APPENDIX I
SPECIAL REQUIREMENTS FOR DEPLOYMENT SUPPORT

1.0 Contract performance at the deployed locations is subject to DoDI 3020.41, Operational Contract Support. Services performed at the deployed locations under this contract are designated as “mission essential.”
2.0 Contingency Plan
2.1 Provide the Government with a Contingency Plan for the retention and replacement of contractors who provide essential contractor services; i.e., those in deployed locations IAW Appendix B. This plan shall provide the Government with ‘reasonable assurance’ of continuation of services during crisis situations.
3.0 Pre- and Post-Deployment Processing
3.1 Have a formal written deployment process that includes all required actions personnel must complete prior to deployment (i.e. training, travel, medical clearance, etc.). All deploying contractor personnel must be medically, dentally, and psychologically fit for deployment IAW DODI 3020.41, Enclosure 3. Additionally contractor personnel must be able to tolerate the environmental and operational conditions of the deployed site.
3.2 Ensure all contingency contractor personnel complete all medical and dental requirements during deployment processing, including a medical and dental assessment within the previous 12 months, completion of all required immunizations, and completion of a tuberculosis skin test within three months prior to deployment.
3.3 Arrange for collection and storage of a DNA reference specimen for all deploying personnel. Samples will be stored and accounted for IAW DoDI 3020.41, Enclosure 3, Paragraph 8.
3.4 Contractor personnel shall not deploy with any of the conditions listed in DoDI 3020.41, Enclosure 3, Paragraph 10 without an approved waiver from the Government Program Manager or Contracting Officer, and Combatant Command medical authority. Waiver requests shall be submitted through the Government Program Manager or Contracting Officer.
Waivers will not normally be approved without a full explanation of the individual’s condition and why other personnel who meet medical standards could not be deployed to fulfill the deployed duties.
3.5 Obtain prescriptions for military-specific vaccinations and immunizations required by DoDI 3020.41, anti-malarials, and eyeglass inserts for chemical protective masks for all deploying personnel. The Government will fill these prescriptions IAW the PWS, Section C, Paragraph 4.2. Contractor shall present a letter on company letterhead stating the individual is scheduled to deploy in support of a Government contract. Include contract number, full name, inclusive dates, location of deployed sites, and specific support required to the military medical facility. All other vaccinations and immunizations are the responsibility of the contractor.

3.5.1 Personnel shall present completed Standard Form 600, Smallpox Vaccination Initial Note, and Smallpox Vaccine Continuation Note as required, signed by the contractor’s medical provider to the Immunization Clinic prior to receiving the smallpox vaccination.
3.6 Deployed contractor personnel shall have two replacement pairs of glasses. (Note: Use of contact lenses in the deployed environment is not recommended.) Contingency Contractor Personnel shall provide their own comfort items, such as hearing aid batteries, sunglasses, insect repellant, sunscreen, etc.
3.7 Certify in writing to the Government Program Office (GPO) that all individual deployment requirements have been completed and that personnel are medically fit to deploy per DoDI 3020.41. Certification shall be completed prior to requesting Letters of Authorization (LOA) approval from the GPO.
4.0 Legal Issues
4.1 The contractor shall ensure that all deployed personnel:
4.1.1 Comply with theater orders and applicable directives, laws and regulations.
4.1.2 Maintain discipline.
4.1.3 Use the contracting officer or designee as the liaison between the deployed commander and the contractor.
4.1.4 Follow the lawful directions of the ranking military commander in emergency situations. The contractor shall not perform inherently governmental functions.
4.1.5 Carry their LOA on their person at all times.
