3. TITLE OF PROPOSED ACTION

Repair Sewer Manholes

4. PURPOSE AND NEED FOR ACTION (Identify decision to be made and need date)

1. What do you intend to accomplish and why is the action necessary?

Repair 23 sewer manholes on Base. Repairing of manholes is necessary to prevent inflow and infiltration to the main in-service sewer system which causes substantial dynamic flow to the base WWTP and spreads bacteria due to surface overflow from the system.

2. What is currently being done to meet the need?

System has been inspected, leak sources have been identified and reported for correction.

3. Provide any additional details related to the Purpose and Need for Action.

The purpose is to reduce the flow volume on WWTP and prevent further spreading of bacteria. Repairing the manholes and leak sources will satisfy the need.

Need Date: 03/29/2024

5. DESCRIPTION OF PROPOSED ACTION AND ALTERNATIVES (DOPAA) (Provide sufficient details for evaluation of the total action.)

1. What other alternatives have been considered (to include the no action alternative)?

No action alternative will continue the inflow and infiltration that damage the in-service WWTP system due to a high-volume flow and spreading of bacteria over the ground surface that contaminates the underground water and surrounding atmosphere.

2. What alternatives were eliminated from consideration and why?

Digging and filling around the back side of the manholes were eliminated from the alternative due to the environmental restrictions and high-cost scope compared to the proposed action.

3. Please provide a description of the construction action and timing when it will occur.

Repair of inside the 23 sewer manholes with fill/grout of leaking using Plain Cement Concrete (PCC) for blocking the abounded network lines inside the manholes and replacing the cover of up to two manholes. Work is intended to begin and complete within April, May, and June of 2024.

4. Describe the project location. Attach map(s)/diagram(s) – make sure to include an overview map of where your requested project area is on the installation.

Site map with locations of all manholes is attached

5. Describe additional project requirements: 1) Construction and site preparation requirements (include approx. area of ground to be disturbed); 2) Does the project require a laydown yard or storage area? If so, describe the location and groundwork required.

1) No site preparations required.

2) A specific laydown area is not needed.

6. Describe additional project requirements: 3) Will soil boring/sampling/potholing occur during a design phase? If so, a separate dig permit will be required; 4) Detailed operational activities; 5) Equipment/material lists.

3) N/A. Ground disturbance will not occur.

4) Same as existing.

5) A mobile UBT vehicle will be used, parked on a hard surface close to each work location.
