SSP Attachment 11 Section L - Instructions, Conditions, and Notices to Offerors
L. PROPOSAL PREPARATION INSTRUCTIONS
L.1. GENERAL INFORMATION
L.1.1.To assure timely and equitable evaluation of proposals, Offeror(s) must follow the instructions contained herein. Offeror(s) are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. The response shall consist of three (3) separate volumes; Volume I - Technical Proposal, Volume II – Past Performance Information, and Volume III - Price Proposal.
L.1.2. IAW FAR Subpart 4.803(a)(10), the Government will retain one copy of all proposals. All additional copies of proposals will be destroyed by the Government after contract award.
L.1.3. Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact date and time specified in this solicitation. Offers shall be submitted on the Standard Form 33.
L.1.4.
The Procurement Contracting Officer (PCO) and Contract Specialist (CS) are the sole points of contact for this acquisition. Offerors are responsible for ensuring receipt of any communication by the designated officials. Verification of receipt of communications can be done via email or telephone. Questions and written requests for clarification shall be done via email only. Points of contact (POCs) are:

L.1.5. Minimum Proposal Acceptance Period: The term “acceptance period,” as used in this provision, means the number of calendar days available to the Government to award a contract from the date specified in this solicitation for receipt of proposals. The Government requires a minimum acceptance period of 240 calendar days from the date of receipt of offers; however, Offerors may specify an acceptance period exceeding the Government’s minimum requirement.
L.1.6. Exceptions to Terms and Conditions: Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub-factors. Offerors must clearly identify any exceptions to the solicitation terms and conditions, to include complete rationale supporting the exception that fully explains potential impacts on the performance, schedule, cost, and specific requirements of the solicitation.

L.1.7. Potential Errors: If an Offeror believes requirements in these instructions contain an error, omission, or are otherwise unsound, the Offeror shall immediately notify the Contracting Officer in writing with supporting rationale. NOTE: The Offeror is reminded that the Government reserves the

L.1.8. Other Information Required. Provide the name, title and telephone number of the corporate POC(s) possessing authority to contractually negotiate/obligate the Offeror.
L.1.9. Non-Government Advisors will not be utilized during source selection.
L.1.10. Proposals must be submitted in both electronic and paper format as specified in L.2.
ORGANIZATION/DISTRIBUTION/NUMBER OF COPIES/PAGE LIMITS. In case of conflict between paper and electronic proposal copies, the paper version will take precedence. Telegraphic, e-mail or facsimile offers will not be accepted. The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the Solicitation Number, and the Offeror’s POC name, address and phone number.
L.1.11. Offerors are cautioned that JBSA Randolph has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass to gain entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HAND CARRYING PROPOSALS. Offerors are responsible for allowing sufficient time to obtain a visitor pass and arrive at the bid depository PRIOR to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.215-1(c)(3) “Submission, modification, revision, and withdrawal of proposals.”
L.2. ORGANIZATION/DISTRIBUTION/NUMBER OF COPIES/PAGE LIMITS

* Pages exceeding the page limitations will not be read or evaluated and will be destroyed as soon as practicable. The page count does not include the transmittal letter, cover pages, tab-divider pages, title pages, table of contents, lists of tables and drawings, glossary of acronyms, cross reference matrix, position qualifications or manpower matrix. Double-sided printed sheets will be counted as 2 pages.
One proposal shall be marked "Original" and three marked "Copy". Any narrative documentation shall be submitted in Microsoft Word (version Office 2007 or higher). The “Original” shall have two holes punched at top of each page. The contents, instructions and formats of each proposal volume are described in the Instruction to Offeror(s) L.3. sub-paragraphs.
L.2.2. Mail four hard copies and two electronic copies on CD or DVD of each volume to the following:

L.2.3. Binding and Labeling: Volumes I, II, and III may not be submitted together within the same binding. A cover sheet should be placed at the beginning of each volume start, clearly marked as to whether it is the original or copy, volume number, title, solicitation identification, and the Offeror's name.

