SECTION SF 30 BLOCK 14 CONTINUATION PAGE

SUMMARY OF CHANGES

The purpose of Amendment 0001 is to make changes to the following:

1. Section J – List of Documents, Exhibits and Other Attachments

a) Attachment 1 – Performance Work Statement (PWS) was revised to read, PWS (Rev 1, dated 1 Dec 15).

b) Attachment 7 – Staffing Matrix was revised to read, Staffing Matrix (Rev 1).

c) Attachment 10 – Past Performance Information was removed and replaced with “RESERVED”.

d) Attachment 16 – Sortie Generation Estimation Model (SGEM) was revised to read, SGEM (Rev 1).

2. Section L – Instructions, Conditions and Notices to Bidders

a) L.2.1, Government Point of Contact – The Alternate POC was changed from Mr. Todd Joyner to Mr. Chuck Wingerter at charles.wingerter@us.af.mil.

b) L.2.9, Distribution – “ATTN” line was changed from “Liz Preston or Todd Joyner” to “Liz Preston or Chuck Wingerter.”

k) L.6.2, Volume IV – Small Business Participation: Removed “and historically black college or universities and minority institutions.” from the paragraph.

3. Attachment 1 – Performance Work Statement (PWS) (Rev 1)

a) Various Changes are shown with “0001” marker throughout the PWS.

b) For the specifics of these changes, see the attached “PWS Summary of Changes.”

4. Attachment 7 – Manpower Matrix (Rev 1)

a) The terms, “1st, 2nd, and 3rd” shifts were redefined as “Dayshift, Swingshift, and Midshift”, respectively, on ALL tabs.

b) The term, “AWD” was spelled out to “Area Wage Determination” and the following statement was added: “Area Wage Determination labor category applies to individuals covered under an Area Wage Determination vice the Collective Bargaining Agreement, see RFP attachment 3” on “Example” tab.

5. Attachment 16 – Sortie Generation Estimation Model (SGEM) (Rev 1)

a) Charts were revised to correct formula errors.

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS

The following have been modified:

L. INFORMATION TO OFFERORS and INSTRUCTIONS FOR PROPOSAL PREPARATION

L.1. GENERAL INSTRUCTIONS

L.1.1 The offeror's proposal must include all data and information requested by this Section L and must be submitted in accordance with these instructions. The offer shall be compliant with the requirements of this Request for Proposal (RFP) (including Contract Line Item Numbers (CLINs) and the Performance Work Statement (PWS)), and Government standards and regulations pertaining to the PWS. Non-conformance with the instructions provided in this Section L and/or failure to meet a requirement may result in an offer being ineligible for award.

L.1.2 The proposal shall be clear, concise, and include sufficient detail for effective evaluation and substantiating validity of stated claims. The proposal must not simply rephrase or restate Government requirements but rather, shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume the Government has no prior knowledge of their facilities or experience and will base its evaluation solely on the information presented in the offeror's proposal.

L.1.2.1 Proposals must demonstrate a comprehensive understanding of the nature and scope of work required. The Government will not search for data to cure problems or address inconsistencies in proposals. Each offeror shall submit a complete proposal in accordance with these instructions. The Government may consider any failure to comply with these instructions to be indicative of what could be expected from an offeror during performance and may consider it a weakness of the proposal. Statements such as "will comply," "noted and understood," "in accordance with industry best practices/standards," etc. without supporting narrative are not acceptable. Cursory responses or responses that merely reiterate or paraphrase PWS language will not be considered to satisfy the requirements of the RFP.

L.1.3 Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.

L.1.4 The Government requires a minimum acceptance period of 280 calendar days from the date of receipt of offers; however, offerors may specify an acceptance period exceeding the Government’s minimum requirement. The offeror’s proposal must affirmatively state an acceptance period equal to, or greater than, 280 calendar days by completing SF 33, Block 12.

L.1.5 In accordance with FAR Subpart 4.8, Government Contract Files, the Government will retain the original copy of all unsuccessful proposals. The Government will destroy extra copies of such unsuccessful proposals.

