FOR THE CONTRACTOR
Item 17. Specify appropriate price group from one of the following groups of effort in developing estimated prices for each data item listed on the DD Form 1423.
a. Group I. Definition - Data which is not otherwise essential to the contractor's performance of the primary contracted effort (production, development, testing, and administration) but which is required by DD Form 1423.
Estimated Price - Costs to be included under Group I are those applicable to preparing and assembling the data item in conformance with Government requirements, and the administration and other expenses related to reproducing and delivering such data items to the Government.
b. Group II. Definition - Data which is essential to the performance of the primary contracted effort but the contractor is required to perform additional work to conform to Government requirements with regard to depth of content, format, frequency of submittal, preparation, control, or quality of the data item.
Estimated Price - Costs to be included under Group II are those incurred over and above the cost of the essential data item without conforming to Government requirements, and the administrative and other expenses related to reproducing and delivering such data item to the Government.
c. Group III. Definition - Data which the contractor must develop for his internal use in performance of the primary contracted effort and does not require any substantial change to conform to Government requirements with regard to depth of content, format, frequency of submittal, preparation, control, and quality of the data item.
Estimated Price - Costs to be included under Group III are the administrative and other expenses related to reproducing and
d. Group IV. Definition - Data which is developed by the contractor as part of his normal operating procedures and his effort in supplying these data to the Government is minimal.
Estimated Price - Group IV items should normally be shown on the DD Form 1423 at no cost.
Item 18. For each data item, enter an amount equal to that portion of the total price which is estimated to be attributable to the production or development for the Government of that item of data.
These estimated data prices shall be developed only from those costs which will be incurred as a direct result of the requirement to supply the data, over and above those costs which would otherwise be incurred in performance of the contract if no data were required. The estimated data prices shall not include any amount for rights in data. The Government's right to use the data shall be governed by the pertinent provisions of the contract.

The delivery method is electronic posting to the PMIS with e-mail notification of posting to receivers in block 14. Final documents may be stored on the PMIS in Adobe PDF format. Delete block 10 of DD Form 1664 in its entirety and see block 16 of this form.
5) PWS, Section A, Para 4.1.1.6.1.1 The Government Furnished Property Report shall be in Excel Spreadsheet format and shall include the following fields at a minimum:
• Nomenclature
• Part Number/National Stock Number (NSN)
• Unit of Issue
• Quantity
• Unit Price
• Authorized Quantity
• Property Location (site/Location on site)
• Equipment Category, (i.e. consumable, reparable, equipment, ADPE, TMDE)
• Date of Last Inventory (DOLI)
• Asset Disposal Date (if Applicable)
• Warranty data (if active warranty)
• Date Property Assigned to Inventory List
• Estimated Life Cycle/Replacement year
• Estimated replacement cost
8) The document will be accept or rejected by the government within 30 working days after receipt. If rejected the contractor shall revise and resubmit the document within 10 working days after receipt unless otherwise directed by the CO.
11) The data contained in the report shall be current through 1 July of each subsequent year.
12) The first delivery of the report shall be 15 Mar 15.

Contractor shall notify the addressees in block 14 via e-mail once report is posted and attach a copy of the report to the notification e-mail. File name should be formatted in this manner. Example: MASS-SUR 14-01: "Title"
5) PWS Sec A Paragraph 2.1.1.5.2. All requests to modify MASS C-E equipment shall be submitted through the CO using an AF Form 1067, Modification Proposal IAW AFI 63-1101 Modification and PWS Para. 2.1.1.5.2.
8) Approval/disapproval/comments shall be provided by the Government within 180 working days after receipt of each submittal.
9) Contractor shall recommend a distribution statement based on technical content.

The contractor shall provide an Annual Facility Survey and Inspection report detailing facility and system deficiencies in the structural, electrical, mechanical, and environmental areas, with an introductory overview of issues/concerns from a CE/real property viewpoint for those sites listed in Appendix J. The Survey and Inspection report shall, at a minimum, include the conditions of facilities (interiors and exteriors), slabs and foundations, airfields, roads, grounds, drainage systems, sanitary sewage system, water supply, electrical system, POL system, other utility systems, power generation equipment, fire protection/detection equipment, and physical security aides. The Survey/Inspection report shall include photographs of the above items at each site in sufficient detail to document all deficient conditions as well as the overall condition of the sites. These photos will be organized into a single file for viewing along with individual photo files in a supporting sub folder on the submitted CD.
The report will be organized by site and include required Testing and Inspection information for the HNCC, Bogota, Colombia and the TADIL site at Cerro Azul, Costa Rica. At a minimum, this will encompass the date of the test and/or inspection, the procedures used, the facility number, the site, test/inspection results, and recommendations for any future work. For power generation, also include a spreadsheet of current status of all power generation equipment. This status will include hours operated since last overhaul for planning purposes.
The report will also include, for those sites listed in Appendix A for which the contractor has facility O&M responsibility, the contractor’s listing of real property and RPIE, corresponding to the official government facility number, so that there will a single document that provides a single overview of contractor maintained facilities.
Block 7: The report will be accepted or rejected by the Government within 30 working days after receipt. If rejected, the contractor shall revise and resubmit within 10 working days after receipt unless otherwise directed by the Contracting Officer.
Block 9: Distribution authorized to the Department of Defense and U.S. DoD contractors only for administrative or operational data. Other requests shall be referred to ACC AMIC/PKC.
Block 14 Report shall be submitted electronic format. The electronic format shall be on CD compatible with Microsoft Office in multiple files. CD will be turned in to ACC AMIC/PCE. This report, minus the subfolders will be posted to the contractor’s MIS with e-mail notification to the addressees.

5) Sec A PWS paragraph 3.1.5 The contractor shall provide a Site Log chronologically documenting on a daily basis. MASS O&M actions conducted at each contractor and FAC manned operating location. The Government recognizes that Site Log information can be conditional to what the Host Nation provides the contractor. The Site Log shall document the following information:
• The initial entry of each Julian date shall provide the current operational site status, calculated Comm Er for the month, personnel on duty and fuel status
• Projected equipment downtime/delays and reason for downtime/delays
• Preventive Maintenance Inspections (PMI) completed. Provide: PMI #, Equipment Control Number (ECN) #, Start (Z), Stop (Z), WO #, Status, and initials
• Initials of person notified at the CC, and all actions taken to return equipment to operation
• WOs opened and closed (0001Z to 2400Z)
• Noun, Part/Number (P/N), quantity, and supply document number of all MICAP parts shipped, received, and installed
• Visitors on site: Provide name, organization, arrival and departure date, and reason for visit of all contractor personnel and all visitors
• Message traffic: Message traffic sent and received by site
