STATEMENT OF WORK
REPAIR DOCKS, BASEWIDE
TYNDALL AFB FLORIDA
Base Project Number: XLWU 17-1022 23 March 2018
1.0 INTRODUCTION
The project intent is to repair select marine related structures that are located at the various facilities on Tyndall Air Force Base (AFB). The Contractor shall perform the services herein and as described in this Statement of Work (SOW) and the included solicitation documents.
2.0 SCOPE OF WORK
The Contractor shall be responsible for all professional services, permits, equipment, labor, tools, materials, and ancillary items necessary to complete the tasks defined by the SOW. The performed tasks shall be based on the SOW, solicitation documents and information covered during a pre-construction/criteria review meeting to be held prior to Contractor mobilization.
2.1 Scope & Applicable Documents: This SOW sets forth the requirements for an SR&M Acquisition Invitation for Bid (IFB) contract site specific construction effort to repair select portions of the docks and supporting marine facilities of Tyndall AFB, Florida. The Contractor shall adhere to the following guidance during construction phase(s) of this project: United States Army Corps of Engineers, Florida Fish and Wildlife Conservation Commission, Uniform Building Code (UBC), Unified Facilities Criteria (UFC) 3-600- 01, those sections of the Southern Standard Building Codes not incorporated in the UBC/International Building Code (IBC) for our respective area, Air Force Standards, Department of Defense (DOD) Directives, Florida State Statutes, and the Tyndall AFB Facility Excellence Plan (Base Regulations), National Electric Code (NEC), Occupational Safety and Health Administration (OSHA) Requirements, Department of Environmental Protection (DEP) Regulations, Environmental Protection Agency (EPA) Regulations, and any related Americans with Disabilities Act (ADA) Compliance Requirements. If there is a conflict between these codes, regulations and specifications the most stringent requirements shall apply at all times. Documents listed by and/or provided by the Government are for reference only and do not relieve the contractor from adherence with applicable codes not specifically addressed.
2.1.1 Description of Work: As part of the proposed effort, the Contractor shall implement means and methods that will limit disturbance to the natural vegetation, dune systems, water bodies and base activities to the greatest extent practicable throughout the project performance. The government provided documents and existing field conditions shall be reviewed by the contractor to identify any potential discrepancies and develop necessary Requests For Information (RFI) prior to construction mobilization.
2.2 Construction efforts shall utilize high quality materials typical for commercial marine facilities. Fasteners and hardware shall be stainless steel unless noted otherwise, and all wood products shall be treated in accordance with the American Wood Protection Association (AWPA) Use Category System. Rehabilitation efforts defined in the project tasks shall restore the existing structures to original or better condition and integrity meeting all applicable codes and standards. Materials defined in the statement of work for replacement shall be replaced with new materials of the same dimensional properties, quality, and at the same spacing unless noted otherwise. All work shall be in strict accordance with the contract documents.
Substitutions and/or deviations will not be accepted unless approved by the Contracting Officer (CO) in writing. Clean up all construction debris daily. Restore disturbed areas and dispose of all waste off site at the conclusion of work. The task descriptions below are summaries of work to be completed in order to result in complete and usable marine facilities meeting the professional standard of quality for the trades utilized. Ancillary efforts required to access and complete the work not specifically listed in the task description shall be anticipated and accounted for in the contractor’s bid documents. Awarded tasks may be performed concurrently. Criteria Drawings, details and satellite imagery are included in SOW Appendix A.
2.2.1 TASK 1 - Bonita Bay Boat Ramp, B5030: Repair efforts are intended to rehabilitate the distressed pavement approach, ramp surfaces, and ramp end erosion at the boat ramp facility. The facility contains two ramps that are estimated to be 45 feet by 15 feet (+675 ft²) with sheet piles around the entire perimeter of each ramp. The distressed portion of the concrete pavement approach is estimated to be 50 feet by 14 feet (+700 ft²) at each of the two ramps. Cast-in-place concrete pavement shall be installed for the ramp approach. The government anticipates that precast, prestressed or cast-in-place concrete ramp systems will satisfy the sustainment and restoration project intent. The general contractor shall provide documents defining the proposed ramp system to the government with the bid documents for consideration. Professional services and permitting are anticipated for this task and are the responsibility of the awarded bidder. Professional services and permitting are estimated to require 90 days. Construction is anticipated to require 60 days.
