Questions & Answers 1)Question: Would letters of recommendation serve the same purpose as PPQs or would they be accepted in addition to the PPQ?
Answer: In accordance with, the solicitation Clause 52.212-1 addendum (C) 2. Part II- Past Performance Information, letters of recommendation is accepted as past performance information and in addition to the PPQ. If you include letters of recommendations, please make sure to include all the following information:
Provide a list of no more than five (5), of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. Relevant contracts include work of a nature, magnitude and scope similar to that required for this contract within the past three years from date of solicitation: relevant contracts include those requiring inspection, maintenance and repair of fire suppression systems.
The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed:
(i) Company/Division name
(ii) Product/Service
(iii) Contracting Agency/Customer
(iv) Contract Number
(v) Contract Dollar Value
(vi) Period of Performance
(vii) Verified, up-to-date name, address, FAX & telephone number of the contracting officer
(viii) Comments regarding compliance with contract terms and conditions
(ix) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.
2) Question: Do you want hard copies of our proposal or would an emailed proposal be sufficient?
Answer: Emailed proposals are sufficient.
3) Question: Does the Offeror need to complete provision FAR 52.212-3 in the solicitation separately that the completed annual representations and certifications electronically via the System for Award Management (SAM)?
Answer: Answer: The Offeror only needs to complete paragraph (b) of provision FAR
52.212-3 if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov. If the Offeror has not completed the annual representations and certifications electronically, the
Offeror shall complete only paragraphs (c) through (p) of this provision.
4) Question: As referenced in provision FAR 52.212-3, does a company with 50 or less employees need to complete an Affirmative Action Program?
Answer: IAW 22.804-1, a non-construction prime contractor and each subcontractor with 50 or less employees is not required to develop a written affirmative action program.
5) Question: The required information on the past performance questionnaire/ information do not apply to all predecessor companies; is it okay , to leave the required information blank?
Answer: Yes, the required information on the past performance questionnaire/ information can submitted blank; if the information is not applicable to the predecessor company.