Include on each proposal cover letter or proposal cover page: name, office phone and fax number (and, if applicable, cell phone number), and e-mail address for each proposal point of contact. Be sure to apply all appropriate markings including those prescribed in accordance with FAR 52.215-1(e), "Restriction on Disclosure and Use of Data.”
L.2.4. Page Format Restrictions and Limitations:
L.2.4.1. Page Format: Pages shall be 8.5 x 11 inches, not including foldouts used for charts, tables, or diagrams. Pages shall be typed. Except for the reproduced sections of the solicitation document, typing shall be single space, 12 font size, and Times New Roman font style. Smaller font size may be used in tables, drawings, charts, graphs, and diagrams, but shall not be less than 10 font size, Times New Roman.
Margins on all four edges of each sheet will be at least one inch. Proprietary statements, security markings, and page numbers can fall within the defined margin area. Pages shall be numbered sequentially by volume. These page format restrictions shall apply to responses to Evaluation Notices (ENs) as well as to the initial proposal.
L.2.4.2. Page Limitations: Compliance with page count limits shall be verified via the hardcopy. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and will be destroyed as soon as practicable. When both sides of a sheet display printed material, it shall be counted as 2 pages. Page limitations shall be placed on responses to ENs. The specified page limits for EN responses will be identified in the letters when forwarding the ENs to the Offeror(s). Any supporting documentation that is in response to an Evaluation Notice, not to include change pages, will not be subject to page limitations. The page limit for the required Quality Control Plan in Subfactor 1, L.3.1.1., is unlimited and will not count against the limit expressed in paragraph L.2.1. Table for Volume I, Technical Proposal.
L.2.4.3. Pages Counted: Each page shall be counted except the following: transmittal letter, cover pages, tab-divider pages, title pages, table of contents, lists of tables and drawings, glossary of acronyms, cross reference matrix, position qualifications or manpower matrix.
L.2.4.4. Indexing: Each volume shall contain a more detailed table of contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections.
L.2.4.5. Foldouts: Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible and shall not exceed 11 x 17 inches in size. Foldout pages shall fold entirely within the volume. Each printed side of a foldout will count as two pages. Foldout pages may only be used for large tables, charts, graphs, diagrams and schematics; not for pages of text. For tables, charts, graphs and figures, the font shall be no smaller than 10 font, Times New Roman. Design drawings may be in the Offeror's format, provided standard commercial policies and practices are followed. All information (except documentation numbers, classification markings, and page numbers) shall be contained within an image area of 11 x 17 inches.
L.2.5. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:
L.2.5.1. Requirements of the RFP (including Contract Line Item Numbers (CLIN) and Performance Work Statement (PWS)) and Government standards and regulations pertaining to the PWS.
L.2.5.2. Evaluation Factors for Award in Section M of this RFP.
L.2.5.3. Elaborate formats, bindings or color presentations are not desired or required.

L.3. SPECIFIC INSTRUCTIONS
Volume Organization The Technical volume shall be organized according to the following general outline:

f) Subfactor 1 - Quality Control (QC)