A. Demolition (Ramp):
1. Remove and dispose of approximately 675 square feet of concrete panel ramp sections at each of the 2 existing ramps (total +1,350 ft²);
2. Remove and dispose of the soil within the demolition limits as necessary to permit reconstruction and grading for a new ramp pavement surface;
B. Demolition (Approach):
1. Remove and dispose of approximately 700 square feet of concrete slab on grade pavement for each of the two ramp approaches, (total +1,400 ft²);
2. Remove and dispose of the soil within the demolition limits as necessary to permit reconstruction and grading of the approach pavement.
C. Construction (Ramp):
1. Grade approximately 675 square feet of the exposed soil materials throughout the extent of the ramp areas, (total +1,350 ft²);
2. Grade the power loading depressions and mounds beyond the toe of the ramp;
3. Install a minimum 12 inches of consolidated stone base for ramp systems requiring base materials;
a. Consolidation may be achieved by means of choking-in with repeated bucket impacts with a minimum of 2 lifts to achieve an unyielding surface.
4. Install anchoring members to secure the ramp from movement during service life (as required based upon installed ramp type);
5. Install approximately 675 square feet of ramp surfaces at each of the two ramps,
a. Cast-in-place concrete, pre-cast or prestressed concrete (total +1,350 ft²);
b. 12% to 15% grade – notify CO if alternate slope is recommended;
c. Minimum 8 inches thick for flat plate ramp systems;
d. Minimum 4,000 psi concrete;
e. Reinforced with epoxy coated reinforcing steel;
f. Surface finished shall be grooved to enhance traction and drainage;
g. All metal hardware and fasteners shall be stainless steel.
D. Construction (Approach):
1. Grade and compact approximately 700 square feet of the exposed soil materials within the demolition limits of each ramp approach, (total +1,400 ft²);
a. Subgrade min LBR 40;
b. In-place relative density 95% of the modified proctor value.
2. Install a minimum 6 inches consolidated base material to the prepared subgrade at each of the two ramp approaches;
a. Base material min LBR 100;
b. In-place relative density 95% of the modified proctor value.
3. Install approximately 700 square feet of new reinforced concrete pavement similar to existing at each of the two ramp approaches;
a. Cast-in-place concrete pavement (total +1,400 ft²);
b. Minimum 6 inch thick;
c. Minimum 4,000 psi concrete;
d. Reinforced with epoxy coated reinforcing steel.
2.2.2 TASK 2 - Bonita Bay Dock Sidewalk, B5030: Repair efforts are intended to rehabilitate the concrete walking surface adjacent to the covered boat dock that has been undermined during its service life and is now unusable due to tripping hazards and out-of–level surface. The concrete pavement to be repaired is estimated to be 6 feet x 116 feet (total +696 ft²). Period of performance for this task is anticipated to require 14 days.
A. Demolition:
1. Remove and dispose of the existing concrete pavement;
2. Remove any unsuitable soil materials, organic materials and debris from the exposed subgrade;
B. Construction:
1. Backfill work area as necessary to accommodate new slab;
a. Compacted select fill material placed in maximum 12” lifts to restore original grade;
b. Flowable fill may be utilized in place of select fill;
2. Install approximately 696 square feet of a new reinforced concrete slab;
a. New slab to match existing with regard to thickness (min 6”), edge dimensions and joints;
b. Minimum 3,000 psi concrete;
c. Top of slab shall be the same elevation as the top of retaining wall head;
d. Slab finish shall match existing.
2.2.3 TASK 3 - 82 ATRS Boat Storage, B9709: Repair the two lower finger piers within the covered dock facility. Each of the two finger piers are estimated to be 4 feet wide and 30 feet long (total +240 ft²) and are supported by timber piles. The work will remove and replace all decking and framed structure leaving only the supporting timber piles. Period of performance for this task is anticipated to require 14 days.
