DSTATEMENT OF WORK

Repair Interior Finishes Bldg 1085 Alert Facility

SABER PROJECT QJVF 11-0142

25 January 2013

1. GENERAL: The Contractor shall provide all labor, material and equipment to accomplish a complete and functional installation of the proposed project. The installation shall be in accordance with the Simplified Acquisition of Base Engineering Requirements (SABER) specifications, attached drawings and requirements of the Statement of Work (SOW) as stated in the following paragraphs.

2. PERFORMANCE PERIOD: This contract shall have 90 days performance period and a total of 60 days on site. Coordinate work area with Contract Management and Using Agency.

3. GENERAL SCOPE OF WORK: Work on site shall include demolition and installation of ceiling drywall, flooring, cove base, ceiling tiles and grids in corridors identified in Building 1085. Additional work shall include the removal and resetting of electrical plate covers, fire extinguisher cabinets, fire alarm covers, exit lights, coaxial outlet cover and water chiller as identified in drawing. Work shall also include demolition, removal and installation of Water closet, Toilet partitions, Lavatory, P-traps, Urinals, Urinal screens, Faucets, waste receptacles, and toilet accessories. Contractor shall also demolish, remove and install new countertops with backsplash in the restroom areas indicated in drawing. All new finishing, mechanical and electrical shall be completed and return to the original state on all areas identified in the project. Transportation, equipment, tools, supervision, labor, materials and performance of all work shall be in accordance with the specifications, attached drawings, standard industry practice, current codes to include National Fire Prevention Association (NFPA) codes, the National Electrical Code (NEC), International Mechanical Code (IMC), International Plumbing Code (IPC), International Building Code (IBC), International Fire Code (IFC) and manufacturers recommendations.

4. NOTIFICATION: Contractor shall provide written notification to the Contracting Officer two weeks prior to commencement of work. Contractor is to provide a work schedule showing area to be worked in, date of work, and estimated time for the completion of the project. Submit schedule to the Contracting Officer (CO) for approval and coordination. Coordinate proposed schedule 7 days in advance of work to be accomplished with the Construction Manager, who will make arrangement with the using agency. Work is to be performed to the satisfaction of the Contracting Officer (CO).

5. DEBRIS: Contractor shall maintain a normal safe work area and shall remove all construction rubble from the work site to a Contract Authorized onsite Construction Dumpster or remove from Minot AFB on a daily basis. Contractor is responsible for the security of tools and equipment being used at the project site.

6. SUBMITTALS: Samples shall be submitted in accordance with submittal schedule if applicable.

Contractor shall provide product data, color chip, wall covering sample, etc., if selecting material sources other than those recommended for work. If submitting specified manufacturers materials, the contractor shall submit only product data (e.g. catalog picture and part number or copies). All materials that may differ in color, texture or tint (i.e., paint, wall covering, floor tile, etc.) shall be purchased with identical part and lot numbers.

Contractor shall provide 5 copies of each submittal. (*) Submittal required on all items marked with this symbol.

7. LOCKS: Upon demolition all existing combination cores and blanks shall be turned over to the government. A construction core must be installed in each existing lock with two keys. All new locks shall be Best Lock compatible with Minot Air Force Base lock system.

8. SITE DAMAGE: Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the government. NO VEHICLES shall be driven on the sidewalks or grass.

9. CONTRACTOR’S WORK HOURS: Working hours for the Contractor will normally be five - 8 hour days between the hours of 7:30 a.m. and 4:00 p.m. excluding Saturdays, Sundays, and Federal Holidays.

10. CONFINED SPACE: The work site is/ is not in a confined space. Therefore the Contractor shall conform to OSHA guidelines and requirements for confined space. Workers will be required to have an attendant with them at all times while working in a confined space. The Using Agency will not provide an attendant for the Contractor.

11. RECYCLING: Recycling/Reuse of excess materials and construction and demolition (C&D) debris. The contractor shall reuse/recycle excess materials and C&D debris to the maximum extent possible. C&D debris are those excess/residue materials resulting from construction or demolition activity that would normally be discarded to a landfill. C&D debris includes, but is no limited to soil, rock, vegetation, concrete, asphalt, and building materials such as windows, wood, metals, and roofing materials. The contractor shall recycle C&D debris and excess materials, if determined to be economical. All C&D debris resulting from demolition shall become the property of the contractor unless specified otherwise or as determined by the Contracting Officer.

• Materials not to be returned to the Govt. and all contractor-generated rubbish, if not recycled, shall be disposed of by the contractor off-base at his own expense, and in compliance with applicable environmental codes and regulations of governing authorities.

