Section L - Instructions, Conditions, & Notices to Offerors or Quoters

ADDENDUM TO 52.212-1 PROPOSAL PREPARATION INSTRUCTIONS
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
The response shall consist of two (2) separate parts, Part I - Price Proposal - Part
II - Past Performance Information.
B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required.
However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
C. Specific Instructions:
1. PART I - PRICE PROPOSAL - Submit original.
(a) Complete blocks 14-20C of the SF 1442. In doing so the offeror accedes to the contract terms and conditions as written in the solicitation, with attachments. The solicitation constitutes the model contract.
(b) Insert proposed unit prices in Schedule B for each Contract Line Item Number (CLIN).
2. PART II - PAST PERFORMANCE INFORMATION - Only references for relevant past efforts/ contracts are desired.
(a) Past Performance Surveys: The Government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance. Provide a list of no more than five (5), of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. Relevant contracts include services of the same scope, complexity, and magnitude of those required under this solicitation. Relevancy definitions are as follows:
VERY RELEVANT: Present/Past Performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.
RELEVANT: Present/Past Performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.
SOMEWHAT RELEVANT: Present/Past Performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.
NOT RELEVANT: Present/Past Performance effort did not involve any of the magnitude of effort and complexities this solicitation requires.
In accordance with FAR 15.305(2)(iii) the evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed.
(i) Company/Division name

(v) Contract Dollar Value
(vi) Period of Performance
(vii) Verified, up-to-date name, address, FAX & telephone number of the contracting officer

(ix) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.
(x) Comment on how your reference(s) meet the above relevancy definition.
If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past relevant contracts. Note: Each teaming partner will be considered a prime contractor and past performance will be considered jointly.
(c) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor's consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
D. Documents submitted in response to this solicitation must be fully responsive to and consistent with the following:
1. Requirements of the solicitation (CLIN & SOW), and Government standards and regulations pertaining to the PWS.
2. Evaluation Factors for Award in accordance the addendum 52-212-1 "Proposal Preparation Instructions"
3. Representations and certifications or self-certification in System for Award Management (SAM)
4. Contract references and Past Performance questionnaires
5. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in 52.212-1 "Proposal
Preparation Instructions" will not be read or evaluated, and will be removed from the proposal
Past performance sources or information can be submitted to 1 Lt Daniel Henry, Contract Specialist, at daniel.henry.14@us.af.mil or with proposal.
INFORMATION REGARDING SUBMISSION OF PROPOSAL: Proposal shall be submitted electronically to Daniel.henry.14@us.af.mil and Bryan.armstrong.3@us.af.mil or through the DoD Safe web application at https://safe.apps.mil/. HARD COPY PROPOSALS WILL NOT BE
CONSIDERED NOR EVALUATED.
(a) Offerors that elect to submit proposals via email shall include a read receipt to confirm successful delivery of proposal.
(b) Offerors whose proposals are too large to be sent via email are encouraged to submit proposals via DoD Safe, https://safe.apps.mil/.
(c) Late proposals will be processed in accordance with FAR 52.212-1(f) "Late Submission, Modifications, Revisions, and Withdrawals of offers."
SITE VISIT NOTICE: A pre-proposal site visit for this solicitation will be held at Travis AFB, CA. Date, time, location, and specific information are as follows:

Location: Visitors' Center (Bldg 605)
Located outside the Main Gate Travis AFB, CA 94535-2632
This visit is open to all interested offerors but will be limited to two personnel per company. All interested offerors will meet and park in the parking lot of the Visitors Center (Building 605), located outside the Main Gate, prior to 10:00 AM where transportation will be provided for site visit. All interest parties shall have a photo ID. All personnel attending the site visit shall abide by all security regulations and requirements of the installation.
All interested personnel shall contact 1 Lt Daniel Henry at (707) 424-7754 or email at daniel.henry.14@us.af.mil no later than
17 December 2019 to provide the names of the personnel that intend to make the site visit.

JOINT VENTURE AGREEMENTS. Joint Ventures are allowable on competitive 8(a) set-asides, however, the joint venture agreement must be received by SBA prior to proposal due date and approved before award of any resulting contract. If you are contemplating a joint venture on this project, you must advise your assigned SBA Business Opportunity Specialist (BOS) as soon as possible. It is also recommended that the agreement be submitted as soon as practicable to ensure compliance with established regulations. Any corrections and/or changes needed can be made only when your BOS has adequate time for a thorough review before the proposal due date.
NO CORRECTIONS AND/OR CHANGES ARE ALLOWED AFTER TIME OF SUBMISSION OF
PROPOSAL OR BIDS.
OFF-RAMP/ON-RAMP
1. It is in the Government's best interest that there remain an adequate number of Contractors eligible to compete for task orders. This Indefinite Delivery, Indefinite Quantity Contract (IDIQ) may not have less than three contractors awarded under
it. The Government does not intend to exceed five contractors assigned under this IDIQ, but may if determined to be in the best interest of the Government.
2. The Government reserves the unilateral right to Off-Ramp non-performing Contractors. Typically, Off-Ramping will occur during annual option exercise timeframes. Off-ramping methods may result from one of the following conditions:
a. Permitting the Contractor's Handyman IDIQ contract term expire instead of exercising the option.
b. Debartment, Suspension, or Ineligibility as defined in FAR Subpart 9.4
c. Termination as defined in FAR Part 49.
d. Contractors who fail to meet the standards of performance, deliverables, or compliance with specifications (i.e. receive less than Satisfactory on CPARS assessments, exceeding delivery dates without excusable delay, constructed work does not meet code).
e. Contractors that fail to propose on five consecutive TOPRs may be off-ramped for non-participation.
3. If a contractor is Off-Ramped, all existing task orders given to the contractor shall continue in accordance with the terms of the task order. The Off-Ramped contractor will be ineligible to receive new task orders.
4. If the Handyman IDIQ has less than three contractors actively assigned, the next TOPR will be sent to local small businesses in an effort to increase the number of contractors assigned to the contract.
The following is the process to On-
Ramp contractors:
a. The Basic IDIQ and TOPR information will be provided to all interested small businesses in the local area and posted through the Government Wide Point of Entry
b. The evaluation criteria utilized will not represent a material difference from the original evaluation criteria at the time of initial award. Contractors vying for incorporation into the Basic IDIQ shall provide past performance information along with their pricing and technical narrative for the TOPR.
c. If a contractor already assigned to the Handyman IDIQ provides the lowest-priced, technically acceptable proposal, the task order will be awarded to that contractor.
d. If a contractor not assigned to the Handyman IDIQ provides the lowest-priced, technically acceptable proposal, that contractor will be incorporated into the Handyman IDIQ and shall receive the task order if also is considered to have acceptable past performance.
e. The period of performance term for any new awards shall coincide with the existing term for all other Contractors.
f. Vendors On-Ramped through the above process will be eligible to compete on all TOPRs issued under the IDIQ.