A. Demolition:
1. Remove and dispose of the four tread stair at each finger pier;
2. Remove and dispose of the deck surface and supporting structure at each of the +120 square foot finger piers (total +240 ft²);
B. Construction:
1. Construct new supporting frame using the in-place timber piles to meet the following requirements:
a. Straighten/true the existing piles as required;
b. Hardware and fasteners used shall be stainless steel unless noted otherwise;
c. Outside stringers shall be at minimum double 2x8 members or equal heavy timber members
(perimeter frame);
2. Install new deck surface to meet the following requirements:
a. Deck Screws shall be stainless steel with minimum 2 screws per stringer;
b. Deck board spacing shall be selected by the contractor based on actual site and material conditions at the time of installation to achieve seasoned (dried) maximum spacing of 3/8” and avoid cupping/buckling from insufficient installation gap;
3. Install new docking related hardware – similar to existing.
2.2.4 TASK 4 - Tyndall Beach Boardwalk, B11300: Repair efforts are intended to rehabilitate the boardwalk to include phones stations and pavilion facilities that are exhibiting age and weather related degradation to the deck surface, supporting structure, and railing. Anticipated work will primarily involve replacement of the walking surface deck boards, railing and supporting structure. The pavilion portion of the work will involve replacement of the walking surface; two (2) intermediate rails;
replacement of degraded stringers; installation of two (2) additional rail posts; and securing the existing loose rail posts. The board walk is estimated to be 8 feet x 2,490 feet (total +19,920 ft²).
Period of performance for this task is anticipated to require 90 days. Task locations and extents are further defined in the Criteria Drawings located in Appendix A.
A. Demolition:
1. Remove and dispose of the decking boards;
2. Remove and dispose of the existing railing, existing carriage bolts in usable condition may be reused;
a. Raised pavilion portion of work is limited to removal of the two (2) intermediate 2x4 rails;
3. Remove and dispose of the deck supporting structure leaving the existing piles for reuse, existing carriage bolts in usable condition may be reused.
a. Raised pavilion portion of work is limited to stringers that are beyond the service life and found to be unusable; estimated to be 30% of the existing stringers.
B. Construction:
1. Adjust / reset existing 4x4 piles to accommodate construction of new supporting frame. Replace up to 20% of existing piles found to be in poor condition;
2. Provide and install new framing for the deck supporting structure in compliance with the Criteria Drawings. Salvaged carriage bolts may be used for connection of the split pile cap members;
a. Raised pavilion portion of work is limited to stringers removed during demolition that were beyond the service life and found to be unusable; estimated to be 30% of the existing stringers.
3. Install new 2x8 deck surface to meet the following requirements:
a. Deck screws shall be stainless steel;
b. Deck board spacing shall be selected by the contractor based on actual site and material conditions at the time of installation to achieve seasoned (dried) maximum spacing of 3/8” and avoid cupping/buckling from insufficient installation gap.
4. Install new railing in accordance with the criteria drawings. Salvaged carriage bolts shall be reused for the rail post connection to the stringer and piles;
a. Raised pavilion portion of work is limited to replacement of the two (2) intermediate 2x4 rails and installing two (2) additional 4x4 rail posts where the span exceeds 8 feet;
b. Raised pavilion portion of work is limited to removal of the two (2) intermediate 2x4 rails;
All work shall be in compliance with all Public Laws (P.L.), Executive Orders (E.O.), Military Standards (MIL-STD), Air Force Instruction (AFI), Code of Federal Regulations (CFR), Department of Defense Instructions (DODI) and Department of Defense Directives (DODD) or other higher authority documents as applicable, as listed in MIL-STD-3007F. Best Management Practices (BMP’s) shall be maintained during all land disturbing and in-water work.
2.2.5 Security Requirements:

AFI 31-101 Air Force Instruction 31-101: The Air Force Installation Security Program.
DOD 5200.1.R, Department of Defense 5200.1.R: Information Security Program Construction Standards UFC 4-010-01 Unified Facilities Criteria Antiterrorism Standards for New and Existing Facilities
2.2.6 Permits: (Where Applicable) The Contractor shall develop, coordinate, and be responsible for labor, professional services and other costs associated with acquiring any and all required state, local, and other applicable regulatory agency permits; base access (including off-base easements and leases), agreements, licenses, and certificates required to perform and complete the project. Coordination with regulatory agencies is required for all tasks. Tasks determined to be exempt from regulatory permitting during the agency coordination shall be documented in writing and submitted to the government prior to starting work for the respective task. The Contractor shall process an Air Force Form 103, Work Clearance Request, through the local Base Civil Engineer (BCE) office. The Contractor shall maintain a library of these documents on site as well as at the corporate facility handling each project. The Contractor shall comply with applicable permit conditions, environmental protection policies, best management practices and appended guidelines.
