FA4887-11-R-0014 Attachment 12 Luke AFB Electrical IDIQ

Statement of Work

1. Contract Objectives: .

A. The contractor’s administration shall include the following:

1.1 Full time site superintendence.

2.1 Submission for approval of: A schedule of values, construction schedule(s) (Microsoft Project), material cut sheets, shop drawings.

3.1 Submission for approval of: Dig permits, confined workspace permits (if applicable), utility outage requests, and invoices for partial payment(s).

4.1 General work products should be in Microsoft Office format. Drawings, O&M manuals and final submittals are discussed in Section Technical Services.

B. After the issuance of this contract, the contractor shall attend all meetings/teleconferences at the location specified by the Contracting Officer’s Technical Representative (COTR).

C. The contractor shall prepare and publish all meeting minutes following Contracting Officer’s approval, track all action items assigned, and shall document all items discussed at a meeting, as well as each meeting’s attendees.

2. Specifications: The division one general requirements shall be included as part of this requirement and specify minimum requirements that shall be used in developing the final design and construction of this project. Final specifications shall be prepared incorporating these requirements into the complete specification requirements for this project using CSI standard 50 division format and the Luke AFB 50 Division Master specifications.

1. Scope: The work will include the following, with a performance period of 240 calendar days after issuance of a written Notice to Proceed

A. Replace existing 12,470 volt primary transformers with new transformers. Existing transformers shall be matched in KVA size and new transformers shall meet the requirements of attached specification 16460 – Oil filled pad mount transformers.

B. The contractor will procure material and provide labor as necessary to complete the repair by replacement of overhead primary 15 KV lines with new underground distribution system. The new distribution system shall include new conduit, conductors, manholes, handholes, high voltage switches, transformers, street lights, secondary feeds to existing facilities, and empty conduits for communications circuits.

C. The contractor is responsible for construction quality on the project. The contractor is required to make their own field investigations to verify dimensions and other information shown on Government furnished reference drawings.

D. The contractor shall visit the job site to re-verify location, conditions, quantities and dimensions previously determined during the design portion of the project, as part of his construction proposal preparation. The contractor shall coordinate with the Contracting Officer prior to the visit to the job site.

E. The contractor shall confine construction operations at the site to areas permitted by law, ordinances, permits, and the contract documents and shall not encumber the site with any materials or equipment without prior written authorization. Subject to prior written approval of exact location, appearance, and color of each, the contractor may be permitted to set-up an office trailer/portable building, a similar job tool trailer, and a waste dumpster in a location designated and approved by the Contracting Officer. The contractor shall store his other materials and equipment at the site designated by the Contracting Officer at the end of each workday.

F. The contractor shall coordinate all of his operations with and secure written approval from the Contracting Officer before using any portion of the site.

G. The contractor shall be responsible for any cutting, fitting, or patching required to complete the work to ensure all parts fit properly.

H. The contractor shall take positive actions to prevent work-generated refuse from entering the base sewage or storm drainage systems. Actions may include installation of temporary plugs, dikes, screens, strainers, or similar measures to prevent entry into the systems or other actions as required.

I. The contractor shall be responsible for assuring that the existing adjacent site features are not damaged, and remain in their original condition at the completion of the contract work. If the contractor fails to clean up at the completion of the work, the Government may do so as required, and the cost thereof shall be charged to the contractor.

J. All contractor-generated refuse and waste shall be hauled from the building construction area and deposited in his waste dumpster, or such waste shall be removed from the site daily to a disposal area to be selected by the contractor, and located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in trucks with tight-fitting beds to prevent spillage on roadways. The construction site shall be kept neat, orderly, and safe for workmen at all times.

K. During the performance of the work within the requirements of this contract, and until final completion and acceptance thereof, the contractor shall exercise the utmost care to avoid accident or injury to persons or property. He shall place and maintain all necessary barriers and safeguards, including watchmen, if necessary, about the work site for the prevention of accidents and shall take all precautions required to protect personnel and property to include the general public and properties adjacent to the work site.

L. The contractor will provide complete testing of applicable equipment to serve as a basis for acceptance of the repaired installation.

