STATEMENT OF WORK

NUEX13-1308 B938 Repair restrooms Luke Air Force Base, Arizona 3 May 2015

1. OVER VIEW

The project will be to remodel, reconfigure, and upgrade one (1) male and one (1) female restroom in B938. The contractor shall relocate and install new electrical circuitry, cables and receptacles, demolish existing tile, water closets, urinals, sinks, countertops and fixtures. The contractor will be responsible for adding new ceiling hung or mounted lighting as well as new wall and floor tile, solid surface countertop with integral sinks, urinals, restroom partitions and water closets

2. DESCRIPTION OF WORK

2.1. This project includes all necessary demolition, construction and testing to perform work as listed in description of work below.

2.2. Performance Period: all work shall be completed within 90 calendar days after the Notice to Proceed is issued.

2.3. Hazardous Material Abatement: No asbestos is present. An Asbestos Report is available for review.

2.4. Design drawings: Contractor shall submit an architectural drawing before construction begins. Drawing shall show new layout of restrooms and measurements for ADA compliant.

2.5. Existing conditions

2.5.1. Contractor shall camera existing waste/sewer main line and verify integrity of plumbing prior to demolition/construction. It is the contractor’s responsibility to keep all waste and plumbing lines free of debris during the project.

2.5.2. Provide a complete architectural plan for the construction area for review by the Contracting Officer (CO) and CE PM prior to construction start. This shall include a electrical plan for new lighting layout.

2.5.3. Contractor shall furnish (4) temporary toilets and (2) Wash Stations serviced twice a week until men’s/women’s restroom are fully operational. (1) Temporary toilet shall be handicap accessible. Location TBD by user.

2.6. DEMOLITION Men’s restroom

2.6.1. Demolish floor tile (12” x 12”) and wall tile (4” x 4”). This includes but is not limited to any dry-pack, compound or adhesive used to seal tile to floor and walls.

2.6.2. Demolish tile walls and floor in shower area.

2.6.3. Demolish lighting fixtures, diffusers, light switches, mirrors and cover plates, receptacles and cover plates.

2.6.4. Demolish gypsum board and/or hardy-backer material on walls. Remove the ceiling gypsum board and suspension system used to hang the ceiling from the structure and counter tops.

2.6.5. Evaluate and demolish rusted or damage studs as required.

2.6.6. Demolish (6) faucets and (6) sinks to include valves. Demolish shower fixtures to include mixing valve.

2.6.7. Demolish (6) water closets and (4) wall hung urinals. This includes but is not limited to supports, flush valves and flanges.

2.6.8. Remove plumbing and electrical lines to accommodate new floor plan.

2.6.9. Demolish above ground cast iron sanitary sewer piping. A minimum of 3” stub out shall remain above finish floor level to attach new ABS piping for waste and vents.

2.7. CONSTRUCTION Men’s restroom

2.7.1. Provide and install new ABS piping for above ground waste lines and vent pipes. Size and location shall be same as cast piping demolished.

2.7.2. Provide and install new shower stall equipped with new shower faucet/trim, shower drain, shower door and all fixtures and components necessary to provide a fully functional shower. Shower shall be tiled around all walls in restroom. Adjust the shower opening to ensure dimensions meet ADA standards for a roll in shower. Provide and install all grab bars, seat and safety devices required by ADA standards. Change the shower area door swing to open into the restroom area. Shower door will have a minimum of ¼” obscured glass with continuous hinge door with full magnet strip and pull handle and be at least 72” tall.

2.7.3. Rebuild wall separating the sinks from the urinal to be using 1-5/8” metal studs and ½” gypsum board to allow enough space to allow for (3) new wall mounted urinals and (1) ADA urinal with urinal privacy partitions. Urinals shall be equipped and installed with all accessories and components to provide full functionality system. This includes but is not limited to flush valves, carriers and flanges.

2.7.4. Install new gypsum board on walls and finish to meet level 4 finish requirements. Provide a textured finished product. Contractor to submit samples of heavy and light orange peel to be selected. Paint all walls (1) primer coat and (2) finish coats per Luke AFB specifications, semi-gloss finish.

