Past Performance Individual Evaluation Worksheet

AFPAA Program Optimization

OFFEROR:

The Government will accomplish Past Performance evaluation by reviewing past performance information provided in the Offeror' s Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, Past Performance Information Retrieval System (PPlRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Contract Tracking and Reporting System (CTARS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers or Contracting Officers or their appointed assistants referenced by the Offeror in any of the past performance information disclosed. Past Performance information is evaluated for the purpose of assigning a confidence assessment in order to make an award that is most advantageous to the Government.

1. The Past Performance evaluation results in an integrated assessment of the Offerors probability of meeting the RFP requirements and the assignment of an overall rating based on consideration of the Offeror's recent, relevant and quality past performance based on the Offeror's CPARS reports and other information.

2. Per the FAR 52.212-2 included in the RFP only offerors with an assessed confidence rating of Satisfactory Confidence or better will be considered for award

3. An integrated assessment of past performance and price factors will be accomplished to determine the best value to the Government.

The contractor will be assigned one of four assessments based on the Past Performance relevancy Table found in the Evaluation Criteria:

Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.

Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.

Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.

Substantial Confidence

Based on the offeror's recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.

Satisfactory Confidence

Government has a reasonable expectation that the offeror will

Neutral Confidence

No recent/relevant performance record is available or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.

Limited Confidence

Government has a low expectation that the offeror will

No Confidence

Government has no expectation that the offeror will be able to
