QUARTERLY/ANNUAL SITE COMPLIANCE INSPECTION REPORT FOR

Description of Facility:
Instructions: Answer each question on the report with yes or no. Note any pertinent details observed during the inspection and as equipment condition, safety hazards, or changes in use of the facility in the comment column. If an inspection item is found to not be in compliance with the SWP3 or if a potential source of storm water pollution is observed, complete a Corrective
Action Report detailing the problem and any corrective action measures taken. Attach it to this report. Any new potential storm water pollutant sources identified and plans for appropriate controls for them must be incorporated into the Storm Water Pollution Prevention Plan (SWP3) within 2 weeks of the inspection. Sign the report certification and submit the site compliance report to 82 CES/DS2/CEV. Keep one copy of the report in the facility files.

Do any pieces of machinery, equipment, vehicles, or aircraft show signs of uncontrolled fuel or fluid leaks?
Do any material storage containers, valves, or pipes show signs of leaks or spills?
Are material storage containers kept in their proper locations?
Are material storage containers properly labeled?
Are appropriate spill control materials readily accessible in the event of a spill?
Is some form of secondary containment provided for all material storage containers?
Are there excess supplies of unnecessary or infrequently used materials?
Are there additional materials present that are not identified in the SWP3?
Are there any waste fluid containers present that are full and in need of emptying?

Are work areas kept in a neat and orderly condition?
Do work areas need cleaning to remove excess dirt, debris, or chemical residues?
Do storm water control measures (BMPs) present at facility need maintenance (i.e., removal of spilled materials, precipitation)?
Does standing water (if present) on outdoor areas or in drainage structures exhibit signs indicating the presence of possible storm water contaminants such as fuel, oil, grease, algae blooms, etc.?
Is there evidence of non-storm-water discharges from the facility not allowed under the storm water permit?
Do personnel working at the facility have knowledge of problems, situations, or practices that may contribute to storm water pollution?
Is information regarding spill response and pollution prevention procedures posted?
Are MSDS sheets posted and current?
Have personnel received required pollution prevention training?
Are personnel trained in spill response procedures appropriate for the facility?
Certification: Based on my findings during this inspection, I recommend that this facility be certified as being in full compliance with the TPDES Multi-Sector General Permit for Storm
Water Discharges Associated with Industrial Activity and its Storm Water Pollution Prevention
Plan.
