5) PWS Ref: 3.1.4.3 The contractor shall report chemical use to the CO at the end of each month on DD Form 1532 or computer generated equivalent if used.
5) PWS Ref: 3.3.2.1, 3.3.2.3 This report shall include Communications-Electronics monthly PMI schedule and any unscheduled corrective maintenance actions. This report shall include the status of all outstanding unscheduled corrective maintenance tasks and the status of identified maintenance requirements, work costs that includes materials, labor, shipping, and all other associated costs. 4) Electronic format shall be compatible with Microsoft Office via the ACC AMIC LAN electronic mail system.

i) Job Control Identifier and AF 332 number or previously closed JCI/AF 332 which an additional charge was required for work or costs.
ii) Description and status of all Corrective Maintenance (CM) tasks, project requirements performed during the month, description and status of all outstanding CM tasks, project requirements, and CM tasks/project requirements not accomplished in the previous month.
iii) Preventive Maintenance task not accomplished or deferred per the annual maintenance schedule and reason for deviation.
iv) Cost of completed work.
v) All associated purchase orders, requisitions, status and dates.
vi) Completion dates.
vii) This report shall also include any pesticide/herbicide usage as a CM task with appropriate data fields as found in the DD Form 1532.
viii) Provide monthly water and electrical readings.

5) PWS Ref: 3.1.2.1.1, 3.3.2.1 The contractor shall provide a C-E Maintenance Control Plan (MCP) that includes processes and procedures for controlling the C-E maintenance production schedule which shall include an annual schedule for C-E scheduled maintenance identified by daily, weekly, monthly, quarterly, and semi-annual and annual PMI schedule.
5) PWS Ref: 3.4.3 "Maintenance Control Plan (MCP)" The MCP shall include a master listing of all PM tasks with task references for all CE, RP, RPIE, RPSE, as well as annual PM schedule. PM tasks in the annual schedule shall be categorized by site, facility, system, equipment and frequency of inspection. The MCP PM tasks and corrosion control program shall be implemented IAW technical orders, commercial standards, applicable manuals, and operational guidelines. The contractor shall extend and comply with the maintenance schedule in place from the previous contract until the initial submittal and approval of the MCP. The MCP shall include scheduling, performing, and documenting Routine work orders.

5) PWS Ref: 3.3.2.9 CSIRs provide both a historical record of communications and information systems installation and a living document that reflects how the system is modified over time. They are used for planning, programming, and supporting communications and information systems operations, maintenance, integration, and engineering and installation (EI) efforts. To be effective, the installation records should show communications and information system equipment, interconnecting cabling, and assigned circuitry as installed for a particular facility, building, or location. Accurate CSIRs support communications and information systems life-cycle management. The communications and information planners and CSIR manager must ensure records are accurate and minimize duplication. CSIRs are required for all Air Force-owned fixed communications and information systems, regardless of who maintains or operates the systems.

4) Electronic format shall be compatible with Microsoft Office via the ACC AMIC LAN electronic mail system. CDRL shall be provided to the Government on a CD along with a paper copy bound by either comb or spiral.
5) PWS Ref: 3.4.3.1, 3.4.6 The report will be organized by site with an introductory overview of issues/concerns from a CE/real property viewpoint and also a spreadsheet of current status of all power generation equipment. This status will include hours operated since last overhaul for planning purposes. The report will also include the contractor's list of real property and RPIE as an appendix so that there will be a single document that provides a single overview of contractor maintained facilities. The contractor shall provide an annual facility survey and inspection report detailing facility and system deficiencies in the structural, electrical, mechanical, and environmental areas. The survey and inspection report shall, at a minimum include the status of current year projects, condition of facilities (interior and exterior), slabs and foundations, roads, grounds, drainage systems, sanitary sewage system, water supply, electrical system, POL system, other utility systems, power generation equipment, fire protection/detection equipment, and physical security aides. The Survey/Inspection report shall include photographs of the above items at each site in sufficient detail to document all deficient conditions as well as overall condition of the sites.
These photos will be organized into a single file for viewing along with individual photo files in a supporting subfolder on the submitted CD.

5) PWS Ref: 3.6.1.2 The contractor shall annually inventory and report on GFP. The inventory report shall be broken down into sections according to type of property (i.e.
consumable, equipment, reparable, ADPE, TMDE, etc.) and forwarded to the government for evaluation. Inventory losses and gains shall be identified and explained on an inventory adjustment document. The first delivery of the report shall be 45 days after contract start.
At a minimum, the report shall include the following fields:
• P/N
• Stock Number (if applicable)
• Nomenclature
• Manufacturer Serial Number
• Model Number
• Unit of Issue
• Quantity
• Unit Price
• Location
• Shelf Life Code
• Repair Code (Equipment Category)
• Equipment Serial Number
• Stock Level Quantity
• Date Of Last Inventory
• Date Property Assigned to Inventory List
11) The data contained in the report shall be current through 1 July of each subsequent year.
12) The first delivery of the report shall be NLT 15 November 2016.
13) Inventory listings shall be by site and types of property (i.e., consumable, nonexpendable, equipment, or repairable). Copy of inventory shall be forwarded to ACC AMIC/LG NLT 1 July of each fiscal year.

5) PWS Ref: 3.6.2 The contractor shall report material prices to the government. The data contained in the report shall list the previous month's purchases. For each equipment purchase over $3,000, the contractor shall receive approval from the CO prior to purchasing. The CO will coordinate and validate equipment purchase pricing with COR before approving the purchase. All contractor-acquired supplies shall become government property.