L.1.6 Submit signed and dated offers to the office specified in L.2.9. prior to the date and time specified in this RFP. Offers shall be submitted on the Standard Form 33 (SF 33).

L.2. GENERAL INFORMATION

L.2.1. Government Point of Contact

The Contracting Officer (CO) is the sole point of contact for this acquisition. Address any questions or concerns to the CO and the Alternate POC listed below. The deadline for all questions, concerns, and/or requests for clarification is 15 working days prior to the proposal due date. The Government will not respond to submissions past the deadline. Contact information is as follows:

L.2.2. Debriefings

The CO will promptly notify offerors of any decision to exclude them from the competitive range if a competitive range is established; whereupon, the offeror may request and receive a debriefing in accordance with FAR 15.505. Offerors excluded from the competitive range may request a pre-award debriefing or a post-award debriefing. NOTE: Offerors excluded from the competitive range are entitled to a single debriefing. The CO will notify unsuccessful offerors in the competitive range of the source selection decision in accordance with FAR 15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing. Offerors desiring a debriefing must make their request in accordance with the requirements of FAR 15.505 or 15.506, as applicable.

L.2.3. Discrepancies

If an offeror believes the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CO in writing with supporting rationale and recommended remedies for the CO to consider in order to correct the issue(s). (NOTE: The offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion.)

L.2.4. Electronic Reference Documents

All referenced documents for this solicitation are available on the Federal Business Opportunities (FBO) website at https://www.fbo.gov/. Potential offerors are encouraged to subscribe for e-mail notifications to be updated when information has been posted to the website for this solicitation.

L.2.5. Conflicting Proposals

Proposals must be submitted in both electronic and paper format as specified in L.2.7 and Table 1.2. In case of a conflict between paper and electronic proposal copies, the paper version will take precedence. Telegraphic or facsimile offers will not be accepted. The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the Solicitation Number, and the offeror’s POC name, address and phone number.

L.2.6. Visitor Pass – Hand Carried Proposals

Offerors are cautioned that Joint Base San Antonio (JBSA)-Randolph has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass to gain entrance. Some delay should be anticipated when hand carrying proposals. Late proposals will be processed in accordance with FAR 52.215-1(c) Submission, modification, revision, and withdrawal of proposals.

L.2.7. Organization/Number of Copies/Page Limits

The proposal shall consist of four (4) separate volumes as set forth in the Proposal Organization Table (Table 1.2 below). Volume titles, submittal formats/quantities, page limits, and due dates shall comply with Table 1.2. The specific contents of each proposal volume are described in the paragraphs below:

L.2.7.1. Page Limitations

Page limitations for each volume are listed in Table 1.2 above and shall be treated as maximums. Each page shall be counted except the following: the transmittal letter, cover pages, tab-divider pages, title pages, table of contents, lists of tables and drawings, glossary of acronyms, cross-reference matrix, position qualifications or manpower matrix. If the proposal volume exceeds the page limitation, the excess pages will not be read or considered in the evaluation of the proposal volume. Page limitations may be placed on responses to Evaluation Notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the offerors.

L.2.7.1.1. Page Size and Format

A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Narrative documentation shall be submitted in Microsoft Word (version Office 2007 or higher). Page size shall be 8.5 x 11 inches, not including foldouts. Typing shall be single-spaced, not less than 12 font size (pitch), and Times New Roman font style. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least 1 inch margins on the top and bottom and ¾-inch side margins. Pages shall be numbered sequentially by volume. These page size and format restrictions shall apply to responses to ENs. These limitations shall apply to both electronic and hard copy proposals.

Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 11 x 17 inches in size. Foldout pages shall fold entirely within the volume, and count as a single page. Foldout pages may only be used for large tables, charts, graphs, diagrams and schematics, not for pages of text. For tables, charts, graphs and figures, the text shall be no smaller than 8 font size (pitch), Times New Roman font style. These limitations shall apply to both electronic and hard copy proposals.

