Q & A AMENDMENT 03
The purpose of this amendment is to:
• Answer questions arising from the site vist/preproposal conference held on 16 May 2012
• Incorporate a revised Statement of Work dated 29 May 2012
• Incorporate a revised Hazmat Survey dated 29 May 2012
1. Questions and answers arising from the site vist/preproposal conference:
A. QUESTION: The seed project specs Statement of Work - 4.0-C-3 notes that the disturbed surfaces are to be patched to match paint and texture. In the Lead Based Paint section item 1A – third paragraph it reads that painting of the abated areas is not included in the project.
Please clarify.
ANSWER: Please see revised Hazardous Materials Survey, Section 028399, 1.01A, page 56, dated 29 May 2012.
B. QUESTION: In the lead based paint removal section 1A – third paragraph – it reads that the lead based paint on all stairwell walls and outside perimeter is to be removed. In section D it reads that lead based paint in areas to be disturbed by core drilling and hanging of pipes is to be abated. Please clarify the extent of the lead based paint abatement.
ANSWER: Refer to question A.
C. QUESTION: The specifications call for a Fire Protection Engineer to stamp the drawings.
Many of the fire suppression companies proposing on this job have NICET Level 3 minimum and are able to prepare designs and receive a letter of acceptance from a fire protection engineer. Fire Protection Engineers will not stamp drawings that they do not design themselves. Will a letter of acceptance from a fire protection engineer be acceptable, if the company doing the design has a NICET Level 3 or greater designation?
ANSWER: See revised SOW dated 29 May 2012, paragraph 4.0 C.1
D. QUESTION: Please provide recent water flow information at building site. Water static pressure, Petot psi, Pressure drop from read hydrant, gallons per minute availability.
ANSWER: Collecting this information is part of the Contractors design process. Successful bidder may contact our water department to determine what information is available.
E. QUESTION: Are there any existing code violations with the existing fire sprinkler system that require correction and should these be included in our proposal?
ANSWER: No known code violations. If encountered they would be considered a differing site condition and treated as required.
F. QUESTION: Per SOW page 3 of 4 Paragraph C note 2 . Contractor is to provide abatement as needed for Fire Protection wall penetrations for pipe supports. Confirm these are the only locations lead paint abatement to take place and not as stated in specification section 028301 Lead-based Paint Removal (Paragraph A, Bullet 3) indicating that all lead paint in stairwell is to be removed.
ANSWER: Please see Hazardous Materials Survey dated 29 May 2012 page 56
G. QUESTION: SOW Paragraph C note 3 (Page 4) states to “Restore all disturbed surfaces to match paint color, texture of adjacent surfaces.” Specification Section 028301 states “Repainting of the stairwell walls is not in this scope of work. Please clarify if painting is required and to what extent.
ANSWER: The contractor will only be responsible for painting and texturing stairwell wall areas disturbed by this project. Please see Hazardous Materials Survey dated 29 May 2012
H. QUESTION: Please verify that, in the case of a joint venture, both members of the joint venture are to submit up to five (5) past performance projects each, for a total of ten (10) permissible projects for two joint venture members.
ANSWER: Yes, see paragraph 2.2.2
I. QUESTION: Please advise whether a joint venture should submit a copy of the executed Joint Venture Agreement with the proposal, and, if so, which volume it should be included in.
ANSWER: Yes, see paragraph 1.4. Please submit information in Volume II, section 4.
J. QUESTION: Please advise if any additional documentation related to the joint venture should be provided as a part of a joint venture proposal and where it should be included.
ANSWER: A list of Joint Venture partners with Volume I summary page. All other approved Joint Venture past performance information should be submitted in Volume II.
K. QUESTION: Please verify that the attendence at the pre-proposal conference was not mandatory to be eligible to submit a proposal.
ANSWER: The pre-proposal conference was not mandatory, all qualified 8(a) interested parties are eligible to provide a proposal for this solicitation.
L. QUESTION: Are completed CCASS evaluations from the PPIRS system and/or previously completed Past Performance Questionnaires (on USACE/NAVFAC Questionnaire Form PPQ-0 or equivalent) acceptable in lieu of a completed Attachment 5?
ANSWER: CCASS evaluations are not a suitable substitute for the Past Performance Questionnaire. Please submit a Questionnaire for each project submitted in your past performance Volume.
M. QUESTION: Page 61 of 67 - 2.2.3. Is the last sentence of the paragraph asking for relevance of our past projects to the seed project or the MATOC program as a whole? In general please explain the last sentence.
ANSWER: The past performance should be relevant to the MATOC Program as a whole.
N. QUESTION: Page 61 of 67 – 2.2.4 The paragraph starts out with “may include” and ends with “is required”. Which is it and what exactly is the paragraph asking for?
ANSWER: Paragraph 2.2.4 relates to the identification of specific issues and risks that have been overcome. The prospective offeror should address as deemed appropriate.
O. QUESTION: On the performance questionnaire it indicates that clients are to send their questionnaires to Richard Hedrick, however, 2.2.6 of the solicitation instructions states to send it to Jeremy Howard. The fax and address are the same but who should clients put this attention to?
ANSWER: Both individuals are correct however MSgt Jeremy Howard at Jeremy.howard.2@us.af.mil would be the most appropriate.
P. QUESTION: Section 2.2.2 outlines the requirements for “Past Performance Information Sheets”. Section 2.2.3 outlines the requirements for “Relevant Contracts”
These two sections seem to be requesting the same, if not very similar information. Is the government’s intent to have a response for “projects” and one for “contracts” or is the request for 5 projects in 3 years, following the guidelines found in 2.2.2 and 2.2.3?
ANSWER: The Past Performance Information Sheets described in 2.2.2 will be used to assist the evaluators in determining project relevancy as described in 2.2.3
Q. QUESTION: Also, section 2.2.2 states to submit up to 5 Past Performance Information sheets. However attachment “2012 MATOC Atch 4 Past Performance Info 1 May 12” states in the title “Limitations: 3 Contracts”. I presume the 5 contracts are correct, please verify.
ANSWER: Five Past Performance Information Sheets is correct.
R. QUESTION: Does the Air Force intend to release the questions and answers generated during and/or as a result of the pre-proposal conference, if so, when can we expect them?
ANSWER: Yes, see amendment 03.