2.7.5. Install new gypsum board ceiling on new metal studs tied into the existing walls. Ceiling height shall match existing. Ceiling shall be secured i.a.w. with all applicable IBC and shall be completed with a level 5 finish. Paint ceiling (1) primer coat and (2) coats finish per Luke AFB specifications, semi-gloss finish.

2.7.6. Sand, prep, prime and paint all existing door frames.(1) coat primer (2) coats finish per Luke AFB specifications, finish shall be semi-gloss.

2.7.7. Install new wall and floor tiles (12” x12”) of the same type and color. Wall tiles shall be installed to 5’-4” AFF around all walls. The top edge of the tile shall be finished with bullnose tile of the same color. Tile shall be applied in a diamond pattern and in a manner that no hollow spots are created under tile.

2.7.8. Provide and install new floor drains (4). Floor shall have proper slope to floor drains.

2.7.9. Provide and install new cleanouts where required.

2.7.10. Provide and install (5) new floor mounted water closets and one ADA compliant water closet. Water closets shall be equipped and installed with all necessary accessories/components to provide full functionality. This includes but is not limited to flush valves, flanges, and wax rings.

2.7.11. Provide and install new restroom partitions. Partitions shall be wall and floor mounted. Partitions shall have toilet paper dispensers and garment hooks on the doors. ADA partitions shall have grab bars. Length and height shall comply with ADA specifications.

2.7.12. Provide and install (2) new solid surface countertops with integral sinks, back splash and support brackets to comply with current ADA standards.

2.7.13. Provide and install (6) new single throw faucets, (3) per countertop. Faucets shall be installed and equipped with all accessories and applicable components necessary for a fully functionality.

2.7.14. Provide and install new 48” tall wall hung mirrors that stretches the entire length of each sink area with metal frame.

2.7.15. Provide and install new wooden bench in shower and locker room area.

2.7.16. Reinstall Kimberly Clark paper towel and soap dispensers location to be determined on site.

2.8. DEMOLITION Women’s restroom

2.8.1. Demolish floor tile (12” x 12”) and wall tile (4” x 4”). This includes but is not limited to any dry-pack, compound or adhesive used to seal tile to floor and walls.

2.8.2. Demolish gypsum board and/or hardy-backer material on walls. Remove the ceiling gypsum board and suspension system used to hang the ceiling from the structure.

2.8.3. Evaluate and demolish rusted or damage studs as required.

2.8.4. Demolish (2) faucets, two (2) sinks, and one (1) countertop.

2.8.5. Demolish (2) water closets. This includes but is not limited to flush valves, flanges and water closet accessories.

2.8.6. Demolish water closet partitions partitions.

2.8.7. Demolish lighting fixtures, diffusers, light switches and cover plates, receptacles and cover plates.

2.8.8. Contractor shall demolish plumbing and electrical lines, all fixtures and any wiring needed to relocate plumbing/electrical lines. Note: contractor shall make every effort to utilize existing plumbing lines and circuitry in the project area.

2.8.9. Contractor shall remove and replace with new, all heat sensors, fixtures, diffusers and escutcheons for fire suppression system.

2.8.10. Remove to be reinstalled Kimberly Clark paper towel, hand sanitizer, soap dispensers, and waste receptacles.

2.8.11. Cut and remove above ground cast iron sanitary sewer piping. A minimum of 3” stub out shall remain above finish floor level and be dressed for attachment of new ABS piping.

2.9. CONSTRUCTION Women’s restroom

2.9.10. Provide and install (1) new solid surface countertops with integral sinks, back splash and support brackets to comply with current ADA standards.

2.9.11. Provide and install (2) new single throw faucets. Faucets shall be installed and equipped with all accessories and applicable components necessary for a fully functionality.

2.9.12. Provide and install new 48” tall wall hung mirrors that stretches the entire length of each sink area with metal frame.

2.9.13. Reinstall Kimberly Clark paper towel and soap dispensers location to be determined on site.

2.10. HVAC Requirements

2.10.1. Provide and install a new exhaust fan with roof curb and bird screen to increase the airflow to meet ASHRAE 62.1 – 2010 requirements.