L.2.7.2. Cross-Referencing

Cross-referencing within a proposal volume is permitted where its use would conserve space without impairing clarity. The offeror shall provide a Solicitation Cross Reference Matrix indicating by Section L, PWS, paragraph number, the corresponding proposal paragraph in that section which addresses the referenced item. The matrix shall be added to the Volume II – Technical Table of Contents. Information required for proposal evaluation which is not found or referenced in its designated volume, and is not indicated on the cross-reference matrix, will be assumed to have been omitted from the proposal.

L.2.7.3. Indexing

Each volume shall contain a detailed Table of Contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections.

L.2.7.4. Glossary of Abbreviations and Acronyms

Volumes I, II and III shall contain a glossary of all abbreviations and acronyms used, and an explanation for each. Glossaries do not count against the page limitations for their respective volumes.

L.2.7.5. Binding and Labeling

Mark original hard copy as “Original” (for contract file), and all others as “Copy.” The “Original” for each volume shall be unbound and also include a standard (5 ½ inch-spaced) two-hole punch applied to the top of each page when viewed in Portrait orientation (for Government filing). Each hard-copy volume (other than the “Original”) of the proposal shall be separately bound in a three-ring, loose leaf binder permitting the volume to lie flat when open. Staples shall not be used. Volumes I through IV shall not be submitted together within the same binding. Each volume should include a cover sheet that clearly indicates the volume number, title, copy number, solicitation identification, and the offeror's name. The same identifying data should be placed on the spine of each binder. Be sure to apply all appropriate markings including those prescribed in accordance with FAR 52.215-1(e), Restriction on disclosure and use of data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection Information.

L.2.8. Electronic Offers

The content and page size of electronic copies must be identical to the hard copies. For electronic copies, use separate electronic folders to permit rapid location of all portions, including subfactors, exhibits, annexes, and attachments. The offeror shall submit volumes I through IV in electronic format, using recordable CD/DVDs. Each volume shall be in a different directory on a CD/DVD. The electronic copies of the proposal shall be submitted in a format readable by Microsoft (MS) Office Word 2007, MS Office Excel 2007, MS Office Project 2007, and MS Office Power Point 2007, or newer.

L.2.9. Distribution

In accordance with Table 1.2 above, mail or hand-deliver required proposal volumes to the address below:

L.3. VOLUME I –PRICE AND OTHER CONTRACTING CONSIDERATIONS

L.3.1. General

The Price and Other Contracting Considerations proposal shall be prepared according to these instructions and will be evaluated in accordance with the evaluation criteria in Section M of this RFP. Compliance with these requirements is mandatory and failure to comply may render the proposal unawardable. The CD/DVD shall include all MS Excel files with all cell formulas viewable. Data submitted beyond that required by this instruction shall not be considered or evaluated.

L.3.2. Volume Organization

The Price and Other Contracting Considerations volume shall be organized according to the following general outline:

(3) Glossary

(4) Price Data

(5) Other Contracting Considerations

(6) Responsibility Determination Supporting Documentation

(7) Teaming Arrangement(s) and/or Letters of Intent

(8) Solicitation Requirements, Terms and Conditions

(9) Subcontracting Plan (Large Business Only)

L.3.2.1. Summary Page(s)

To aid the Government’s understanding, describe how the offeror’s proposed price is reasonable and matches the proposed technical approach to the meet the requirement. The Summary Page(s) shall not exceed two (2) pages.

L.3.3. Price Data

L.3.3.1. Price Reasonableness

The source selection will be conducted with the expectation of adequate price competition for this acquisition per FAR 15.403-1(c)(1); therefore, certified cost or pricing data are not required per FAR 15.403-1(b)(1). If at any time, however, during this competition the CO determines that adequate price competition no longer exists or if price reasonableness cannot be determined, offerors may be required to submit certified cost or pricing data or additional data other than certified cost or pricing data, as appropriate, for the CO to determine price reasonableness. The proposed prices shall be based on the offeror’s own technical approach submitted in the Technical Proposal (Volume II) and the Government’s PWS requirements.