L.3.1. Volume I – Technical Proposal (Factor 1): Volume I shall be prepared in accordance with these instructions and Paragraph L.2. of this Section L. Proposals will be evaluated in accordance with the Section M evaluation criteria. The Government will assess risk associated with the proposed approach as a component of technical acceptability. Risks assessed to have potential to disrupt schedule, increase costs, degrade performance, increase need for Government oversight, or increase likelihood of unsuccessful contract performance will be identified as “unacceptable risk.” An “unacceptable” rating in one or more technical subfactors will constitute an overall unacceptable rating for the technical evaluation. Each Offeror shall submit information for the following sub-factors:
L.3.1.1. Subfactor 1 – Quality Control (QC): This subfactor assesses the Offeror’s draft plan to evaluate the quality of operation and maintenance; identify, document and immediately correct short-term deficiencies and trends, resulting in corrective action and continual process improvement in terms of quality, workmanship, and customer support. The Offeror’s draft plan must demonstrate that QC is the contractor’s (not the Government’s) responsibility. The Offeror shall:
L.3.1.1.1. Present a QC approach for your organization, including lines of authority, and provide a proposal that will successfully evaluate compliance, find and document problems, and monitor corrective actions ensuring successful and timely problem resolution. Explain how recurring problems will be prevented.
L.3.1.1.2. Describe a system for recording, computing/assessing performance measurement data, and monitoring corrective actions to ensure successful and timely problem resolution.
L.3.1.1.3. Describe the documents and records your company will develop, maintain, and utilize as part of the Quality Control Plan.
L.3.1.1.4. Describe the method for gathering information and evaluating performance based on the requirements of the PWS, and reporting the results to the Government Contracting Officer Representative (COR) (PWS para 1.2.2.2.1).
L.3.1.1.5. Present examples of how to successfully develop and implement quality control and/or inspection procedures to meet contract requirements.
L.3.1.2. Subfactor 2 – Workforce and Organizational Structure: This subfactor will provide a graphical representation of the operation, maintenance and logistics/administrative organizational structure for the base period and each option period of this contract for each place of performance (Luke AFB, AZ and Holloman AFB, NM). The Offeror shall:
L.3.1.2.1. Present a graphical organizational structure and the rationale for proposed roles, responsibilities, and relationships for each organizational element, work center, or shop.
L.3.1.2.2. Provide the number of full-time and part-time equivalents utilized and the rationale with calculations for the number of full-time and part-time equivalents based on workload projections provided in the PWS.
L.3.1.2.2.1. Provide a detailed plan to provide Site Managers and Assistant Site Managers at each location with required experience IAW PWS para 1.2.1.1.
L.3.1.2.2.2. Provide a complete staffing/skill (job classification)/manpower matrix that is consistent with proposed methodology and provides Full or Part Time Equivalents sufficient to accomplish the projected workload provided in the PWS detailed to the “shop/function” level.
L.3.1.2.2.3. Provide the number of Full Time Equivalents (FTE)/part-time, temporary staff-year equivalent hires. Provide the rationale and calculations used to arrive at the proposed FTE level. If the proposal includes fewer FTEs than typically required given the workload projections and PWS requirements, explain how you will achieve the efficiency and still accomplish the workload provided in the PWS (PWS Appendix 6).
L.3.1.2.2.4. Identify “one-deep” positions on the organizational chart and explain the plan for backing up these positions.
L.3.1.2.2.5. Demonstrate effective cross-utilization of personnel within each functional area throughout work centers. The proposal must detail percentage of workforce that will be cross-utilized, while maintaining required skill mix and staffing levels.
L.3.1.3. Subfactor 3—Understanding the Requirement: This subfactor will describe an overall approach and methodology for accomplishing work requirements identified in the PWS with the appropriate skills, equipment and materials. The Offeror shall:
L.3.1.3.1. Demonstrate their ability to have trained personnel in sufficient number at the right place and at the right time to meet the PWS requirements.
L.3.1.3.2. Provide a detailed approach to include sound innovations and efficiencies that enhance contract performance and achieve reduction in both contract and Government Operation and Maintenance costs.
L.3.1.3.2.1. Describe an approach for scheduling work so that sufficient skills are available to meet the ACTS O&M and support requirements provided in the PWS.
L.3.1.3.2.2. Describe procedures/plans for operations/maintenance scheduling, tracking and completing special inspections, scheduled and unscheduled maintenance while continually meeting equipment availability standards.
L.3.1.3.2.3. Describe an approach for ensuring maintenance data input and forms required in the PWS are accomplished and that the status indicated on forms and in data systems is accurate.
L.3.1.4. Subfactor 4—Service Provider Training Program: This subfactor will describe an approach for developing a robust internal formal training program for operation, maintenance, and logistic personnel.
The proposal must adequately address both remedial and on-going training.
L.3.1.4.1. Describe a training plan and address how you will train, qualify, certify and maintain currency for employees prior to assuming a position and throughout the life of the contract.
L.3.1.5. Subfactor 5—Transition Plan: This subfactor will describe a transition approach and ability to adequately achieve full performance on the first day of contract operations. The approach shall provide:
L.3.1.5.1. A detailed description that addresses recruiting, training, certifying, and qualifying personnel during the (1)-month mobilization period that results in sufficient numbers of fully qualified employees by contract start date. A graphical time-line schedule that addresses start and completion of each event (e.g., interviewing, hiring, human resource actions, training, badging and start work dates). The time-line shall address inventory for transfer of Government equipment and facilities, assumption of work, materials, and work procedures.
L.3.1.5.2. A plan that describes acquiring GSA leased vehicles in sufficient number and type that you plan to provide for use in performance of this contract and the timeline for acquiring them.
L.3.2. Volume II - Past Performance Information (Factor 2): Volume II shall be prepared in accordance with these instructions and Paragraph L.2. of this Section L. Each Offeror’s proposal will be evaluated in accordance with the evaluation criteria in Section M. Volume II shall contain the following:
L.3.2.1. Volume Organization. The Past Performance Volume shall be organized according to the following general outline:

c) Past and Present Performance Information Sheets (PPIS)
d) Specific Content
e) Subcontractor Letters of Consent/Client Authorization Letters
f) Organizational Structure Change History
g) Past Performance Questionnaires (PPQ)
L.3.2.2. Summary Page describing the role of the Offeror and each subcontractor, teaming partner, and/or joint venture partner for whom the Offeror is required to provide Past Performance Information Sheets in accordance with paragraph L.3.2.3.
L.3.2.3. Past and Present Performance Information Sheets (PPIS). Each Offeror shall complete and submit a PPIS in accordance with the format contained in Attachment 16 to the RFP. No more than five
(5) contracts total (including proposed major subcontractors, teaming partners, joint venture partners) can be submitted that Offerors consider relevant consistent with the definitions in Section M in demonstrating your ability to perform the proposed effort. The Offeror shall submit a PPIS identifying active or completed contracts (either Government or commercial). PPIS shall be submitted for each prime, teaming partner/joint venture partner and for a subcontractor performing meaningful work determined to be critical to overall successful performance. Each submitted contract must also meet the recency requirements of Section M, that is, each proposed effort must have been performed during the last three
(3) years of the date of issuance of this solicitation and includes at least three (3) months performance prior to the date of issuance of this solicitation. Relevancy is further defined in Section M.
L.3.2.4.Specific Content. Offerors may include a discussion of efforts accomplished by the Offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to an “Unacceptable” rating, since the problems encountered may have been on a more complex program, or an Offeror may have subsequently demonstrated the ability to overcome the problems encountered. The Offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.
L.3.2.5.Subcontractor Letters of Consent and Client Authorization Letters.
L.3.2.5.1. Subcontractor Letters of Consent (Attachment 15 to the RFP). Past performance information pertaining to a subcontractor cannot be disclosed to the prime Offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor. Consent Letters are executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing the release of past performance information so the Offeror can respond to such information.
L.3.2.5.2. Client Authorization Letters (Attachment 14 to the RFP). If applicable, Offerors must also include Client Authorization Letters for each identified effort performed for a commercial customer, authorizing release to the Government of requested information on the Offeror’s performance.
L.3.2.6. Organization Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include with the proposal volume a “roadmap” describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance acceptability assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the Offerors, your “roadmap” should be both, specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
L.3.2.7. Past Performance Questionnaires (PPQs). The Government will evaluate the quality and extent of Offeror's performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the Offeror and other sources such as other Federal Government offices and commercial sources, to assess Acceptable/Unacceptable past performance. Offerors are requested to complete the information in Section A of the PPQ at Attachment 17 to the RFP, in both paper and electronic format. The Offeror will send out a PPQ to each of the Points of Contact (POC) identified in the Past Performance Information Sheets. The POC will be asked to submit completed questionnaires directly to the Government. Once the PPQ is completed by the POC and submitted to the Contracting Officer for this acquisition, the information contained therein shall be considered sensitive and shall not be released to the Offeror.
L.3.3. Volume III – Price Proposal (Factor 3): The Price Proposal shall be prepared according to these instructions and will be evaluated in accordance with the evaluation criteria in Section M of the RFP for the Price Factor. Compliance with these requirements is mandatory and failure to comply may render the proposal unawardable. Data submitted beyond that required by this instruction shall not be considered or evaluated. Volume III shall contain the following:
The Price volume shall be organized according to the following general outline:

c) Glossary/Acronym Abbreviation List
d) Price Data (Section B on SF33)
e) Statement of Compliance with applicable SCA WD(s) and CBA(s)
f) Responsibility Determination Supporting Documentation
g) Teaming Arrangement(s) and/or Letters of Intent
h) Contracting/other
L.3.3.1. Price Reasonableness. The source selection will be conducted with the expectation of adequate price competition for this acquisition per FAR 15.403-1(c)(1); therefore, certified cost or pricing data are not required per FAR 15.403-1(b)(1). If at any time during this competition the Contracting Officer determines that adequate price competition no longer exists or price reasonableness cannot be determined, Offerors may be required to submit certified cost or pricing data or additional data other than certified cost or pricing data, as appropriate, for the Contracting Officer to determine price reasonableness. The proposed prices shall be based on the Offeror’s own technical approach submitted in the Technical Proposal (Volume I) and the Government’s PWS requirements of this RFP. Unbalanced pricing may pose an unacceptable risk to the Government and may be a reason to reject an Offeror’s proposal. Offers should be sufficiently detailed to demonstrate reasonableness and balance.
L.3.3.2 The Offeror must complete the following and include in Volume III, Price Proposal:
L.3.3.2.1 Table of Contents. The table of contents should provide a list of titles for the contents of the price volume organized in the order in which they appear.
L.3.3.2.2 Summary Page. The summary page shall provide an overview of the overall pricing that clearly demonstrates that total price of the proposal of all FFP CLINs.
L.3.3.2.3 Glossary/Acronym Abbreviation List. Provide a glossary for a list of terms or abbreviations with accompanying definitions, if necessary, in order to explain or define difficult or unusual words, terms and expressions.
L.3.3.2.4 Price Data, Section B: The Offeror shall insert proposed unit prices and total “Estimated Amounts” in the bid schedule for each Contract Line Item Number (CLIN), including all option years.
The Government will determine a Not-To-Exceed (NTE) amount for each Cost Reimbursable (CR) CLIN for the base period and each option period. Offerors will not be required to submit a proposal for CR CLINs nor will they be included in the Total Evaluated Price (TEP). The TEP is obtained by multiplying the unit price for each CLIN by the estimated quantity of that CLIN to obtain the total “Estimated Amount” for each CLIN for all periods of the contract; the total “Estimated Amounts” for each CLIN for all periods are then added together to calculate the TEP. The TEP will be the sole basis for evaluating price for contract award decision purposes and will be the only discriminator among Offerors. The proposed prices in Section B must be based on the Offeror’s own technical approach submitted in the Technical Proposal and the Government’s PWS requirements as set forth herein. Additionally, FAR 52.217-8, Option to Extend Services, will be incorporated into the contract, and upon exercise, will extend the terms and conditions of the contract period being extended; the evaluation of all options is included in the initial evaluation of the proposal (includes base year and all option periods).
Data beyond that required by section L.3.3 shall not be submitted, unless it is essential to document or support the Offeror’s price position. All data relating to the proposed price, including all required supporting documentation, must be included in the Price volume (Volume III). Under no circumstances shall this data and documentation be included elsewhere in the proposal.
L.3.3.2.5 Statement of Compliance with applicable Service Contract Act Wage Determination(s) and Collective Bargaining Agreement(s)*. The contractor shall submit the correct Wage Determination(s) (WD) based on location(s) of training, as part of their price proposal. The WD information shall include the specific labor categories utilized in the proposal. WD information can be found at http://www.wdol.gov. Upon determination of the place(s) of performance, the PCO will obtain the WD(s) for the location(s) proposed and ensure that the correct WD(s) were used for the proposal. In the event that the proposal does not include a required WD or CBA, the proposal may be ineligible for award.
The Offeror shall provide a Statement of Compliance with the Service Contract Labor Standards (SCLS). The Offeror shall provide a statement acknowledging:
i. Compliance with the requirements of the SCLS WD(s);
ii. The prices proposed for labor covered by the SCLS WD(s) were formulated in consideration of the rates specified in the document as required for covered contractor personnel; and
iii. The proposed wage and benefit rates covered by the SCLS WD are not escalated beyond the basic period of performance (FAR 52.222-43(a) and (b)).
* Note: A Collective Bargaining Agreement (CBA) is applicable for Luke AFB and Gila Bend AFAF, AZ, and Offerors shall conform with FAR 22.1008-2 Successorship with Incumbent Contractor Collective Bargaining Agreement.