2.2.7 Construction: Upon approval by the CO for all preconstruction requirements, submittals, and permit documentation; the Contractor may commence on-site construction activities. The Contractor shall conform to all specifically referenced and other applicable requirements during performance of the work as described in the Contract Documents:
1. Mobilization: Mobilize equipment, labor, tools, materials, and ancillary items necessary to complete the work defined in the Contract Documents. Materials staging, storage and laydown are expected to occur within the work area footprint. Contractor use of areas outside of the work area footprint shall be requested in writing for approval in advance of use.
2. Demolition / Construction: Perform awarded tasks as defined in Section 2.2.
a. Protect work area with barricades, warning tape, signs and/or other appropriate methods for the work being completed to ensure the safety of pedestrians and Contractor personnel;
b. Care should be taken to avoid damaging installed utilities. In the event that other utilities are damaged during performance of the contract, the Contractor shall repair the damage at no additional cost to the Government.
c. Remove and dispose of all demolition and general construction debris from the site (off base);
d. Perform ancillary efforts required to complete the work in order to provide clean, complete and functional facility meeting the standard of quality for each trade utilized;
e. Limit disturbance to the natural vegetation, wildlife, water bodies and base activities to the greatest extent practicable.
2.2.8 Government Furnished Equipment: None.
3.0 GENERAL REQUIREMENTS
Applicable provisions of the Contract Clauses in the contract will govern work under each section of the specifications.
3.1 Pricing Schedule: The Contractor shall complete a Pricing Schedule and include the completed schedule with the submitted bid documents.
3.2 Kick-Off Meeting: A kick-off meeting shall be accomplished with the Contractor, Contracting Officer, Civil Engineering Squadron (CES), and user in accordance with the Table of Deliverables. The meeting objective is to review project details, determine construction start date, security procedures for site, dumpster locations, and any applicable requirements during construction. The Contractor shall visit the job site to verify location, conditions, and dimensions prior to starting work.
3.3 Facility Occupancy: The facilities covered by this contract is currently open to base personnel and experience light to moderate use. During performance of this contract, non-work areas will remain functional to normal base activities. The Contractor is responsible for diverting and restricting pedestrian and vehicle traffic in the active work areas for the duration of the task performance period. The contractor shall notify the Government a minimum of ten (10) days in advance of mobilization.
3.4 Identification of Employees: The Contractor shall be responsible for obtaining security clearances and badges for each employee, and for requiring each employee engaged on the work to display identification as approved and directed by the CO and Security Forces. The contractor shall submit personnel background check applications for badging in accordance with the table of deliverables. Badge processing typically requires fourteen (14) days to complete and badges are issued to the applicant in-person at the Tyndall AFB Visitor’s Center. During performance of the contract, prescribed identification shall immediately be delivered to the CO for cancellation upon release of any employee. When required, the Contractor shall obtain and provide fingerprints of persons employed on the project. All Contractor and sub-contractor personnel shall wear identifying markings clearly defining the company for whom the employee works.
3.5 Employee Parking: Contractor employees shall park privately owned vehicles in an area designated by the Contracting Officer. Contractor employee parking shall not interfere with existing and established requirements of the military installation.
3.6 Job Site Tools and Equipment: All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be in accordance with applicable OSHA regulations.
3.7 Notification Requirements: The Contractor shall notify the Government in writing a minimum of ten (10) days in advance of mobilization. The Contractor is required to notify the CO and Government Project Manager of critical issues that may affect the contract performance and/or human health and the environment. For critical issues pertaining to human health and the environment, the Contractor shall immediately notify the CO and stop work. Work shall only resume at the direction of the CO following resolution of the issue.
3.8 Environmental Protection: Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, critical wildlife, water bodies, and the general site environment. This includes the protection of trees, shrubs and other vegetation not in the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. If appropriate, the Contractor shall conserve uncontaminated topsoil by removal, storage, or redistribution. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. All fill materials shall be non-contaminated. The Contractor shall conduct operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone depleting chemicals, generation of hazardous waste, and use of energy and water. The Contractor shall plan, construct, operate, maintain, optimize, and commission systems necessary to control storm water run-on and runoff.
At a minimum, the Contractor shall employ best management practices consistent with the Florida Development Manual, Chapter 6, Storm-water and Erosion and Sediment Control Best Management Practices for Developing Areas.