• Reporting tonnage of C&D debris: Contractor shall track and report the tonnage of C&D debris in two categories.

• Tons of C&D that is or will be recycled or reused.

• Tons of C&D that is deposed in a landfill.

The C&D recycled/disposed tonnage report shall consist of a written memo with the tonnage of each category (recycled and disposed tonnage) submitted to 5 CES/CEVP through the Contracting Officer. Submit C&D recycled and disposed tonnage report when the project is completed.

12. PERMIT:

• AF Form 103: Contractor shall coordinate with appropriate utility locating agencies, including base communications cable maintenance (item 1 through 15) on an AF Form 103, Base Civil Engineering Work Clearance Request. AF Form 103 will be given to the Contractor during the pre-construction meeting. Contractor shall complete and follow all instructions on AF Form 103 before start of work on base. A copy of AF Form 103 instruction must be on the construction premises at all times while work is being performed.

• AF Form 592: When welding, cutting, brazing or use of any open flame device is to be used, an AF Form 592 permit must be obtained from the Base Fire Department (Building 897, 435 Peacekeeper Place) prior to beginning such work. During such work, a fully charged and operable fire extinguisher, appropriate for the task, shall be provided by the Contractor. Contractor shall process form for approval.

PROJECT REQUIREMENT

1. DIVISION 01 – GENERAL REQUIREMENTS

1.1. The Contractor shall provide separation dust and construction barriers between occupied spaces during demolition and construction. The work area is to be cleaned on a daily basis along with any dust/debris that is transmitted in to the corridor/shared areas of the facility.

• Provide barriers as required to prevent public entry to construction areas and to protect adjacent occupied spaces from damage from construction operations.

• Provide temporary partitions and ceilings as required to separate work areas from Using

Agency occupied areas, to prevent penetration of dust and moisture into occupied areas, to prevent damage to existing areas and equipment.

• Provide temporary protection for installed products. Control traffic in immediate area to minimize damage.

• Control accumulation of waste materials and rubbish; periodically dispose of off-site daily.

• Clean interior areas prior to start of finish work, maintain areas free of dust and other contaminants during finishing operations.

• Remove temporary materials, equipment, services, and construction prior to substantial completion inspection.

• Clean and repair damage caused by installation or use of temporary facilities and services;

restore existing facilities used during construction to specified, or to original condition.

• The Contractor shall maintain all work areas free from excess dust to such reasonable degree as to avoid causing a hazard or nuisance to the Using Service or to others.

1.2. The work shall mean to provide and install a complete finished, operating, functioning, assembly, framework, system, fixture, surface or apparatus ready for use by the government.

1.3. Contractor shall install all piping and fixtures following UPC and North Dakota Plumbing Codes.

Inspection is required prior to concealing any utilities in floors, ceilings or walls. Inspectors will insure the proper installation of piping, insulation, and supports.

1.4. Fixtures, fittings and equipment items may have approved equals. **Submittal required for any change in equipment.

1.5. Contractor shall verify all dimensions including drawings and SOW prior to any construction activities and check drawings for locations.

1.6. All construction products shall be used as identified by government or an approved equal.

2. DIVISION 02 – SITEWORK

2.1. The Contractor shall obtain an approved AF Form 103, "Base Civil Engineer Work Clearance Request" prior to proceeding with work.

2.2. The Contractor shall comply with all OSHA and environmental codes for demolition of structures, safety of adjacent structures, dust control, runoff control and disposal of materials.

• Conduct demolition to minimize interference with adjacent occupied spaces.

• Protect existing walls and utilities which are not to be demolished.

• Cease operations immediately if adjacent structures appear to be in danger. Notify the

Contracting Officer and do not resume operations until directed.

• Conduct operations with minimum interference to public or private accesses. Maintain egress and access at all times.

• Disconnect, remove, cap and identify designated utilities within demolition areas.

• Remove materials to be re-installed or retained in manner to prevent damage. Store and protect in accordance with directions from the Contracting Officer.

• Remove demolished materials from site.

2.3. ITEMS TO BE REMOVED: Ceilings, Walls, Floor coverings, cove base, electrical plates, lights, diffusers, toilet paper holder, toilets partitions, water closets, urinals, traps and countertops.

• Protection: Do not interfere with use of adjacent offices. Erect and maintain dustproof partitions as required to prevent the spread of dust, fumes or smoke to other parts of the building.