Offers should be sufficiently detailed to demonstrate reasonableness and balance.

L.3.3.2. Section B

The proposed prices in Section B must be based on the offeror’s technical approach submitted in the Technical Proposal (Volume II). In addition, the price proposed for the Base Period (CLIN 0001, Mobilization) must be supported by rationale and must correspond to efforts described in the Technical Proposal for the mobilization approach. The offeror shall propose and insert a whole dollar unit price for each FFP CLIN of the Base Period and each Option Period in the Section B, Price Schedule, of the RFP. A whole dollar unit price will be a price without the presence of hidden decimal places. The total extended price for each FFP CLIN must equal the whole dollar unit price multiplied by the quantity of units. If a discrepancy occurs in the extended amount due to hidden decimal places, the proposed whole dollar unit price multiplied by the quantity will take precedence. In addition, if a unit price is proposed with decimal places, the unit price will be rounded up or down and the resulting whole dollar will be multiplied by the quantity and that extended price will take precedence. The sum of all FFP CLINs, for the Base Period and each Option Period, will be the offeror’s Total Evaluated Price (TEP).

The offeror shall not provide a price for cost-reimbursable CLINs.

Data beyond that required by this instruction shall not be submitted, unless it is essential to document or support the offeror’s price. All data relating to the proposed price, including all required supporting documentation, must be included in the Price volume (Volume I). Under no circumstances shall this data and documentation be included elsewhere in the proposal.

L.3.4. Other Contracting Considerations

L.3.4.1. Include on each proposal cover letter or proposal cover page: name, office phone (and if applicable, cell phone number), and e-mail address for the proposal point of contact. The offeror must complete the following and include in Volume I:

(a) Standard Form 33 (SF 33), Blocks 13-18, of the RFP: In completing these blocks, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. Include in the “Original” hard copy proposal the originally signed (“wet” signature) copy of the RFP.

(b) Standard Form 30 (SF 30), Blocks 15A-15C, for any/all amendments. Include in the “Original” hard copy proposal the originally signed (“wet” signature) copy of any/all amendments.

(c) Complete all necessary fill-ins and certifications in Sections I through K. Ensure compliance with RFP requirements if using the annual representations and certifications electronically via the System for Award Management (SAM) website at https://www.sam.gov/portal/public/SAM/. Ensure the Tax Identification Number (TIN) is loaded in SAM or provided in the proposal submission. If the CO is unable to access/retrieve an offeror’s annual representations and certifications, that offeror may, upon request by the CO, be required to submit a hard copy of their annual representations and certifications. This request will not be construed as communications or discussions with an offeror as defined by FAR 15.306.

(d) Submit a letter, signed at the vice-president level or equivalent or higher, that identifies the individual(s) with the authority to obligate, negotiate or otherwise bind the offeror under the resulting contract and, if necessary, future contract modifications.

(e) Statement of Compliance with the Service Contract Labor Standards (SCLS). The SCLS Wage Determination (WD) provided in Section J, Attachment 3, is applicable to this requirement. The offeror shall provide a statement acknowledging:

i. Compliance with the requirements of the SCLS WD;

ii. The prices proposed for labor covered by the SCLS WD were formulated in consideration of the rates specified in the document as required for covered contractor personnel;

iii. The wage and benefit rates specified in the SCLS WD are not escalated beyond the basic period of performance (FAR 52.222-43(a) and (b)) and likewise, the prices proposed for labor covered by the SCLS WD were not escalated beyond the basic period of performance; and

iv. The proposed prices for each CLIN demonstrate a logical correlation to the staffing (e.g., Full-Time Employees’ man-years, man-hours, labor categories, skills mix) proposed in the Technical Proposal at Volume II.

