The purpose of this amendment is to correct Section L paragraph L-2.2.2, changing the reference to the Publications and Forms appendix from Appendix G to Appendix F, update PWS Appendix M to add country clearance lead times for Guatemala and Panama, and add or update the follow ing required clauses: FAR 52.232-40, FAR 52.244-6, DFARS 252.211-7003, and DFARS 252.225-7006.

252.211-7003 ITEM UNIQUE IDENTIFICATION AND VALUATION (DEC 2013)

(a) Definitions. As used in this clause'

Automatic identification device means a device, such as a reader or interrogator, used to retrieve data encoded on machine-readable media.

Concatenated unique item identifier means--

(1) For items that are serialized within the enterprise identifier, the linking together of the unique identifier data elements in order of the issuing agency code, enterprise identifier, and unique serial number within the enterprise identifier; or

(2) For items that are serialized within the original part, lot, or batch number, the linking together of the unique identifier data elements in order of the issuing agency code; enterprise identifier; original part, lot, or batch number;

and serial number within the original part, lot, or batch number.

Data Matrix means a two-dimensional matrix symbology, which is made up of square or, in some cases, round modules arranged within a perimeter finder pattern and uses the Error Checking and Correction 200

(ECC200) specification found within International Standards Organization (ISO)/International Electrotechnical

Commission (IEC) 16022.

Data qualifier means a specified character (or string of characters) that immediately precedes a data field that defines the general category or intended use of the data that follows.

DoD recognized unique identification equivalent means a unique identification method that is in commercial use and has been recognized by DoD. All DoD recognized unique identification equivalents are listed at http://www.acq.osd.mil/dpap/pdi/uid/iuid_equivalents.html.

DoD item unique identification means a system of marking items delivered to DoD with unique item identifiers that have machine-readable data elements to distinguish an item from all other like and unlike items. For items that are serialized within the enterprise identifier, the unique item identifier shall include the data elements of the enterprise identifier and a unique serial number. For items that are serialized within the part, lot, or batch number within the enterprise identifier, the unique item identifier shall include the data elements of the enterprise identifier; the original part, lot, or batch number; and the serial number.

Enterprise means the entity (e.g., a manufacturer or vendor) responsible for assigning unique item identifiers to items.

Enterprise identifier means a code that is uniquely assigned to an enterprise by an issuing agency.

Government's unit acquisition cost means--

(1) For fixed-price type line, subline, or exhibit line items, the unit price identified in the contract at the time of delivery;

(2) For cost-type or undefinitized line, subline, or exhibit line items, the Contractor's estimated fully burdened unit cost to the Government at the time of delivery; and

(3) For items produced under a time-and-materials contract, the Contractor's estimated fully burdened unit cost to the Government at the time of delivery.

Issuing agency means an organization responsible for assigning a globally unique identifier to an enterprise (e.g., Dun & Bradstreet's Data Universal Numbering System (DUNS) Number, GS1 Company Prefix, Allied Committee

135 NATO Commercial and Government Entity (NCAGE)/Commercial and Government Entity (CAGE) Code, or the Coded Representation of the North American Telecommunications Industry Manufacturers, Suppliers, and

Related Service Companies (ATIS-0322000) Number), European Health Industry Business Communication Council

(EHIBCC) and Health Industry Business Communication Council (HIBCC)), as indicated in the Register of Issuing

Agency Codes for ISO/IEC 15459, located at http://www.nen.nl/web/Normen-ontwikkelen/ISOIEC-15459-Issuing-

Agency-Codes.htm.

Issuing agency code means a code that designates the registration (or controlling) authority for the enterprise identifier.

Item means a single hardware article or a single unit formed by a grouping of subassemblies, components, or constituent parts.

Lot or batch number means an identifying number assigned by the enterprise to a designated group of items, usually referred to as either a lot or a batch, all of which were manufactured under identical conditions.

Machine-readable means an automatic identification technology media, such as bar codes, contact memory buttons, radio frequency identification, or optical memory cards.

Original part number means a combination of numbers or letters assigned by the enterprise at item creation to a class of items with the same form, fit, function, and interface.

Parent item means the item assembly, intermediate component, or subassembly that has an embedded item with a unique item identifier or DoD recognized unique identification equivalent.

Serial number within the enterprise identifier means a combination of numbers, letters, or symbols assigned by the enterprise to an item that provides for the differentiation of that item from any other like and unlike item and is never used again within the enterprise.