3.9 Hazardous Waste Management: Contractor shall notify 325 CES/CEIEC if any hazardous waste is encountered or generated. The Contractor shall stop all work in the event 325 CES/CEIEC identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within two (2) hours of notification by 325 CES/CEIEC. All hazardous waste shall be labeled and an inventory management system will be initiated to insure timely removal and proper disposal. No on-base disposal will be allowed. All drums will be labeled with a hazardous waste label. The label shall include the proper Department of Transportation (DOT) shipping name, United Nations (UN) or North American (NA), EPA waste number, generator information, and accumulation start date. The label shall be placed on the side of the drum. All drums used to store hazardous waste shall be non-leaking and safe to handle. Contractor shall be responsible for over packing drums that are rusted, dented, or leaking. Drums and/or over packs shall be provided by the Contractor. All drums shall be "new" DOT approved containers. The storage location for the hazardous waste drums shall be approved by the 325 CES/CEIEC prior to the generation of hazardous waste. The Contractor shall document inspection of drums for leaks on a daily basis or if not working in the area daily, then a weekly inspection will suffice. A copy of the inspection checklist shall be forwarded to 325 CES/CEIEC every Friday. Hazardous waste transportation and disposal shall be coordinated through 325 CES/CEIEC. The Contractor shall be responsible for transportation and disposal of all hazardous waste at an EPA approved treatment, storage, disposal facility (TSDF). The transportation and disposal facilities shall be approved by 325 CES/CEIEC prior to their use. Manifests shall be signed only by 325 CES/CEIEC. Drums shall be disposed of within 90 days of placing the first drop in the container.
3.10 On-Site Field Oversight: The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure responsible oversight throughout accomplishment of the construction period. It is the responsibility of the Contractor to perform quality control measures to ensure full compliance with the contract documents. Contractor shall establish and maintain open communications between salient authorities associated with accomplishment of this contract. Oversite may be performed by a single Contractor representative for concurrent performed work.
3.11 Federal Holidays: Tyndall AFB observes the following Federal Legal Holidays;

3.12 Working Hours: The normal duty hours of the Base are from 7:00 AM to 5:00 PM, Monday thru Friday excluding Federal Holidays. All work shall be performed during normal duty hours. Work outside of the previously stated days and hours to include weekend and holidays shall be requested through the CO for approval. This request shall be submitted no less than three (3) workdays prior to the time requested.
3.13 Availability and Use of Utility Services: The Government will make all reasonably required utilities available to the Contractor from existing outlets and supplies, as specified in the contract.
3.14 Sanitation: The Contractor shall provide and maintain within the construction area minimum field-type sanitary facilities approved by the CO. Government toilet facilities will not be available to Contractor's personnel.
3.15 Clean Up: At the end of each working day, the Contractor shall clean up the site by the removal of all construction debris, waste materials, packaging material and the like. If the before mentioned items could become airborne they shall be placed in refuse containers immediately. Clean up of the work site shall include the construction area(s), construction office area(s), material storage area (s), parking and eating area(s), restroom facilities used by Contractor personnel, and any other area(s) affected by the construction process. Any dirt or mud which is tracked into a building, on to any street, parking area, sidewalk, patio, or driveway shall be cleaned away daily. Materials resulting from demolition activities which are salvageable shall be stored within the work area or at an approved supplemental storage area.
Stored material not in trailers, whether new or salvaged, shall be neatly stacked when stored in an approved location.
3.16 Contractor’s Management and Use of Electromagnetic Emission Devices on Base: (Electromagnetic emission devices include: Radio/Radar Transmitters; Navigational Aids/Instrumentations; Signaling, Intrusion Detecting, and Identification Devices; Mobile and Fixed Business Radio Communications Equipment; and MARS, CBs, and Amateur Radio Stations.) The Contractor shall comply with the Air Force Military Training Center (AFMTC) Regulation 700-14, Jul 89.
3.17 Preparation of Progress Schedules and Reports: The reports contemplated by Clause 52.236-15, Schedules for Construction Contracts, shall be accomplished on and in accordance with instructions pertaining to AF Form 3064, Contract Progress Schedule, and AF Form 3065, Contract Progress Report.
Contract Schedules and Reports shall run from Monday through Sunday and be submitted to the CO in accordance with the Table of Deliverables.
4.0 GENERAL SAFETY REQUIREMENTS
4.1 Workplace: Provide a safe and healthful workplace for employees and the general public while performing the contract work.
4.2 Personal Protective Equipment: Supply each employee with personal protective equipment. This equipment includes but is not limited to protection for the eyes, face, head, ears and extremities. Use the proper protective equipment for the work performed.