(f) Statement Of Compliance with the Collective Bargaining Agreement (CBA). The CBA provided in Section J, Attachment 4, is applicable to this requirement. The offeror shall provide a statement acknowledging:

i. Compliance with the requirements of the CBA;

ii. The prices proposed for labor covered by the CBA were formulated in consideration of the rates specified in the document as required for covered contractor personnel;

iii. The wage and benefit rates specified in the CBA are not escalated beyond the basic period of performance (FAR 52.222-43(a) and (b)) and likewise, the prices proposed for labor covered by the CBA were not escalated beyond the basic period of performance; and

iv. The proposed prices for each CLIN demonstrate a logical correlation to the staffing (e.g., Full-Time Employees’ man-years, man-hours, labor categories, skill mix) proposed in the Technical Proposal at Volume II.

L.3.4.2. Responsibility Determination and Supporting Documentation

To assist in determining contractor responsibility in accordance with FAR 9.104-3(a), the offeror shall:

(a) Explain how they plan to maintain adequate financial resources for a contract of this magnitude and length;

(b) Address a fiscal plan for covering mobilization, unexpected or emergency situations, and initial performance costs prior to recoupment in the form of monthly invoice payments;

(c) Provide complete financial statements for one (1) fiscal year and year-to-date financial information through the last quarter available. The year-end financial statements must be certified by an independent accounting firm;

(d) If the offeror intends to rely on internally available working/operation capital, documentation of availability must be submitted. If the offeror plans to rely on financial support from external sources, documentation of availability and maximum lines of credit available (based upon the inclusion of this contract effort) must be submitted. Evidence of external funding must be provided in the form of a dated letter from the external source.

IAW FAR 9.104-5, the offeror is required to comply with the submission regarding FAR 52.209-5, Certification Regarding Responsibility Matters found in Section K of this RFP. Since it is the Government’s intent to make Award Without Discussions IAW FAR 52.215-1, the Government may request an offeror submit additional information so a responsibility determination can be made. This request for additional information will not be construed as communications or discussions with an offeror as defined by FAR 15.306. An incomplete proposal may be considered to indicate lack of understanding of the requirement and may result in the entire proposal being determined “Unacceptable” and eliminated from further consideration.

L.3.4.3. Teaming Arrangements and/or Letters of Intent

If a teaming arrangement is contemplated, provide complete, verifiable information detailing the arrangement, to include any relevant and recent information on previous teaming arrangements with same partner. Submit information relative to the formation, organization and operation of the business unit. Additional information to be submitted shall include cost and revenue sharing agreements and any other information which describes the business arrangement. Disclose existing or planned agreements between and among the principals, each company’s responsibility for financial management of the venture, the business systems contemplated for use by the entity and their location.

L.3.4.4. Solicitation Requirements, Terms and Conditions

Exceptions taken to the terms and conditions or any other requirements of this RFP shall be identified. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost and specific requirements of the solicitation. Failure to comply with the terms and conditions of the solicitation may result in the offeror being removed from consideration for award. Under no circumstances shall exceptions be included elsewhere in the proposal.

L.3.4.5. Subcontracting Plan (Large Business Only)

IAW FAR 19.705-2, large business prime offerors shall provide a clear, detailed and realistic plan that complies with FAR 52.219-9 and DFARS 252.219-7003 and satisfies all of the requirements of FAR 19.704, DFARS 219.704, and AFFARS 5319.704(a)(1) in order to be determined acceptable by the CO prior to contract award. The offeror's submission must provide sufficient information to support the CO's review of the subcontracting plan to determine acceptability. If small business subcontract possibilities do not exist or are limited, the offeror’s plan must state such and provide rationale supporting the assertion. An approved Subcontracting Plan is required for contract award to a large business offeror. (NOTE: Small business participation may be addressed in a large business offeror’s subcontracting plan; however, it shall also be addressed in the offeror’s Small Business Participation Volume IV, as required by L.6 and M.6. criteria; however, FAR 52.219-9 Small Business Subcontracting Plan does not apply to small business concerns.)

For reference purposes, FY15 DoD subcontracting goals are provided below (NOTE: FY16 DoD subcontracting goals have not yet been published). These goals represent the percent of small business subcontracting as it relates to total subcontracting dollars, not the total contract dollar value.

Offerors should propose goals that are realistically obtainable and provide rationale for the proposed goals. The Subcontracting Plan Checklist is provided as guide in Section J, Attachment 9.

