STATEMENT OF WORK

MOBILE SHELVING UNITS DELIVERY, INSTALLATION AND ASSEMBLY

1. SCOPE: The work covered by this Statement of Work consists of furnishing all labor, tools, equipment and supplies necessary to deliver, install and assemble all relocatable mobile safety shelving units at the 334th, 335th and 336th AMUs.

Special instructions: These buildings will be occupied. Contact building Facility Managers to schedule delivery and installation of the required shelving units within the specified contract completion time frame.

All work is to be accomplished at Seymour Johnson AFB NC. All measurements shall be taken and provided in traditional English format. Measurements provided using an alternate means will be rejected. It is the responsibility of the Contractor to take accurate measurements for the purposes of installation and manufacturing of all required shelving and ancillary components. All color and material selections will need to be approved prior to manufacturing and delivery. Proposed colors will be submitted to Contracting Office after award to be reviewed and approved by End User. Vendor may submit more than one color option to expedite approval process. End User representatives will conduct QA inspections to ensure all requirements of the SOW are being met. The point of contact for this project will be Mr. Bryan Brown. 2. SPECIFICATIONS: See attachments 2, 3 and 4.

3. HOURS OF OPERATION: The hours of operation shall be from Monday through Friday 0730-1630. Monday may be utilized for setup of equipment and minimal other preparations. Friday is provided to complete the install and encourage an economical and safe environment for the contractor to work within. If Friday is utilized, there shall be no deliveries or large installs planned on that day unless unforeseen circumstances warrant it. The work will be accomplished by competent crews to install the new shelving equipment. The work group will be large enough to accomplish this entire contract in three days or less per site.

4. STANDARDS: At the end of the day the contractor will need to do a thorough check in and around the area where they have been working to make sure that all trash and broken down delivery boxes have been picked up and taken with them for proper disposal. This is to ensure that their work area is tidy and safe before their departure. The contractor shall not schedule work on any Federal recognized holiday which include: New Year’s Day, Martin L. King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day.

5. WORKMANSHIP: All workmanship will comply with standard trade practice. The contractor shall provide all qualified personnel, equipment, and services required to perform the work.

6. CLEANING: The contractor shall keep the work site free from accumulation of waste materials. All waste generated by any work under this contract shall be handled, transported, stored, and disposed of off base by the contractor in accordance with all applicable federal, state, and local laws. Daily and upon completion of this contract, the contractor shall leave the work site and premises in a clean, neat condition, satisfactory to the contracting officer or representative overseeing the work.

7. PROTECTION OF FACILITY: The contractor shall be responsible for protection of grounds, buildings, and equipment therein from loss or damage while performing services called for in this contract. ·

8. WARRANTY: Contractor will guarantee all workmanship and material defect free for a lifetime from completion of this work.

9. SAFETY: The contractor will abide by all OSHA requirements and any other local or standard trade practices to make a safe work area for not only their workers but our office staff as well.

10. STORAGE AND STAGING: All items in this contract will be stored at the contract awardees' expense. Under no circumstances will the Government provide storage for any contractor for any reason. Under no circumstances will the Government receive or unload any items in this contract or otherwise for the contractor. If a truck or package arrives and there are no contractors here to retrieve it, it will be refused at the contractor's expense. The only role that the government will take is getting the contractors on base from the gate, escorting the contractors to the site and assisting with staging. A staging area near the work space will be provided by the Government to the Contractor. This area can be utlized for placing a storage container or shipping crates. The staging area will be located directly behind the support sections entrance and will be specifically defined during the initial site visit. This area must be kept clean at all times, and may not at anytime impede the normal traffic flow/mission requirements.

11. AUTHORITY: The Government, with the exception of the 4th Contracting Squadron, has no authority to make changes to any part of the contract including the statement of work and all of the items in this contract. If anything on the contract changes, arrangements will need to be made with Contracting Office to approve the changes. Any changes made by the Contractor without express written consent of the 4th FW Contracting Squadron, will be at the Contractor’s expense.

12. STANDARDS OF CONDUCT ON SITE: Be professional and courteous at all times. No misconduct will be allowed before, during or after set-up to any guest, resident, or contractor. Finally, there will be NO SMOKING, except for within appropriately designated smoking areas. Guest to SJAFB whom are violators of the installations rules and standards will be asked to leave and not return.

13. DISPOSAL: All waste material generated by any work under this contract shall be handled, transported, stored, and disposed of off base, by the Contractor, in accordance with all applicable federal, state, or local laws, ordinances, regulations, court orders, or other types of rules or rulings having the effect of law.