2.10.2. Install new round metal ductwork to support additional airflow.

2.10.3. Provide and install new diffusers and branch duct balance dampers for each inlet.

2.11. ELECTRICAL Requirements

2.11.1. Provide and install new lighting system in accordance with UFC 3-530-01 to ensure adequate light is provided to all areas of the project area. The restrooms do not have adequate light over water closet area.

2.11.2. Provide and install all necessary and applicable electrical wiring to provide functional electrical system. This includes but is not limited to lighting, receptacles, switches, as well as fire alarm systems that are moved and/or relocated. All outlets and switches must be replaced with new.

2.11.3. Verify panels have adequate power capable of handling increased/additional electrical circuitry.

2.11.4. Replace all fire alarm systems and components in the affected project area per NFPA 101.

3. GOVERNMENT FURNISHED INFORMATION, PROPERTY, OR SERVICES

3.1. Water and electrical is available for the contractors use at the project location. There is no Government furnished equipment or services.

3.2. Upon the Contractor’s request, the Government will provide electronic and/or hard copies of available documents and correspondences pertinent to the work.

4. GENERAL INFORMATION

4.1. GENERAL

The contractor shall furnish all materials, equipment, and personnel necessary to design, manage, and accomplish the project. The Contractor shall thoroughly review the request for proposals for constructability and advise the contracting officer of potential problem areas prior to submitting a cost proposal. No alterations, additions or deletions shall be made unless prior written approval is given by the contracting officer. Under no circumstances is the Contractor to begin any work on the project until a Notice to Proceed is issued by the Contracting Officer.

4.2. SITE VISIT

The Contractor shall inspect the site where services are to be performed. This site review shall satisfy the Contractor’s need to identify all general and local conditions that may affect the cost of the contract. Failure to inspect the site will not constitute grounds for claims after contract award.

4.3. WORKING HOURS

All tasks will be performed during normal work hours of regular workdays, from 0700 to 1600, Monday through Friday, unless otherwise stated in writing by the Contracting Officer. Upon written request from the contractor to perform work outside of these days and times will be reviewed by the CO for approval or disapproval.

4.4. CODE REQUIRMENTS

All work shall be done in accordance with the most current editions, based on the award date of this contract, of the Unified Facility Criteria (UFC), International Building Code (IBC), National Electric Code (NEC), Life Safety Code (NFPA), International Plumbing Code (IPC), International Mechanical Code (IMC), current Engineering Technical Letters (ETLs), and Luke Air Force Base Master Specifications unless expressed written permission is issued from the Contracting Officer for any deviations.

4.5. QUALITY CONTROL & SAFETY

4.5.1. The contractor shall provide a superintendent for the full duration of the project and he/she must be on site at all times while work is being performed.

4.5.2. Contractor shall provide a Quality Control (QC) person who shall inspect, test and perform duties consistent with Contractor submitted quality assurance program. The Contractor(s)’ QC and Superintendent cannot be the same person, unless the Contracting Officer has specified otherwise.

4.5.3. Contractor shall submit a Safety Plan and Quality Control Plan to the Contracting Officer. If the Contractor has an IDIQ contract, SABER contract, or MACC contract with Luke AFB, Quality Control and Safety plans only need to be submitted one time, and can be updated if and when Option years are exercised by the government.

4.5.4. Contractor’s QC shall draft daily reports and provide them to the government project manager by COB of the first work day of the following week; these reports shall show in detail the following information:

4.5.4.1. Subcontractor’s work performance for each day.

4.5.4.2. Any findings by the QC on any particular day.

4.5.4.3. Date and time safety meetings were held and what issues were discussed.

4.5.4.4. Date and time required inspections and tests were performed. Provide factual evidence that the inspections or tests have been accomplished. Indicate the results, the nature of any defects, causes for rejection, the proposed remedial action as well as the corrective action(s) taken to resolve the defect.