Serial number within the part, lot, or batch number means a combination of numbers or letters assigned by the enterprise to an item that provides for the differentiation of that item from any other like item within a part, lot, or batch number assignment.

Serialization within the enterprise identifier means each item produced is assigned a serial number that is unique among all the tangible items produced by the enterprise and is never used again. The enterprise is responsible for ensuring unique serialization within the enterprise identifier.

Serialization within the part, lot, or batch number means each item of a particular part, lot, or batch number is assigned a unique serial number within that part, lot, or batch number assignment. The enterprise is responsible for ensuring unique serialization within the part, lot, or batch number within the enterprise identifier.

Type designation means a combination of letters and numerals assigned by the Government to a major end item, assembly or subassembly, as appropriate, to provide a convenient means of differentiating between items having the same basic name and to indicate modifications and changes thereto.

Unique item identifier means a set of data elements marked on items that is globally unique and unambiguous. The term includes a concatenated unique item identifier or a DoD recognized unique identification equivalent.

Unique item identifier type means a designator to indicate which method of uniquely identifying a part has been used. The current list of accepted unique item identifier types is maintained at http://www.acq.osd.mil/dpap/pdi/uid/uii_types.html.

(b) The Contractor shall deliver all items under a contract line, subline, or exhibit line item.

(c) Unique item identifier. (1) The Contractor shall provide a unique item identifier for the following:

(i) Delivered items for which the Government's unit acquisition cost is $5,000 or more, except for the following line items:

(iii) Subassemblies, components, and parts embedded within delivered items, items with warranty requirements, DoD serially managed reparables and DoD serially managed nonreparables as specified in Attachment Number ----.

(iv) Any item of special tooling or special test equipment as defined in FAR 2.101 that have been designated for preservation and storage for a Major Defense Acquisition Program as specified in Attachment Number ----.

(v) Any item not included in paragraphs (c)(1)(i), (ii), (iii), or

(iv) of this clause for which the contractor creates and marks a unique item identifier for traceability.

(2) The unique item identifier assignment and its component data element combination shall not be duplicated on any other item marked or registered in the DoD Item Unique Identification Registry by the contractor.

(3) The unique item identifier component data elements shall be marked on an item using two dimensional data matrix symbology that complies with ISO/IEC International Standard 16022, Information technology--International symbology specification--Data matrix; ECC200 data matrix specification.

(4) Data syntax and semantics of unique item identifiers. The Contractor shall ensure that--

(i) The data elements (except issuing agency code) of the unique item identifier are encoded within the data matrix symbol that is marked on the item using one of the following three types of data qualifiers, as determined by the

Contractor:

(A) Application Identifiers (AIs) (Format Indicator 05 of ISO/IEC International Standard 15434), in accordance with

ISO/IEC International Standard 15418, Information Technology--EAN/UCC Application Identifiers and Fact Data

Identifiers and Maintenance and ANSI MH 10.8.2 Data Identifier and Application Identifier Standard.

(B) Data Identifiers (DIs) (Format Indicator 06 of ISO/IEC International Standard 15434), in accordance with

ISO/IEC International Standard 15418, Information Technology--EAN/UCC Application Identifiers and Fact Data Identifiers and Maintenance and ANSI MH 10.8.2 Data Identifier and Application Identifier Standard.

(C) Text Element Identifiers (TEIs) (Format Indicator 12 of ISO/IEC International Standard 15434), in accordance with the Air Transport Association Common Support Data Dictionary; and

(ii) The encoded data elements of the unique item identifier conform to the transfer structure, syntax, and coding of messages and data formats specified for Format Indicators 05, 06, and 12 in ISO/IEC International Standard 15434, Information Technology-Transfer Syntax for High Capacity Automatic Data Capture Media.

(5) Unique item identifier.

(i) The Contractor shall--

(A) Determine whether to--

(1) Serialize within the enterprise identifier;

(2) Serialize within the part, lot, or batch number; or

(3) Use a DoD recognized unique identification equivalent (e.g. Vehicle Identification Number); and

(B) Place the data elements of the unique item identifier (enterprise identifier; serial number; DoD recognized unique identification equivalent; and for serialization within the part, lot, or batch number only: Original part, lot, or batch number) on items requiring marking by paragraph (c)(1) of this clause, based on the criteria provided in MIL-STD-

130, Identification Marking of U.S. Military Property, latest version;

(C) Label shipments, storage containers and packages that contain uniquely identified items in accordance with the requirements of MIL-STD-129, Military Marking for Shipment and Storage, latest version; and

(D) Verify that the marks on items and labels on shipments, storage containers, and packages are machine readable and conform to the applicable standards. The contractor shall use an automatic identification technology device for this verification that has been programmed to the requirements of Appendix A, MIL-STD-130, latest version.

