FROM: 502d Contracting Squadron 1655 Selfridge Ave JBSA Lackland, TX 78236
SUBJECT: Request for Past Performance Questionnaire Completion
1. The 502d Contracting Squadron of the Air Force Air Education and Training Command is in the process of selecting a contractor for the JBSA Lackland full food service requirement to provide food service for 13 dining facilities, one flight kitchen, one Meals Ready to Eat (MRE) storage warehouse, one expendable supply storage warehouse, and one field facility for basic military trainees (BMTs) at JBSA Lackland, the Lackland Training Annex, and Camp Bullis. Contractor tasks include dining facility management; accounting; cooking; food requisition and preparation; serving and replenishing food; cleaning facilities; maintaining information technology hardware; preparing vegetables and fruits for the salad bar; cleaning/sanitizing tables in dining areas; performing cashier services; ordering and handling foods, supplies, and property; maintaining quality control; ensuring operator maintenance and repair of food service property; and in the event of contingency, performing all required tasks to include continued service.
2. One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.
3. Our areas of interest in the offeror are summarized in the enclosed questionnaire. Our schedule is extremely tight and we need your written response no later than 24 Feb 17. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
4. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and "faxed" to 210-671-1199 (Attention: Lauren Macias) or e-mailed to lauren.macias.1@us.af.mil. Please call Ms. Lauren Macias at 210-671-6415 prior to fax transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.
5. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
