PERTINENT INFORMATION FOR OFFERORS

SOLICITATION FA4407-16-T-0003

FIRE ALARM DETECTION AND FIRE EXTINGUISHER MAINTENANCE

10 AUGUST 2016

This document highlights pertinent information for offerors when preparing their offers. The pertinent information is listed below:

• Pages 31-34 of the solicitation contain the Addendum to FAR 52.212-1, Instructions to Offerors—Commercial Items.

• Pages 35-36 of the solicitation contain the Addendum to FAR 52.212-2, Evaluation –

Commercial Items.

• Reference addendum to FAR 52.212-1, paragraph F. A site visit will be held on Tuesday, 30 August 2016 at 9:00 a.m. CST at Scott AFB IL. Contractors must contact either the contract specialist, Ms. Penny Boggis at (618) 256-9253, or email penny.boggis.1@us.af.mil, or the contracting officer, Ms. Marcia Wuebbels at (618) 256-9308, or email marcia.wuebbels@us.af.mil by 2:00 p.m CST Monday, 29 August 2016 to confirm attendance and allow the Government sufficient time to assure your Base Pass is processed. The site visit begins promptly at 9:00 a.m.

CST. Please allow ample time to obtain your pass from the Visitor Control Center to enable access to the installation. All interested parties will meet in the 375th Contracting Squadron Conference Room located in Building 50.

• Please review specific instructions on quote preparation/requirements contained in addendum to FAR 52.212-1, paragraph G.

• Reference addendum to FAR 52.212-1, paragraph G.2.(a). Offerors are required to submit past and present performance questionnaires on at least three, but no more than five similar services of Fire Alarm Detection and Fire Extinguisher Maintenance, which your company has provided within the last three years. Offerors must forward the past and present performance questionnaire to the company for who your company provided the service. In turn, the recipient/respondent is to email the completed questionnaire DIRECTLY to the contract specialist, Ms. Penny Boggis at penny.boggis.1@us.af.mil, or the contracting officer, Ms. Marcia Wuebbels at email marcia.wuebbels@us.af.mil. If an email response is not possible, please contact Ms. Boggis at

(618) 256-9253 or Ms. Wuebbels at (618) 256-9308 to coordinate fax or mail submissions to the addresses provided in Addendum to FAR 52.212-1, paragraph H.

• Reference addendum to FAR 52.212-1, paragraph H. Offers are due no later than 2:00p.m.

CST 15 September 2016. Email is the preferred method of submission. However, if an email submission is not possible, please contact Ms. Boggis at (618) 256-9253 or Ms. Wuebbels at

(618) 256-9308 to coordinate fax or mail submissions to the fax/mail addresses provided in Addendum to FAR 52.212-1, paragraph H.

• Offerors MUST submit a current and valid National Institute for Certification in Engineering Technologies (NICET) Certification Fire Alarm Level 2 for all contractor employee(s) and, as applicable, subcontractor(s) employee(s) who will perform service.

• All offerors interested in submitting an offer for this acquisition MUST be:

o Registered in the System for Award Management (SAM) in order to be considered for this award; and o A small business or any socioeconomic category vendor

• Reference FAR 52.212-3, Offeror Representation and Certifications—Commercial Items (Apr 2016), plus Alternate I (Oct 2014), for completion of required representations and certifications unless they are updated electronically.

• Complete the following provisions and submit with your offer:

252.209-7998, Representations Regarding Convictions of a Felony Criminal Violation Under any Federal or State Law (Deviation 2012-00007) (MAR 2012)

252.209-7999, Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or

A Felony Conviction Under any Federal Law (Deviation 2012-00004) (JAN 2012)

REAL ID Act Requirements

HQ Air Force has directed that state issued identification cards or driver's licenses from Missouri, Washington, Minnesota, or America Samoa will no longer be accepted for unescorted access to Air Force installations, including Scott AFB (many personnel who have business/work on Scott come from Missouri).

Visitors to Scott AFB who do not possess a DoD identification card must have a Passport or another federally recognized photo identification for unescorted access to the base. If visitors do not have a Passport another form of acceptable ID, then they must be escorted by a sponsor while on Scott AFB (see list below).

Passed by Congress in 2005, the REAL ID Act enacted the 9/11 Commission's recommendation that the Federal Government "set standards for the issuance of sources of identification, such as driver's licenses."

The Act established minimum security standards for state-issued driver's licenses and identification cards and prohibits Federal agencies from accepting for official purposes licenses and identification cards from states that do not meet these standards. To date, it was determined that the above states and territories have not made adequate progress in meeting these security standards and therefore, these identification cards may not be used for unescorted base access.

What does this mean? For visitors coming to Scott AFB, we are no longer authorized to approve visitor passes through the Scott AFB Visitor Control Center or through other channels for unescorted access. If they do not have one of the listed alternate forms of approved federal identifications (for example, a passport), they must be physically escorted onto Scott by a member with a DoD identification card.

Visitors driving (or as a passenger) may enter Scott AFB if they are escorted by another passenger who possesses a DoD identification card. All deliveries and transportation entities must comply with the Act.

The following is a list of acceptable federally recognized photo IDs that can be used in lieu of a driver's license from Missouri or the other states/territory as noted.

Acceptable Forms of Federally Recognized Photographic Identification

• Driver’s licenses or other state photo identity cards issued by Department of Motor Vehicles (or equivalent) in accordance with REAL ID enforcement schedule (https://www.dhs.gov/current-status-states-territories)

• U.S. Passport

• U.S. Passport Card

• U.S. Permanent Resident Card (Form I-551)

• Employment Authorization Document issued by DHS (Form I-766)

• An ID card issued by Federal, State, or local government agencies with all the required REAL

ID Act information

• U.S. Coast Guard Merchant Mariner Cards/Credentials

• Personal Identification Verification (PIV) card issued by the Federal Government

• Interoperable (PIV-I) cards issued by Non-Federal Government entities

• DHS “Trusted Traveler” Cards (Global Entry, NEXUS, SENTRI, FAST)

• Merchant Mariner card issued by DHS

• Border Crossing Card (Form DSP-150)

• U.S. Certificate of Naturalization or Certificate of Citizenship (Form N-550)

• U.S. Refugee Travel Document issued by DHS containing a photograph (Permit to Re-enter

Form I-327 and Refugee Travel Document Form I-571)

• Veterans Health Identification Card issued by the U. S. Department of Veterans Affairs

• Foreign Government Passport