(ii) The issuing agency code--

(A) Shall not be placed on the item; and

(B) Shall be derived from the data qualifier for the enterprise identifier.

(d) For each item that requires item unique identification under paragraph (c)(1)(i), (ii), or (iv) of this clause or when item unique identification is provided under paragraph (c)(1)(v), in addition to the information provided as part of the Material Inspection and Receiving Report specified elsewhere in this contract, the Contractor shall report at the time of delivery, as part of the Material Inspection and Receiving Report, the following information:

(1) Unique item identifier.

(2) Unique item identifier type.

(3) Issuing agency code (if concatenated unique item identifier is used).

(4) Enterprise identifier (if concatenated unique item identifier is used).

(5) Original part number (if there is serialization within the original part number).

(6) Lot or batch number (if there is serialization within the lot or batch number).

(7) Current part number (optional and only if not the same as the original part number).

(8) Current part number effective date (optional and only if current part number is used).

(9) Serial number (if concatenated unique item identifier is used).

(10) Government's unit acquisition cost.

(11) Unit of measure.

(e) For embedded subassemblies, components, and parts that require DoD unique item identification under paragraph (c)(1)(iii) of this clause, the Contractor shall report as part of, or associated with, the Material Inspection and Receiving Report specified elsewhere in this contract, the following information:

(1) Unique item identifier of the parent item under paragraph (c)(1) of this clause that contains the embedded subassembly, component, or part.

(2) Unique item identifier of the embedded subassembly, component, or part.

(3) Unique item identifier type.**

(4) Issuing agency code (if concatenated unique item identifier is used).**

(5) Enterprise identifier (if concatenated unique item identifier is used).**

(6) Original part number (if there is serialization within the original part number).**

(7) Lot or batch number (if there is serialization within the lot or batch number).**

(8) Current part number (optional and only if not the same as the original part number).**

(9) Current part number effective date (optional and only if current part number is used).**

(10) Serial number (if concatenated unique item identifier is used).**

(11) Description.

(12) Type designation of the item as specified in the contract schedule, if any.

(13) Whether the item is an item of Special Tooling or Special Test Equipment.

(14) Whether the item is covered by a warranty.

(e) For embedded subassemblies, components, and parts that require DoD item unique identification under paragraph (c)(1)(iii) of this clause or when item unique identification is provided under paragraph

(c)(1)(v), the Contractor shall report as part of the Material Inspection and Receiving Report specified elsewhere in this contract, the following information:

(f) The Contractor shall submit the information required by paragraphs (d) and (e) of this clause as follows:

(1) End items shall be reported using the receiving report capability in Wide Area WorkFlow (WAWF) in accordance with the clause at 252.232-7003. If WAWF is not required by this contract, and the contractor is not using WAWF, follow the procedures at http://dodprocurementtoolbox.com/site/uidregistry/.

(2) Embedded items shall be reported by one of the following methods--

(i) Use of the embedded items capability in WAWF;

(ii) Direct data submission to the IUID Registry following the procedures and formats at http://dodprocurementtoolbox.com/site/uidregistry/; or

(iii) Via WAWF as a deliverable attachment for exhibit line item number (fill in) ----, Unique Item Identifier Report for Embedded Items, Contract Data Requirements List, DD Form 1423.

(g) Subcontracts. If the Contractor acquires by contract any items for which item unique identification is required in accordance with paragraph (c)(1) of this clause, the Contractor shall include this clause, including this paragraph (g), in the applicable subcontract(s), including subcontracts for commercial items.

INSTRUCTIONS TO OFFERORS

L-1 SPECIAL NOTICE TO OFFERORS

L-1.1 The exclusive responsibility for Source Selection will reside with the Government. Each individual involved in this acquisition will execute a Non-Disclosure Agreement with ACC AMIC prior to having access to any proposals.

L-1.2 Proprietary information submitted in response to this solicitation shall be clearly marked as such and will be protected from unauthorized disclosure as required by Subsection 27 of the Office of Procurement Policy Act as amended (41 U.S.C.423) (hereinafter referred to as “the Act”) as implemented in the FAR. Any unmarked proprietary information will be considered releasable under the restrictions of the Freedom of Information Act.