4.3 Roads: Obey the posted base speed limits. Wear Seat belts on base in accordance with AFI 91-207.
4.4 Accidents: Report Contractor accidents involving injury to Air Force personnel or damage to Government property to the Base Law Enforcement Desk and the CO.
4.5 Safety Regulations: Familiarize yourself and comply with the general safety requirements in accordance with DOL regulations, OSHA 29 CFR Part 1926, as well as applicable Air Force Office of Safety and Health (AFOSH) standards and Air Force Regulations (AFR). These standards and AFRs are available in the Wing Ground Safety Office, Building 662.

4.7 OSHA Inspections: The Contractor shall be subject to no-notice inspections under OSHA program by inspectors of the Department of Labor.
4.8 Protection and Maintenance of Traffic: During construction, the Contractor shall provide access and temporary relocated paths as necessary to maintain traffic that may be affected by construction activities.
The Contractor shall maintain and protect traffic on all affected roads/pedestrian paths during the construction period except as otherwise specifically directed by the CO. Measures for the protection and diversion of traffic, including the provision of watchmen and flagmen, erection of barricades, placing of lights around and in front of equipment and the work, and the erection and maintenance of adequate warning, danger, and direction signs, shall be as required by the State and local authorities having jurisdiction. The traveling public shall be protected from damage to person and property. The Contractor's traffic on roads/waterways selected for hauling material to and from the site shall interfere as little as possible with public traffic. The Contractor shall investigate the adequacy of existing roads and the allowable load limit on these roads. The Contractor shall be responsible for the repair of any damage caused by construction operations.
4.9 Barricades: The Contractor shall erect and maintain temporary barricades to prevent public access to hazardous areas as required by OSHA, Florida Department of Transportation, and/or like Agencies. Such barricades shall be required whenever safe public access to paved areas such as roads, parking areas, sidewalks or other work areas are prevented by construction activities or as otherwise necessary to ensure the safety of both pedestrian and vehicular traffic. The minimum required number of barricades shall be securely placed in a neat and orderly fashion, clearly visible with adequate illumination to provide sufficient visual warning of the hazard during both day and night. Barricades shall be maintained in an orderly fashion during the duration of their placement.
4.10 Severe Weather: Upon receipt by the CO of a severe weather warning with anticipated winds of 35 knots or higher, the following sequence of actions shall be carried out:
4.10.1 The CO or the Contracting Officer's Representative (COR) will notify the Contractor of the severe weather warning.
4.10.2 The CO or the COR will instruct the Contractor to secure all materials and equipment.
4.10.3 The Contractor shall take immediate action to tie down, remove, protect, and/or secure construction materials, equipment and refuse to the satisfaction of the Air Force Inspector in order to reasonably assure that Government property will not be damaged. If the Contractor fails or refuses to secure materials and equipment to the satisfaction of the Air Force Inspector, the work will be accomplished by Air Force personnel and the cost thereof charged to the Contractor.
4.11 Fire Regulations and Preventative Practices: The Contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association (NFPA), other recognized fire prevention agencies, and base regulations.
4.12 Explosive Operated Hand Tools: The Contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The Contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.
4.13 Welding: Welding shall not be performed without first obtaining a welding permit issued by the Base Fire Department.
5.0 GENERATED REFUSE
5.1 Refuse: The Contractor shall take positive action to prevent work-generated refuse from entering the sewer system and water bodies. All Contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in with tight-fitting beds and covers to prevent spillage on roadways and waterways. The construction site shall be kept neat, orderly, and safe for workmen at all times.
5.1.1 All waste materials other than uncontaminated recyclable metals generated by any work under this contract shall become the property of the Contractor and shall be handled, transported, stored and disposed of off-base by the Contractor at the end of each day's work. Such material shall be hauled OFF BASE to a Contractor selected disposal area and disposed of in accordance with applicable Florida Administrative Code 62-701, federal, state and local laws, ordinances, regulations, court orders or other type of rules or rulings having the effect of law. The Contractor shall provide the CO with the waste manifest tickets for all disposal runs associated with the project.
5.1.2 Salvageable, goods shall be turned in to the appropriate base agency. The Contractor shall separate and turn in recyclable materials to the 325th Services Squadron Recycling Facility, building 6029, in accordance with base policies. Precious scrap and waste metals (e.g. steel, iron, etc.) shall be recycled and/or disposed in accordance with base policies.
