEND OUT THE PAST PERFORMANCE QUESTIONNAIRE RESTS SOLELY

WITH THE OFFEROR.

Once the questionnaires are completed by the POCs, the information contained therein will be considered source selection sensitive and will not be released outside the Government. The POCs shall forward their completed questionnaires directly to the government - NOT BACK TO THE OFFEROR.

PPQs shall be sent directly from your reference to this office marked to the attention of Mr. John Jeffrey and SrA Tyler Norris via e-mail to john.jeffrey.1@us.af.mil and tyler.norris.1@us.af.mil.

(c) Past Performance Information: Provide up to ten past performance efforts of the most relevant contracts performed for any customer. (See Section M, paragraph B). Furnish the following information for each contract listed:

(1) Company/Division name

(2) Product/Service

(3) Contracting Agency/Customer

(4) Contract Number

(5) Contract Dollar Value

(6) Period of Performance

(7) Verified, up-to-date name, mailing and e-mail addresses, and telephone number of the contracting officer (Point-of-Contract).

(8) Comments regarding compliance with contract terms and conditions

(9) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.

(10) Include rationale supporting your assertion of relevance and identify aspects (scope, magnitude of effort, and complexity) of the contracts deemed relevant and how they relate to the proposed effort.

If an offeror has no past performance history of relevance, the offeror must state affirmatively it possesses no past performance history of relevance.

(d) The evaluation of past performance information (i) will not take into account past performance regarding subcontractors that will perform major or critical aspects of the requirement when such information is relevant to this acquisition. (ii) will take into account past performance information regarding sister companies, and (iii) will pay particular notice on airfield projects. Major or critical aspects of the requirement are primarily airfield pavements (runways, taxiways, aprons, airfield roads, hangar floors. See Attachment 2) and road pavements (roads, parking lots, driveways, bike paths, golf cart paths. See Attachment 3).

(e) Relevant Contracts: Relevant past performance information must demonstrate airfield projects. Submissions may be for individual projects but entire programs are preferred. If you currently have a similar award to this program, you may submit a representative sample of individual projects in addition to providing a brief narrative for the overall relevancy of the program.

(f) Supplemental Worksheet. Provide data in Section A for up to ten (10) contracts/projects completed or currently in-progress within the last three (3) years (from RFP issuance date). Provide annual contract/project portfolio dollar value for the last three (3) years in Section B. This data will be captured by completing Attachment J-13, Past Performance Supplement. Instructions for completing each section are provided in the attachment.

(g) Future Workload Estimates. To aid offerors indetermination of relevancy, the following future workload estimates are provided:

(1) The Government has a high estimate of anywhere from $18M to $22M annually in total volume over the next seven (7) years.

(2) The Government's high estimate for the number of projects per year is 10, with an estimate of 60% being airfield projects.

*Please not that all future workload data listed above is only an estimate to aid in your proposal development.

There is no implied guarantee of the quantity and volume of projects. Please refer to Sections B and I of the solicitation for ordering limits and maximum order and contract values.

(h) If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past contracts of relevance.

C. Documents submitted in response to this RFP must be consistent with the following:

(1) Requirements of the solicitation (CLINs and SOW) and government standards and regulations pertaining to the SOW.

(2) Evaluation Factors for Award in Section M of this solicitation.

(3) If a Part in a proposal exceeds the specified page limit cited in the Part Content, the excess pages may not be evaluated.

(4) Format for proposal Parts I, II, and III shall be as follows:

(a) Each Part of the proposal shall contain a Table of Contents, Cross References, and the List of Tables and Drawings (if applicable). Cover pages, Table of Contents, and Tabs will not be counted against the page limits.

However, pages depicting tables, charts, graphs and figures will count toward the page limit.

(b) The proposals will be formatted to 8 1/2" x 11" pages, except for fold-outs used for charts, tables, or diagrams, which may not exceed 11" x 17".

(c) All documents shall be single-spaced. Spacing characteristics are applicable to charts and graphs.

(d) Texts shall not be less than 11 font size. Size of print is applicable to charts and graphs.

(e) Page margins shall be a minimum of 1 inch top, bottom, and each side.

(f) Price data of any kind shall only be present in the Price Proposal Part. The Price Part shall be submitted in Excel spreadsheet format.

(g) Each Part shall be complete in itself and shall not reference information contained in other Parts. Page 1 of a Part is defined as the first page after the Table of Contents. All pages shall be numbered except for excel spreadsheets.

(h) Elaborate formats and color presentations are not desired or required.

(i) All files must be in Microsoft Office format and must be able to be opened on Government computers. The same format used for the initial proposal shall be used for the final proposal resulting from any discussions and /or updates, if applicable. Offerors are advised to submit proposals in time to verify that the Government received the files and that the files can be opened and read. The Government is not responsible if files submitted are not readable or are in the wrong format. Replacement files will not be accepted after the deadline for submittal of proposals.

All spreadsheets must be unlocked and all formulas must be included and readable by the Government.

D. General Information:

(1) Late proposals will be processed in accordance with FAR 52.215-1(c)(3), "Submission, modification, revision and withdrawal of proposals."

(2) Offerors are cautioned that JBER has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance.

A site visit for the Seed Project will be held at Joint Base Elmendorf Richardson (JBER) on 05 February 2021 to start at 10:00 AM Alaska Time (AKT) at Building 5257. All costs associated with attending the site visit shall be borne by the offerors. Prospective offerors are urged to attend the site visit. If a contractor is unable to attend, there will be no future site visits scheduled.

All contractors, including those that have access to Joint Base Elmendorf-Richardson (JBER), wishing to participate in the site visit shall provide the following information for individuals attending the scheduled site visit:

Please go to the Boniface Visitor Gate to obtain a visitor pass, if needed. Please allow some delay while obtaining the pass. The following information will be required when getting your pass:

(a) License

(b) Registration

(c) Proof of Insurance

Note: Currently, the JBER Wing Commander has declared a Public Health Emergency. JBER base access has been limited to those with a bona fide need requirement. The following are the new conditions of JBER Access for all persons:

(a) Base access is granted for those deemed mission essential and has current base passes.

(b) If new base access passes are required, it must be approved by Unit Commanders in the grade of O-4 and above or comparable director level supervisors. This may cause some delays in new base access requests. If base passes for new individuals are needed, this does not mean you will not receive them, it means it may take longer than usual for access.

(c) Mask Wear is mandatory on JBER for all persons. Exceptions: an individual as the lone operator of a vehicle or when eating or drinking.

(d) Dining or drinking off base: "No person is permitted to enter JBER after having entered a business primarily engaged in preparing and serving food or alcoholic beverages for on-site consumption. This includes but is not limited to, restaurants, bars, breweries, pubs and nightclubs."