L-1.3 Funds are not presently available for this effort. No award will be made until funds are available. The

Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the

Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.

L-2.2 Bidder’s Library.

L-2.2.1 The library will be available by appointment only from 23 September 2013 through the proposal closing date and the hours of operation are as follows: 8 a.m. - 11a.m. and 1 p.m. - 4 p.m., Monday through Friday, except on holidays. The library will be located at: 11817 Canon Blvd, Suite 501; Newport News, VA 23606. Only one offeror with no more than two (2) representatives per offeror may use the library at any given time. To schedule an appointment email the address listed in paragraph L-2.1 at least 7 business days in advance to arrange access. Title the e-mail “Bidders Library Appointment – Company Name” (example: “Bidders Library Appointment – ABC

Company”). One computer will be available in the Bidders’ Library. Offerors will not be allowed to remove any documents labeled For Official Use Only (FOUO) from this area. The Bidders’ Library does not contain any classified documents/materials. Offerors will be allowed to make copies, photograph, or scan documents except documents marked “FOUO”. A copier will be available; however the government will not provide paper.

L-2.2.2 None of the forms, regulations, manuals, Technical Orders, or other publications referenced in the solicitation will be issued with the solicitation. Documents cited within PWS, Appendix F – Publications and

Forms, annotated as being available electronically, are available via public domain at http://www.e-publishing.af.mil/ or http://www.dtic.mil/whs/directives with the exception of documents labeled “FOUO" in the title or publication description. L-2.3 Site Visit and Pre-Proposal Conference.

L-2.3.1 Site Visit Location. The site visit will be held from 18 – 22 November 2013 and will start with a visit to the current CONUS Warehouse in Melbourne, FL. After visiting Florida we will visit three sites in Colombia: Tres

Esquinas, Bogota and San Jose. The government will provide transportation from Bogota to the other sites in

Colombia and back to Bogota. The final visit will be to the SATCOM site in Key West, FL. A detailed agenda will be posted to FBO.

L-2.3.2 Site Visit Reservations. Please provide list of attendees for the site visit via email to the address listed in paragraph L-2.1 by 8 October 2013. Title the e-mail "Site Visit - Company Name" (example "Site Visit - ABC

L-2.3.3 Pre-Proposal Conference Location. The Government will host a pre-proposal conference to explain the requirements of this solicitation and respond to questions raised by offerors. The conference will be held in

Newport News, VA on 18 December 2013 from 1:00 to 3:00 pm. An agenda will be posted to FBO.

L-2.3.4 Pre-Proposal Conference Reservations. Please provide list of attendees for the pre-proposal conference via email to the address listed in paragraph L-2.1 by 16 December 2013. Title the e-mail "Pre-Proposal Conference -

Company Name" (example "Pre-Proposal Conference - ABC Company"). Due to space limitations, offerors shall limit the number of attendees to a maximum of three individuals. Please provide name, title, and contact information for all participants in the body of the email.

L-2.3.5 Questions. Submit all questions regarding the solicitation in writing via e-mail to the address listed in paragraph L-2.1. During the pre-proposal conference, the Government will address questions received from offerors no later than 17 December 2013, and field any additional questions. The Government will provide official answers to questions in writing.

L-2.3.6 The Government will not be liable for expenses incurred by offerors to participate in the site visit and pre-proposal conference or provide travel, with the exception of the transportation within Colombia noted in paragraph

L-2.3.1.

L-3 SUBMISSION OF PROPOSALS

L-3.1 The original and copies of the proposals shall be sent or hand delivered to the issuing office listed in Block 7 of the Standard Form (SF) 33 on/before the date/time specified on the solicitation. Mark the front of the package with the following: “Proposal No. FA4890-13-R-0007, ATTN: Capt Alfred Fryman and Mr. John Bonafede, ACC

AMIC/PKC, “DO NOT OPEN IN MAIL ROOM.” If proposals are hand delivered, offerors shall contact the CO or

CM to schedule delivery. Offerors are cautioned to strictly comply with all instructions within this solicitation to ensure submission of a complete proposal. All offeror and team member/subcontractor information must be incorporated into one coherent proposal to be considered complete. Failure to furnish a complete proposal at the time of proposal submission may result in an unacceptable proposal that the Government eliminates from consideration for award. The Government will not accept any changes to offerors’ proposals after the closing date of this solicitation (see FAR Clause 52.215-1 for further information regarding late proposals).