4.5.5. Notice of Non-Conforming Work: the QC, or other designated representative, shall acknowledge receipt of the Notification of Non-Conforming Work (for technical, safety, or other reasons) issued by the Contracting Officer. Such non-conforming work shall be immediately replaced or corrected as necessary. Immediately bring any disagreements to the attention of the Contracting Officer, in writing, for resolution.

4.6. FINAL INSPECTION

Upon completion of all punch list deficiencies discovered during the pre-final inspection, the Contractor shall request that the Government conduct a final inspection, and publish the findings in a Final Inspection Report, in accordance with the format specified at the pre-construction conference. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final report. The final inspection report shall also certify that all items of the design have been implemented and that the construction is complete. The final inspection report shall also include a record of “sealed and signed” design drawings, as-built drawings, and specifications, verifying that all development standards have been met. At the final inspection, the contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the Base Civil Engineer (BCE) or their representative for signature and acceptance.

4.7. DELIVERY/WARRANTY

4.7.1. The contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the contractor shall deliver to the Government the shop drawings in the format and media as required by this contract. Further details regarding project closeout will be provided by the 56 CES/CENM project managers.

4.7.2. The Contractor shall provide a warranty for all workmanship and new equipment installed under this contract for a period of one (1) year. The warranty period shall start upon final acceptance by the Government. Extended warranties offered by the Contractor and its subcontractors or suppliers may be accepted by the Government at its discretion. This Warranty protects the Government from any and all freeze damage as well as roof penetration/damage leaks.

4.8. REQUIRED SUBMITTALS

4.8.1. Submittals are required for this project and are listed along with their due dates on the AF Form 66, Schedule of Material Submittals.

4.8.2. When required per Schedule of Submittals, Contractor shall submit on an AF Form 3000 all product, material, shop drawing and designs to include Material Safety Data Sheets (MSDS) to the Contracting Officer for approval before any and all items are brought to or installed on project sites at Luke AFB.

4.8.3. Project Schedule: within one week of Notice to Proceed, provide a legible project schedule on AF Form 3064 in printed and PDF format for construction services to include dates of anticipated work completion.

4.8.4. Contractor shall submit on Luke AFB Form 317 Contractor’s Reporting Environmental Entry Form any and all products used on this project. These items will be tracked monthly and reports will be turned into Mr. Oscar Mabry or Ms. Teri Wesley at the Hazmat office phone number (623) 856-4748 or 4749.

4.8.5. Demolition List: the Government reserves the right to salvage any/all equipment or materials prior to construction start. The Contractor shall submit a list of equipment to be demolished for Government review. A list of all items with delivery instructions will be provided in return to identify items for Government salvage. All other items will be the Contractor’s responsibility to dispose.

4.8.6. Operation and Maintenance Manuals: provide O&M Manuals including all warranties, vendor contacts, relevant product data including proper maintenance and operation, and pertinent shop drawings describing all installed materials, systems and equipment in the project. Manuals must be organized by CSI specification sections.

4.8.7. As-Built Construction Drawings and Specifications: upon completion of construction, provide as-built drawings and specifications incorporating any departures from the original contract documents.

4.8.7.1. Electronic As-built Drawings and Specifications: provide electronic drawing files in a format compatible with AutoCAD 2010 including all external references required to view and print the drawing files properly. Include all necessary *.ctb, *.pc3 or similar files needed to properly set line weights for printing. Drawing files should be organized in general accordance with the Department of Defense A/E/C CAD Standard (ERDC/ITL TR-12-X) available in PDF format on many federal websites. In addition, provide drawings in PDF format for on-screen viewing and reproduction. Provide final project specifications in PDF and Microsoft WORD *.doc or *.rtf format.

4.8.7.2. The file naming convention and file structure shown in the As-built Submittal Guidelines shall be used in lieu of what is found in the Department of Defense A/E/C CAD Standard.

4.8.7.3. Hardcopy As-built Drawings and Specifications: Provide two (2) full size and two (2) half size printed copies of the as-built drawings and two (2) copies of the final specifications.

4.8.8. The government reserves the right to request submittals on any product or service for duration of the contract.