L.3.3.2.6 Responsibility Determination Supporting Documentation. To assist in determining contractor responsibility IAW FAR Part 9.104, the Offeror shall explain how they plan to maintain adequate financial resources for a contract of this magnitude and length. Offerors shall address their fiscal plan for covering mobilization, unexpected or emergency situations, and initial performance costs prior to recoupment in the form of monthly invoice payments. Offerors must provide complete financial statements for one fiscal year and year-to-date financial information through the last quarter available.
The year-end financial statements must be certified by an independent accounting firm, if practicable, or at least by an authorized officer of the organization. If the Offeror intends to rely on internally available working/operation capital, evidence of availability must be submitted. If the Offeror plans to rely on financial support from other sources, evidence of availability and the maximum lines of credit that may be available must be identified and must be based upon the inclusion of this contract effort. Evidence of external funding must be provided in the form of a dated letter from the external source. Documentation to support the amounts must also be included.
L.3.3.2.6.1 IAW FAR 9.104-5, the Offeror is required to comply with the submission regarding FAR 52.209-5, Certification Regarding Responsibility Matters, found in Section K of this RFP (within FAR clause 52.204-8(c)(1)(vii)). Since the Government reserves the right to award without discussions IAW FAR 52.215-1(f)(4), the Government may request that an Offeror submit additional information so a responsibility determination can be made. This request for additional information will not be construed as communications or discussions with an Offeror as defined by FAR 15.306. Failure to certify the required responsibility matters outlined in Section K, may render the Offeror’s proposal ineligible for award.
L.3.3.2.7 Teaming Arrangements and/or Letters Of Intent. If a teaming arrangement is contemplated, provide complete, verifiable information as to the arrangement, including any previous teaming endeavors on previous teaming arrangements with same partner. Submit information relative to the formation, FA3002-18-R-0010, Air Combat Training System http://www.wdol.gov/ organization and operation of the business unit. Information shall include cost and revenue sharing agreements and any other information which describes the business arrangement.
L.3.3.2.8 Contracting/Other.
L.3.3.2.8.1 Standard Form (SF) 33, Blocks 13-18 of the RFP. Completion of blocks 13, 14 (Offeror may sign and date each Standard Form (SF) 30 acknowledging receipt of each amendment to the solicitation), 15, 16, 17 and 18 and signature and date for blocks 17 and 18 of the SF 33. Signature by the Offeror on the SF 33 constitutes an offer, which the Government may accept. The "original" copy should be clearly marked under separate cover.
L.3.3.2.8.2 All necessary clause fill-ins in Sections A through K. Ensure compliance with RFP requirements if using the annual representations and certifications electronically via the System for Award Management website at https://sam.gov.
L.3.3.2.8.3 Submit a letter, signed at the vice-president level or equivalent or higher, that identifies the individual(s) with the authority to obligate or otherwise bind your company to the resulting contract and, if necessary, future contract modifications. Provide the name, title, and telephone number of the company/division point of contact regarding decisions made with respect to your proposal. Also, identify those individuals authorized to negotiate with the Government. In addition, provide the name, title, and telephone number of the CEO, Division President, and/or Vice President.
L.3.3.2.8.4 Provide company/division's name, street address, county and facility code; CAGE code;
DUNS code; TIN; and size of business (large or small). List all locations where work is to be performed and indicate whether such facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location.
L.3.3.2.8.4.1 Organization. The above information shall be provided for the prime and all subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition.
L.3.3.2.8.5 Rounding. All dollar amounts provided shall be rounded to the nearest dollar.
L.3.3.2.8.6 Exceptions to Terms and Conditions. The Offerors shall list any exceptions to, or deviations from, any of the clauses or requirements to the RFP along with the justification, using the table format at L.1.6.
L.3.3.3 Subcontracting Plan (Large Business Only). IAW FAR 19.705-2, large business prime Offerors shall provide a clear, detailed and realistic Small Business Subcontracting Plan that complies with FAR 52.219-9 and DFARS 252.219-7003 and satisfies all of the requirements of FAR 19.704, DFARS 219.704, and AFFARS 5319.704(a)(1) in order to be determined acceptable by the CO prior to contract award. An approved Subcontracting Plan is required for contract award to other than Small Business offeror and will be reviewed as a part of the CO’s responsibility determination. The proposed goals must be supported by clear rationale.

L.3.3.3.2 Provide completed Attachment 13, Small Business Subcontracting Plan Checklist.
L.3.3.4 An incomplete proposal may be considered to indicate lack of understanding of the requirement and may result in the entire proposal being determined “unacceptable” and eliminated from further consideration.