L-3.2 Proposals in response to this solicitation will be valid for 300 calendar days from the solicitation issuance date. Offerors shall indicate in the proposal that the offer is valid for 300 calendar days in block 12 of Standard

Form 33.

L-3.3 Contact Information. Offerors shall provide:

L-3.3.1 Names, addresses, telephone numbers, and e-mail addresses of offeror points of contact (minimum of two personnel) to conduct any contracting business with the Government in connection with this solicitation.

L-3.3.2 Name, title, and signature of person authorized to sign the proposal. Proposals signed by an agent shall be accompanied by evidence of that agent’s authority.

L-4 DISPOSITION OF UNSUCCESSFUL PROPOSALS

L-4.1 In compliance with FAR Subpart 4.8, the Government will retain one copy of all unsuccessful proposals.

Extra copies of unsuccessful proposals will be destroyed. No destruction certificate will be provided.

L-5 PROPOSAL PREPARATION INSTRUCTIONS

L-5.1 The instructions below prescribe the format of proposals, and describe the approach for the development and presentation of the proposed data. The proposal shall include all of the information requested in the specific instructions. Failure to include all information requested may adversely affect the evaluation. A proposal that merely reiterates or promises to accomplish the requirements of the PWS will be considered unacceptable.

L-5.2 A proposal that is orderly and sufficiently documented will enable the Government to easily understand and perform a thorough and fair evaluation. The Government may incorporate into this contract, by reference or full text, all or portions of the successful offeror’s proposal submitted in response to this solicitation as revised and supplemented through the final proposal revision.

L-5.3 Proposal Volumes. A complete proposal consists of the following three (3) separate and distinct volumes:

Volume I – Technical

Volume II – Past Performance

Volume III – Price

L-5.4 Cross-Reference Matrix. Each volume shall be written on a stand-alone basis so that its contents may be evaluated without cross referencing to other volumes of the proposal. Information required for proposal evaluation which is not found in its designated volume will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is permitted where its use would conserve space without impairing clarity.

The offeror shall complete the cross reference matrix, provided as Section L Attachment 3, indicating the proposal reference information as it relates to the RFP, PWS, and applicable CLIN references. This is a tool to assist in the proposal preparation process. Offerors are ultimately responsible for ensuring that their proposal fully addresses all areas and that all requested information is completed in accordance with Section L and Section M.

L-5.5 Proposal Format. Specific guidance regarding format and content is discussed in the following paragraphs.

All pages provided by the offeror in excess of the stated limitations will not be evaluated. The proposal content shall be formatted as follows:

a. Text: Single-spaced and printed on single-sided paper versus duplex

b. Font Size: Microsoft Office Word, Times New Roman, 12 point font

c. Font Size for charts, tables, graphs and figures shall be no smaller than

Microsoft Office Word, Times New Roman, 10 point font

d. Margins: One Inch (1”) left, right, top and bottom

e. Paper Size: 8½” x 11” with a background color of white or ivory stock only

f. Page Numbering: Sequential (i.e., 1 through 100)

L-5.6 Legal Size Paper/Fold-Outs. Any pages within the proposal over the standard 8½” x 11” size paper shall be kept to a minimum, and only used where a table or other graph cannot be accommodated by a single 8½” x 11” page. Any text included in the fold-out shall be directly related to the fold-out contents. Fold-outs will be counted as the appropriate number of pages based on the number of folds (i.e., fold-outs with one fold shall count as one page; each additional fold counts as one additional page).

L-5.7 Electronic Copies. Electronic copies of all proposal information for each volume shall be submitted on CD

ROM or CD-R using Microsoft Office 2003 or later and/or searchable Portable Document Format (PDF) using

Acrobat Adobe 9.0 or later. Offerors shall NOT submit any document in PDF that are copied as images. Offerors may use JPEG images for charts or screen shots. Volume III Price – MASS CLIN Spread Sheet shall be submitted in Microsoft Excel 2003 or later formats. Offerors shall name files in an unambiguous manner, using plain language text, which facilitates the evaluator’s ease of accessing the files for evaluation, using standard naming conventions

(e.g., offeror name-volume name.doc for MS Word, offeror name-volume name.xls for MS Excel, offeror name-volume name.PDF for Portable Document Format, etc.). Each volume shall be in a single searchable file, with copy/paste capability, and stored in a folder that corresponds to the proposal volume it represents. Offerors shall insert the file name in the header of each document. All disks shall be virus checked prior to submission.