• Demolish in an orderly fashion; being careful not to damage any new work or existing work to remain. Any areas damaged shall be repaired by the Contractor at no expense to the Government.

• Remove all materials from the site daily as a minimum, dispose of off-base and in accordance with all applicable regulations of the State of North Dakota and the Environmental Protection Agency.

• Remove all dustproof partitions upon completion of the project and patch to match existing conditions.

6.1. Men Restroom

• Demo and remove the existing Countertop and the four inches backsplash in restroom, remove caulking compound adhering to the wall tiles.

• Provide and install new 9′6″L x 2′0″W Countertop with new four inches (4″) backsplash in restroom. **Submittal required.

6.2. Women Restroom

• Remove and reset the existing countertop and the 4” backsplash in the women restroom.

7. DIVISION 07 – THERMAL & MOISTURE PROTECTION

8. DIVISION 08 – DOORS & WINDOWS

8.1. Remove and reset the existing solid wooden door at the women restroom, paint over frame and re-varnish the existing door. **Submittal required.

8.2. The contractor shall field verify all dimensions and identify the indicated door opening prior to starting work.

9. DIVISION 09 – FINISHES

9.1. Corridor Areas/Bedroom

• Demo and remove the existing Vinyl Cove Base in all indicated walls in Corridor and Bedroom areas. Provide and install new 4″ cove base along the Corridor and Bedroom areas.

**Submittal required.

• Contractor shall remove and reset the recessed fire exstinguisher cabinet cover in the corridor areas where applicable.

• Contractor shall not remove or reset the sprinkler heads in the corridor areas.

• Contractor shall demo, remove and replace the existing drywall with the adhered burlap materials in the Bedroom areas. Patch up damage spots on walls where applicable. Prepare and paint over the existing drywall in the Corridors and Bedroom areas. The new paint color sample shall be selected by the Users. **Submittal required.

• Demolish and remove the existing ceiling tiles and grid along the indicated Corridors and Bedroom in the Basement floor areas.

• Provide and install new ceiling tile and grid in the basement Corridor and Bedroom areas.

New ceiling tile texture shall be a Glacier panels, 2′ x 2′x ¾″ foil back, class A, white standard ceiling tile. **Submittal required.

• Demo and remove the existing carpet along the Corridor and Bedroom areas as indicated in drawing. Along the same Corridor and Bedroom areas provide and install new medium traffic, commercial grade carpet and a floor transition strips. **Submittal required.

9.2. Women Restroom

• Demo the existing ceiling drywall in women restroom area and ensure not to damage the existing wall drywall during ceiling wall demolition. Replace with new 2′ x 2′x ¾″ kitchen / bathroom style suspended ceiling tiles. **Submittal required.

• Patch up damage spots on wall where applicable in the restroom, prepare and paint over the existing drywall. The new paint color sample shall be selected by the Users. **Submittal required.

• As stated in (Division 08) above, prepare and paint over the existing door frame and re-varnish the existing solid wooden door.

• Re-caulk around corners of the existing wall where the glazed 4″ x4″ ceramic tiles meet, apply the same procedure around areas between the 2″x 2″ square floor tiles and the glazed wall tiles in the shower areas. ** Submittal required.

9.3. Men Restroom

• Demo and remove the existing 2″x 2″ square ceramic floor tiles in the restroom area.

• In the same area, clean out area, provide and install new 12′ x 12′ ceramic floor tile as indicated in drawing. **Submittal required.

• Re-tile area of the shower room with missing 4″ x4″ glazed ceramic wall tile. Ensure new

4″x4″ glazed ceramic wall tile match the existing. **Submittal required.

• As mention above in section 9.4, re-caulk around corners of the existing wall where the glazed 4″ x4″ ceramic tiles meet, apply the same procedure around areas between the 2″x 2″ square floor tiles and the glazed wall tiles in the shower areas. ** Submittal required.

• Demo the existing ceiling drywall in restroom and replace with new 2′ x 2′x ¾″ kitchen / bathroom style suspended ceiling tiles. **Submittal required.

• Clean, prep and paint the existing restroom walls as indicated in drawing. ** Submittal Required.

10. DIVISION 10 – SPECIALTIES

10.1. Men/Women Restroom

• Detach and remove the existing wall hang urinal in the Men’s Restroom, replace fixture with a new wall hung urinal. **Submittal required

• In both restroom, provide and install new floor and ceiling anchored toilet cubicles as shown in drawing. **Submittal required

• Demo the existing female/ male door sign and install a new female/male flexible door sign on the restroom main entrance door. Sign should be a 8″x8″, 5/8″ letters, adhesive back with Braille. ** Submittal required

• Demo the existing surface mount paper towel/ tissues receptacle in both restroom. Then provide and install new paper towel/tissues dispensers & waste receptacles in both restroom.** Submittal required

• Remove and reset the existing wall mount 72″ x 24″ mirrors mounted on mens and womens bathroom walls. Ensure to clean the existing mirrors in both restrooms before resetting.

