RESPONSES TO QUESTIONS

1. Will completed project evaluations for other Federal/DOD projects reflecting the same detailed information, but sent directly to you from the evaluator be acceptable in terms of Past Performance evaluations? These evaluations have been completed within the past three months and are current with multiple points of contact for verification. After reading through page 37, paragraph c., it does not state that the evaluation must be received from either the Prime contractor or the issuing Government Representative. Based on paragraph c, should I send you the evaluations that I have in my possession for the representative projects directly? They are still verifyable.

Paragraph c. refers to Attachment #7 which is the Past Performance Information Form and not the questionnaire.

See paragraphs (d)(3)f. and (d)(3)g. for information on requirements for the Past Performance Questionnaire. Para. (d)(3)g. reads: “Any past performance questionnaires sent directly from a proposing offeror will not be accepted or evaluated.”

The Past Performance Questionnaire in the solicitation must come directly from the evaluators. The Past Performance Information Form comes from the offeror and a similar form from another agency may be submitted.

2. The proposal preparation instruction asks One Hard Copy and One CD for each of Part I and Part II, and One Original and Three Copies of Part III. Elaborate formats, bindings or color presentations are not desired or required. As such, we plan to put all three parts in one three-ring binder with three tabs separating the three parts with contents as described above. Please confirm this is adequate and meets your binding requirements.

This is adequate and meets the solicitation requirements. However, a binder is not required.

3. Part III asks past performance information to be limited to no more than 5 pages per contract listed. Fourth sentence in (3)b states that “Provide a list of at least three, but no more than five of the most relevant contracts performed……… within the last three years.” We have asked our past clients to send in the Past Performance Questionnaire form (Attachment 8) to meet the above criteria. Do you also need us to provide a project description for each of the reference in addition to your “Past and Present Performance Information” form (Attachment 7)?

No, you do not need to provide a project description for each of the references in addition to the Past and Present Performance Information form (Attachment 7).

4. In SF 1442 Block 13B, an “offer guarantee” is requested. You did not specify the requirement as to % of guarantee and other requirement. Also, where should we provide this in the proposal (i.e., which Part) if this is required?

An offer guarantee (SF 1442 Block 13B) is not required. The solicitation will be amended to reflect this change. Reference amendment #4.