Replacement disks (CD ROM/CD-R) shall be required to update the final proposal resulting from any discussions, should they occur. If there is a discrepancy between the paper and electronic copies, the paper copy will take precedence and be used for evaluations.

L-5.8 Copies and Page Limit. Evaluators will only read up to the maximum number of pages specified. If the page limit is exceeded, the pages in excess of the limit will be removed from the applicable volume and will not be considered as part of the evaluation. Data submitted prior to the proposal submission due date will not be considered in the Government’s evaluation; therefore, such data should not be relied upon or incorporated in the proposal by reference. Copy requirements and page limitations are as follows:

NOTE: offerors shall submit an electronic copy and a signed hard copy of the SF 33 with Volume III.

L-5.8.1 One copy of each Volume shall be marked “Original”. The organizational chart, acronym list, cross reference matrix, and overview section (for exceptions) will not be included in the page count. Covers for volumes, tables of contents, indices, title pages, and section dividers/tables will not be included in the page count if they are inserted solely to provide ease to the reader in locating parts/sections of the proposal. They will be counted if they contain any other information, e.g., diagrams, extraneous data, etc. Pages marked “This page intentionally left blank” will not be counted.

L-6 EXCEPTIONS. The Government reserves the right to award without discussions; therefore, offerors are advised that taking exception to any requirements specified in this solicitation may result in the Government finding the proposal unacceptable. Clarification of Government requirements shall be handled by submitting questions/recommendations prior to submission of proposals. If offerors still find it necessary to take exception to any of the requirements specified in this solicitation, clearly identify each exception in the overview section of the appropriate volume along with a complete explanation of why the exception was taken and the resulting benefit to the Government. An overview section is only required if the offeror takes exception to any requirement in the solicitation (the overview section will not be included in the proposal page limitation described above). While offerors may propose exceptions to the solicitation requirements, the Government is not obligated to accept such offers. Any change to the requirement as a result of accepting an exception shall be done so with an amendment to the solicitation.

L-6.1 No Exceptions. In the event the offeror takes no exception to the stated requirements, a statement to this effect shall be included at the front of the subject volume.

L-7 INSTRUCTIONS FOR VOLUME I, TECHNICAL (FACTOR 1)

L-7.1 To facilitate evaluation, the Technical Volume shall be specific, detailed, and complete to clearly and fully demonstrate that offerors have a thorough understanding of the MASS requirements. Statements that the offeror understands, can, or will comply with the PWS or parts thereof (including referenced AF publications); and phrases such as “standard procedures will be employed” or “well known techniques will be used” etc., will be considered unacceptable. Offerors should note that data submitted prior to the proposal submission including data submitted for current contracts and other solicitations will not be considered in the Government’s evaluation. Therefore, such data should not be relied upon nor incorporated in the Technical Proposal by reference. The offeror’s proposal shall at a minimum be prepared in a form consistent with the Technical Criteria set forth in Section M, Evaluation Criteria, of this solicitation. All information that the offeror deems appropriate to address for this factor and its subfactors and aspects must be included in the proposal in an orderly format.

L-7.2 Glossary of Abbreviations and Acronyms. A glossary of abbreviations and acronyms shall be provided as an attachment within the Technical Volume. Glossaries do not count against the page limitations for the volume.

L-7.3 Technical Subfactors.

L-7.3.1 Subfactor A: Program Management. The offeror’s proposal shall address all of the following aspects:

L-7.3.1.1 Aspect 1: Management and Integration. Provide an organizational structure and explanation of the proposed program management approach to meet PWS requirements. At a minimum provide:

L-7.3.1.2 Aspect 2: Human Resources. Describe your process to ensure personnel are qualified, trained and certified. At a minimum provide:

1. Approach to attract, train, and retain qualified management and technical personnel, including overseas locations that may be difficult to staff

2. Training approach for ensuring that certified technicians are available at all required locations at contract start date

3. Summary of key personnel positions and their minimum qualifications and authorities to include, at a minimum, the following information:

a. Position/Title

b. Work Experience

c. Education

d. Security Clearance

e. Certifications

f. Language Capabilities

L-7.3.1.3 Aspect 3: Quality Management System (QMS). Describe your QMS controls to successfully accomplish

PWS requirements. At a minimum provide your approach to create a quality culture to effectively integrate all aspects of the Quality Management System (QMS) into corporate management, maintenance, and routine business processes; provide your approach to meet or exceed the current ANSI/ISO/ASQ 9001 requirements and Appendix C of the PWS; provide your approach to create and sustain compliance with contractual requirements, regulatory guidance, technical data, and all other technical guidance; and provide your approach for scheduling and system of controls to successfully provide quality/timely services or products that safely fulfill mission requirements.