• Provide and install a new Urinal screen partition in the men’s restroom as indicated in drawing. **Submittal required.

• Provide and install new toilet accessories as listed:

Robe hook. ** Submittal required Stainless steel surface mount single roll toilet dispenser. **Submittal required Sink faucets head and levers. ** Submittal required.

Female sanitary dispenser. **Submittal required.

P-traps. **Submittal required.

15.1. Corridor/Bedroom Areas

• Demo and remove the existing surface mount water fountain in the Corridor areas that leads to both restrooms see drawing for location. Provide and install new surface mount water fountain. In the corridor areas. **Submittal required

• Remove and reset the existing thermostat covers along the Corridor and Bedroom areas as indicated in drawing.

• Contractor shall disconnect and remove the existing diffusers and grilles along the Corridor and Bedroom areas as indicated in drawing. Provide and install new 2′x2′ diffusers and grille in the Corridor and bedroom areas, ensure the fixtures are properly secured to the flex ducts.

**Submittal required.

15.2. Men/Women Restroom

• Demo and remove the existing 6″X6″air return aluminum grilles in the restroom area as indicated in drawing. After demolition, provide and install new 6″X6″air return aluminum grilles. **Submittal required

• Replace the existing diffusers in the second floor women/men restroom with new 24″x24″ diffuser.

• Demo and remove the existing floor mount water closets in the lower level men and women restroom, ensure to cap opening to avoid sewer gas escaping. At the same opening, provide and install new water closets in restroom as indicates in drawing. **Submittal required

• Do not disturb the existing shower heads and faucets in the shower area.

• Demo the existing counter top lavatories with all the associated accessories, cap the existing water lines and plug the P-trap pipe. At the same location , provide and install new counter top lavatories as indicates in drawing. **Submittal required

• Demo the existing urinals in the lower level men’s restrooms and replace the existing with new wall mount urinals. Ensure the new urinals properly fit the existing rough-in in wall.

16. DIVISION 16 – ELECTRICAL

16.1. Corridor Area

• The contractor shall remove all switch, receptacle, fire alarm, coaxial cable, thermostat and electrical panel box covers on all walls in the identified areas as necessary to perform repainting of existing corridor walls. All removed items shall be reinstalled upon completion of work.

• The contractor shall remove all emergency lighting in the corridor areas. The contractor shall provide and install new emergency lighting. Demolition shall not include electrical whips.

**Submittal required.

• Contractor shall remove all emergency exit signs in the corridor areas and shall provide and install new LED emergency exit signs. **Submittal required.

• At the existing water fountain areas , Contracto r shall remove the existing wall receptacles, and replace with new GFCI receptacle that is used to power the existing water fountains.

**Submittal required.

• Remove and reset the existing surveillance cameras, loudspeakers & fire detectors in ceiling during ceiling removal and installation.

• Disconnect or remove the existing 1′x4′florescent lightings in Corridor and Bedroom areas as indicated in drawings. At the same locations provide and install new 2′x4′ T8, florescent lights. **Submittal required.

16.2. Bedroom Areas

• Disconnect or remove the existing 2′x4′florescent lightings in Bedroom areas as indicated in drawings. At the same locations provide and install new 2′x4′, T8, florescent lights.

**Submittal required.

• The contractor shall remove all switch, receptacle, fire alarm, coaxial cable, and thermostat covers from walls in the identified areas as necessary to perform repainting of existing corridor walls. All removed items shall be reinstalled upon completion of work.

16.3. Men/Women Restroom

• Disconnect or remove the existing 2′x4′florescent lightings in restroom areas as indicated in drawings. At the same locations provide and install new 2′x4′ florescent lights.

**Submittal required.

• Contractor shall provide and install new 2′ x2′ florescent light with the associated whip in the women restroom area as indicated in drawing. **Submittal required.

• Remove and reset the existing GFCI receptacle above the restroom area.

• In each restroom areas, contractor shall disconnect or remove the existing 1′x4′florescent lightings above the wall install mirrors. See drawing for location. At the same locations provide and install new 1′x4′ florescent lights. **Submittal required.