M. The contractor will provide training, and a complete operations and maintenance package, for the future operation of the upgraded Electrical system..

N. The contractor will coordinate and schedule final turn-over and provide complete as-built documentation.

2. Statement of Work: This project work will include all, demolition, disposal, material, labor, and testing to deliver complete, operational, and usable systems. An outline of the work follows and the outline order of work is not presented in required order of accomplishment of total work. Demolition and disposal off base, any existing transformers, poles, conductors, hardware, switches, panels, and associated obsolete equipment. The work will include but not necessarily be limited to:

A. Electrical Distribution System: The contractor shall install the necessary repairs or modifications to the existing Electrical Distribution System.

1.1 Repair by replacement the overhead 12,470 volt three phase electrical distribution located along Gillespe drive from Bong lane to Fighter Country Lane. Estimated length is 2,000 linear feet. Minimize disruptions to existing facilities and traffic flow.

2.1 Provide new street lighting to match existing.

3.1 Transformer Replacement – Contractor to field verify the locations, foot prints, and voltages to match existing transformers.

4.1 Remove existing pad mounted oil filled transformers.

5.1 Install new pad mounted oil filled transformers on existing pads.

6.1 Insure replacement transformers meet the color scheme at Luke AFB, AZ. factory baked finish (mellow brown or light tan).

7.1 Procure coordination and approval for all scheduled utility outages (3) weeks in advance of event to minimize mission impact.

8.1 Insure coordination with Civil Engineering Electrical Systems shop providing data for transfer of transformer from inventory. Contractor to test and dispose of transformers in compliance with all local, state, and federal, laws rules, and regulations.

9.1 The contractor shall repair any affected working areas to original condition if altered during the construction.

10.1 Restore work area cleanliness and order, and dispose of construction debris.

11.1 Provide as-built drawings (AutoCAD) and transformer manufacturer test reports.

B. Summary Status Reports: The contractor shall provide weekly and monthly progress reports for this contract. All information will be maintained in an electronic database (compatible with Microsoft Office Professional, current version) to facilitate summary retrievals. Reports will be acceptable in contractor format.

C. Quality Control (QC) Oversight: The contractor shall review, plan and/or develop QC procedures for this contract, and submit the Quality Control Plan within 15 calendar days after award under this contract for government approval.

D. Hazardous Materials: Materials survey may include destructive testing as necessary to ensure adequate investigation for hazardous materials. Base Environmental personnel and standards will be consulted for approval in this regard.

E. Construction: Upon the approval of the Contracting Officer, the contractor may commence onsite construction activities. Further details regarding construction management requirements, inspection and testing, construction facilities and temporary controls, environmental protection, quality control system, construction quality control, and project closeout will be provided by the 56 CES/CEPM project manager.

1.1 Security: The contractor shall obtain and monitor security badges used by their own staff during the duration of this contract, in accordance with Clause L-727 of this contract entitled Contractor Access to Air Force Installations.

2.1 On-Site Field Oversight: The contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services during the construction phase of this contract.

3.1 Safety Requirements: The contractor shall conduct operations in accordance with FAR Part 52.236-13 and Alternate 1.

4.1 Storage: The contractor shall conduct operations in accordance with FAR Part 52.236-10.

5.1 Pre-Final Inspection: The contractor shall conduct a pre-final walkthrough inspection and publish the pre-final inspection findings in a pre-final inspection report.

6.1 Final Inspection: The contractor shall conduct a final inspection and publish the findings in a Final Inspection Report in accordance with the format specified at the pre-construction conference. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final report. The final inspection report shall also certify that all items of the design have been implemented and that the construction is complete. The final inspection report shall also include a record of “signed and sealed” design drawings and as-built drawings and specifications verifying that all development standards have been met. At the final inspection, the contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the Base Civil Engineer (BCE) for signature and acceptance.

7.1 Delivery/Warranty: The contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the contractor shall deliver to the Government the as-built drawings in the format and media as required by this contract. Further details regarding project closeout will be provided by the 56 CES/CEPM project manager. The warranty shall be issued in accordance with FAR 52.246-21. Extended warranties offered by the contractor and its subcontractors or suppliers may be accepted by the Government at its discretion.