L-7.3.2 Subfactor B: Logistics. The offeror’s proposal shall address the following aspects:

L-7.3.2.1 Aspect 1: Depot Repair Functions. Describe your approach, including subcontractor/vendor depot repair functions to ensure timely/quality depot repair of prime mission equipment (PME) and PME support equipment.

L-7.3.2.2 Aspect 2: Diminishing Manufacture Sources (DMS). Describe your approach to resolving DMS for PME repairable and consumable obsolescence issues.

L-7.3.2.3 Aspect 3: Required Licenses and Regulations. Describe your approach to obtaining and retaining necessary Department of State licenses and processing International Traffic in Arms Regulation (ITAR) assets to ensure they are not delayed in transportation/customs channels.

L-7.3.3 Subfactor C: Transition. Provide your executable transition plan for all locations. At a minimum:

L-7.3.3.1 Aspect 1: Key Milestones. Identify and explain the key milestones to ensure transition of responsibilities is complete by contract start.

L-7.3.3.2 Aspect 2: Organizational Structure. Provide an organizational chart that identifies personnel along with a brief functional statement and timeline for employment for each position involved in the transition.

L-7.3.3.3 Aspect 3: Qualified Workforce. Describe your approach to hiring qualified employees or training and certifying non-incumbent personnel to ensure full operations at contract start.

L-8 INSTRUCTIONS FOR VOLUME II, PAST PERFORMANCE (FACTOR 2)

L-8.1 Past Performance Volume. Past Performance Information shall be provided in a format that facilitates an easy assessment according to the past performance evaluation criteria in Section M, Evaluation Criteria, of this solicitation. Specific guidelines and requirements about volume content and format are provided in this section.

L-8.2 Past Performance Recency. Offerors shall submit performance information on contracts they consider recent

IAW Section M, paragraph M-3.2.3, of this solicitation.

L-8.3 Past Performance Relevancy. Offerors shall submit performance information on contracts they consider relevant to this effort. This information should include rationale supporting their assertion of relevance IAW

Section M, paragraph M-3.2.4, of this solicitation. Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the MASS PWS they relate.

L-8.4 PP Volume Contents. Offerors shall submit a Past Performance Volume containing the following:

L-8.4.1 Summary. Describe the roles of the references submitted for the offeror, subcontractors, teaming partners, and/or joint venture partners in relation to proposed performance for the subject requirement. Include a detailed matrix that depicts how each reference meets the relevancy criteria defined in paragraph M-3.2.4. Also include a detailed explanation of how the combination of all submitted references meets the relevancy criteria.

L-8.4.2 Past Performance Information (PPI) Reference Sheets (Attachment 1). Submit a separate PPI sheet for each reference with a minimum of 12 months of active performance prior to the date of the solicitation issuance.

Submit a minimum of two (2) and a maximum of five (5) references; two (2) of the references provided shall be on the offeror if available. The remaining three (3) references may be provided for subcontractors, teaming partners, and/or joint venture partners considered critical to successful performance of the requirement. PPI Reference Sheets will be treated as source selection information in accordance with FAR Part 2.101 and 3.104.

L-8.4.3 Subcontractor/Teaming Partner Consent Letter (Attachment 2). PPI references concerning subcontractor and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent.

Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner PPI reference information to the Prime during exchanges. These letters are only required for subcontractors and teaming/joint venture partners for which the offeror submits past performance information as part of the Past Performance Volume. Completed consent forms shall be submitted as part of the

Past Performance Volume.

L-8.4.4 Organizational Structure Change History. Many companies have acquired, have been acquired by, or have otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate the Government’s past performance evaluation, offerors shall include in this proposal volume a detailed "roadmap" describing all such organizational changes (if applicable), to include specific names and dates as well as specific DUNS Numbers and CAGE codes.

L-8.5 Other Sources. The Government may utilize references other than those identified by the offerors in evaluation of the offerors’ Past Performance. While the Government may elect to consider data obtained from other sources, the burden of providing accurate and complete, recent and relevant PPI references that demonstrate the ability to perform this requirement rests with each offeror.

L-8.6 Responsibility Determination. Even though the assessment of Past Performance is separate and distinct from the Determination of Responsibility required by FAR Part 9.1, PPI references contained herein may be used to support the Determination of Responsibility for the successful offeror.