6.0 UTILITY OUTAGES
6.1 Definition: Utility outages shall be defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.
6.2 Outage Coordination: All utility outages shall be coordinated with the Using Agency through the CO or the COR. Notice of the necessity for an outage shall be provided by the Contractor to the CO, in writing, not less than seven (7) working days prior to the time of the required outage and written approval for the outage will be issued by the Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.
6.3 Restoration: Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.
6.4 Cancellation: In the event a scheduled utility outage is cancelled by the Government, notification will be given to the Contractor at least 24 hours in advance of the time for the outage to start. Rescheduling of a cancelled outage must be coordinated with written notification as described above.
7.0 EXISTING UTILITIES
7.1 Reference Drawings: Record drawings showing approximate location of existing underground utilities are located in Building 421, Tyndall AFB. The Contractor shall avail himself of the drawings. Any utility line shown on the record drawings (or made known to the Contractor) and damaged during construction work shall be repaired immediately by the Contractor at no cost to the Government.
8.0 DELIVERABLES / SUBMITTALS
8.1 Contract Progress Schedule (AF 3064): The Contractor shall submit a signed AF 3064 (or approved equivalent), Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance and submitted as described in the Table of Deliverables.
The line items, associated project percentages, and dates shall accurately depict the planned work to be performed. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The progress schedule shall be signed by the either the COR or the government contract inspector recommending approval and approved by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.
8.2 Contract Progress Report (AF 3065): The Contractor shall submit AF 3065 as described in the Table of Deliverables. This report shall be signed by the Contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. The Contractor shall submit the Contract Progress Reports to the CO to obtain acceptance by signature of the actual reported progress. Additionally, the Contractor shall submit an updated AF 3064 as an attachment to each submission of the AF 3065.
8.3 Material Approval (AF Form 3000): The Contractor shall prepare and submit the AF Form 3000 with all submittals to the Government. The form shall document the contents of the submittal and be signed by the contractor. Form submission is a formal statement that the contractor has reviewed the contents and verified full compliance with the contract documents for of the submitted items. Approval of the submittals by the government is an acknowledgement of receipt and does not relieve the contractor from compliance with the contract documents.
8.4 Work Clearance Request / Digging Permit: Obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE REEQUEST (often called a digging permit) from 325 CES Customer Service, (850) 283-4949. Contractors shall request a digging permit in accordance with the Table of Deliverables. In-place utility locations shall be verified by the Contractor with on-site verification as needed prior to the start of construction activities.
8.5 Construction Material Testing: The Contractor shall employ a qualified third party material testing company to verify that soil and concrete material requirements are met. Test reports will be submitted to the government when the formal reports are received. Field reports shall be maintained on site for government reference at all times.
8.6 Hazardous Material and Hazardous Waste: The Contractor shall submit TAFB Form 81 (Contractor Questionnaire) and TAFB Form 82 (Chemical Inventory) in in accordance with the Table of Deliverables and prior commencement of work on site. The Contractor should note that Tyndall AFB is required to report chemicals such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, solvents, oils, cleaners, degreasers, portable fuel containers and pesticides. Copies of manufacturer-specific Safety Data Sheets must be attached to TAFB Form 82. After submission, 325 CES/CEIEC will notify the Project Manager and/or CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit TAFB Form 83 (Reporting Entry Form) at the end of the project in accordance with the Table of Deliverables. 325 CES/CEIEC must be notified if anything changes from the original submittal (i.e. new chemical is added, size of container or unit of issue changes or if the manufacturer changes).
8.7 Transfer And Acceptance of Military Real Property (If Applicable): The Contractor shall prepare and submit a DD Form 1354, Transfer and Acceptance of Military Real Property document. The draft and final copies shall be submitted in accordance with the Table of Deliverables.
8.8 Pre-Final Inspection: The Contractor shall conduct a pre-final walk through inspection with Base personnel and publish the pre-final inspection findings in a pre-final inspection (punch list) report in accordance with the Table of Deliverables. The Contractor shall include an updated draft DD Form 1354, Transfer and Acceptance of Real Property to the COR for review.
8.9 Final Inspection: The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report for each project task upon substantial completion of work. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the Contractor’s Quality Control personnel have verified full compliance with the contract documents and the pre-final inspection (punch list) report has been resolved. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the BCE or other appropriate organization for signature and acceptance, if required.
8.10 Delivery/Warranty: The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the warrant documentation. The warranty shall be issued in accordance with FAR 52.246-21.
8.11 Table of Deliverables: Project deliverables shall submitted in accordance with following table.

9.0 CONTRACTOR’S QUALITY CONTROL
9.1 Quality Control: The Contractor is responsible for quality control and shall establish and maintain an effective quality control system in compliance with FAR Clause 52.246-12, Inspection of Construction. The quality control system shall consist of plans, procedures, and organization necessary to manage all delivery orders to produce end products which comply with the contract requirements. The system shall cover all construction operations, both on site and off site, and shall be keyed to the proposed construction sequence. The Government will hold the project manager responsible for the quality of work on the job and is subject to removal by the CO for non-compliance with quality requirements specified in the contract. The Contractor shall verify full compliance with the contract documents prior to requesting final inspection(s).
9.2 Field Measurements: The Contractor shall be required to make his/her own field investigations to verify existing conditions, dimensions and other information shown on Government furnished reference drawings, solicitation documents and construction documents. Deviations, errors, or omissions in the government supplied reference materials or the contract documents shall be identified and addressed by the contractor prior to the start of construction.
9.3 Siting Location: The contractor is responsible for field reconnaissance, surveys, site investigations and testing required to obtain information necessary for the accomplishment of the project in accordance with the contract documents. In-place utility locations shall be verified by the Contractor prior to the start of construction activities. Verification shall be accomplished by a Work Clearance Request / Digging Permit and shall include on-site verification as needed.
10.0 INSPECTION AND ACCEPTANCE
10.1 Provisions: Government inspections and final acceptance shall be scheduled by the Contractor as the work is completed with the following additional provisions:
a. The Contractor schedule all requested inspections in accordance with the Table of Deliverables.
b. The Contractor shall verify that the work to be inspected is in full compliance with the contract documents prior to the requested date of inspection. The presence of an unreasonable number of deficiencies, as determined by the COR, may cause the inspection to be rescheduled.
c. The Contractor will be given a certificate of acceptance by the COR for each successfully completed step of the work. The certificate will be signed by the COR and shall be initialed and dated by the Contractor’s approved representative. One copy of the certificate will be given to the Contractor.
d. Contractor shall complete a CES approved Contractor Daily Report form and submit to COR by 1000 hrs the next day after work is performed and inspected by the Contractor’s Quality Control (CQC) Representative.
11.0 DETERIORATED CONSTRUCTION AND FINISH
11.1 Authority: The COR will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement.
12.0 WATER CONSERVATION
12.1 Water Conservation: Conserve water to the greatest extent possible.
12.2 Restrictions: If Tyndall AFB implements water usage restrictions, the Contractor is required to comply with these restrictions, at no additional cost to the Government. In the event the Contractor cannot comply with water usage restrictions and still meets other contract requirements (particularly when installing sod or hydro-mulching), then he can request permission from the CO for relief from the restrictions.
12.3 Waivers: Submit request for waivers to water conservation restrictions in writing. The CO will consider such cases on an individual basis and grant waivers only in extreme cases. Submit detailed requests with appropriate manufacturer’s/supplier’s data to show unavoidable noncompliance with the water usage restrictions. In the event a restriction is conditionally waived, use only the minimum amount of water necessary to comply with contract requirements.
13.0 CONTRACTOR STORAGE/STAGGING AREA
13.1 Operations and Storage Approval: Notwithstanding Contract Clause, FAR 52.236-10, Operations and
Storage Areas, subject to approval by the CO and in turn, availability and need, a storage/staging area shall be provided at the subject project site.
13.2 Storage Area Clean Up: At work completion, clean the storage area. This area must be free of any materials, trash or debris before receipt of final payment. Existing fencing shall remain Government property.
14.0 CONTRACT ADMINISTRATION / POINTS OF CONTACT
14.1 Contracting Officer Authority: CO, the term used herein, does not include any representative not acting within the scope of his/her authority. Notwithstanding any of the provisions of this contract, the CO shall be the only individual authorized to in any way amend or modify the terms of this contract.

14.4 Technical Representative: 325 CES/CENMP is designated as the technical representative to ensure progression, workmanship, and inspection of materials for work being performed under this contract. This clause in no way authorizes anyone other than the CO to commit the Government to changes in the terms and conditions of the contract.
14.5 Field Changes: The Contractor and the Government may agree to perform a no cost field change. Field changes are made when the change appears to be mutually beneficial to all parties and would not require changing the negotiated items. All field changes must be approved by the CO prior to execution.