L-9 INSTRUCTIONS FOR VOLUME III, PRICE (FACTOR 3)

L-9.1 Pricing Table. The offeror shall provide the proposed price for all CLINs, including Transition, Base, and all

Option Periods, listed in Section B of the solicitation (Schedule of Supplies and Services (CLINs)). This section shall include all unit prices (if applicable) rounded to the nearest dollar and extended amounts for all FFP CLINS.

An estimated (EST) amount has been pre-established for Cost Reimbursable (CR) CLINs. These pre-established amounts shall be included in the offeror’s proposal with no change. For the CR Supplies and Services CLIN(s)

X009, offerors shall provide the proposed, fixed fully-burdened handling rate that will be applied to costs, expressed as a percentage, in the description field of each applicable CLIN. Each offeror shall provide their price data in the attached excel spreadsheet (see RFP Attachment 5) and are NOT to input their pricing in Section B.

L-9.1.1 Labor Rates. Offerors shall use fully burdened labor rates in their proposal. The proposal’s pricing narrative shall break out the fully burdened labor rates, including all burdens, e.g., G&A, overhead, fringe, profit, etc., for each labor category as described in MASS PWS Appendix E, Contractor Minimum Education and

Experience Requirements, taking into consideration experience, education, etc. A rate is required for each labor category and service location. The offeror shall indicate which rates apply to CONUS labor subject to the Service

Contract Act and those for OCONUS labor.

L-9.1.2 Total Overall Evaluated Price. For ease of administration, offerors shall round all unit prices of proposed

FFP CLINs to whole dollar amounts BEFORE they are multiplied by the quantity. The unit price multiplied by the number of units shall equal the extended price of each FFP CLIN. If submitted unit prices are not properly rounded to the nearest dollar, the Government will round the unit prices to whole dollar amounts at the time of contract award. If the decimal is followed by 5, 6, 7, 8, or 9, the price will be rounded up to the nearest whole dollar. If the decimal is followed by 0, 1, 2, 3, or 4, the price will be rounded down to the nearest whole dollar. After all CLIN prices for all options are calculated and adjusted for rounding, if necessary, the Total Overall Evaluated Price will be determined by adding all CLIN extended prices into a single sum.

L-9.2 Service Contract Act. Offerors shall comply with clause 52.222-43 for their price proposal with regard to employees covered by the Service Contract Act (SCA). Proposed option year prices shall not include allowances for any contingency to cover increased wage or benefits costs for which contract price adjustment is provided by clause

52.222-43. The SCA only applies to CONUS labor; therefore, the increased wage or benefits costs of SCA covered employees for CONUS Support services shall only be increased IAW FAR 52.222-43. The offeror shall include rationale for proposed exemption of any CONUS labor normally subject to the SCA.

L-9.3 Reasonableness. Any inconsistency, whether real or apparent between promised performance/technical approach and price, shall be explained in the proposal. The burden of proof as to price credibility rests with the offeror.

L-9.4 Proposal Documentation. Offerors shall provide the following as a separate section contained in the Price

Volume and shall be completed as follows:

L-9.4.1 Complete in its entirety the “Offeror” portion of the Standard Form 33. An official having the authority to legally bind the company contractually must sign and date the Standard Form 33. One copy of the Standard Form

33 must bear an original signature and if applicable, all amendments. Failure to acknowledge any amendments to the solicitation can result in an offeror being deemed nonresponsive. Award cannot be made to a nonresponsive offeror. Note: Be sure to sign and date where required.

L-9.4.2 Complete in its entirety, Section K of the SF 33. State that your information is accurate and correct in the

Online Representations and Certifications Application (ORCA) within the System for Award Management (SAM) at https://www.sam.gov.

L-9.4.3 The offeror shall acknowledge any and all amendments per the directions on the amendment cover page

(SF 30). Failure to acknowledge any amendments to the solicitation may result in the offeror being deemed nonresponsive. Award cannot be made to a nonresponsive offeror.

L-9.4.4 Offeror shall identify the cognizant Defense Contract Audit Agency (DCAA) and Defense Contract

Management Agency (DCMA) field offices that have oversight to the offeror’s organization. Provide information concerning the adequacy of your accounting system pertaining to accumulation of costs for the cost reimbursable

CLINs. If DCAA has already determined the offeror’s accounting system adequate, the offeror shall provide the referenced DCAA audit report number and report date.