L.4. VOLUME II - TECHNICAL

L.4.1 General

Each offeror shall submit a Technical Volume with its proposal. The Technical Volume should be specific, clear and complete. Submissions will be evaluated against the Technical subfactors defined in Section M, Evaluation Factors for Award. Using the instructions provided below, provide a detailed approach to accomplish/satisfy all subfactors. Do not merely reiterate the objectives or reformulate the requirements specified in the RFP or PWS. By submitting a proposal, the offeror affirms that its organization will perform all requirements specified in the RFP and PWS.

L.4.2 Format and Specific Content

L.4.2.1 Technical.

In the technical volume, address your technical solution for meeting the Government’s minimum performance or capability requirements of each technical subfactor.

L.4.2.2 Volume Organization.

The Technical volume shall be organized according to the following general outline:

(2) Cross Reference Matrix

(3) Summary Page

(4) Glossary

(5) Subfactor 1 (Management Approach)

(6) Subfactor 2 (Maintenance Approach)

L.4.2.3 Summary Page(s).

To aid the Government’s understanding, provide an executive summary of the offeror’s proposed technical approach that explains how the approach will efficiently and effectively meet the requirement. The Summary Page(s) shall not exceed two (2) pages.

L.4.3 Technical Subfactors

L.4.3.1 Subfactor 1 (Management Approach)

This subfactor assesses the offeror’s approach to integrate and transition to achieve full performance on the first day of contract operations, sustain compliant mission support and effectively manage performance to ensure timely quality of services including workmanship, products, and customer support to meet aircraft maintenance requirements over the life of the contract.

L.4.3.1.1 Mobilization.

Provide a detailed approach, with timelines (to include start and completion dates for all elements listed below) to achieve full performance on the first day of contract operations. The approach must address, but is not limited to:

a) Approach to recruit, train, qualify/certify and obtain required security clearances for employees.

b) Approach to provide materials, equipment and vehicles (including mobile radio installations). Complete the Vehicle Matrix (Section J, Attachment 8): identify vehicle type, quantity, towing/seating capacity, fuel efficiency, and intended purpose by work section for each vehicle.

c) Transfer of Government furnished equipment, tools and facilities (e.g. inventory & acceptance).

L.4.3.1.2 Quality Management System (QMS).

Provide a detailed QMS approach that assures high quality maintenance with unbiased/independent internal inspection procedures. The proposed approach shall include, but is not limited to:

a) Complete Quality Control organizational chart with a supporting narrative identifying precise lines of authority and responsibilities (include augmentees, if applicable).

b) Methods to identify and track discrepancies, analyze root causes, and document discrepancy trends.

c) Corrective action strategy to include implementation, follow-up, monitoring and timely closure of identified discrepancies.

d) System to record, compute, assess and report performance measurement data.

e) Link QMS trends/results into training program focus areas.

L.4.3.1.3 Training.

Provide a detailed training program that ensures a trained, qualified, and certified workforce throughout the life of the contract. The program shall address/include, but is not limited to:

a) Overarching training management approach. Describe approaches to formal classroom training, on-the-job training (OJT), and training documentation. Additionally, address cross-utilization and remedial training programs.

b) Instructor staffing approach to include roles and responsibilities.

c) Procedures for certification/recertification of all employees. Describe unique approaches for new hires, entry level employees, and transitional employees.

L.4.3.1.4 Government Furnished Property.

Provide a detailed approach to manage Government furnished property/equipment (GFP/GFE) to include:

a) Procedures for maintaining accurate accountability and documentation of GFP/GFE throughout the life of the contract.

b) Process for implementing TC-Max (Tool Asset Management Software) throughout the maintenance organization.

L.4.3.1.5. eTools & Technical Data.

Provide a detailed approach to manage, distribute, and maintain eTools and hardcopy technical orders. Describe eTools hardware/software configuration processes, and explain methods to keep eTools and hardcopy technical orders current.

L.4.3.1.6. Data Integrity.

Provide a detailed approach to ensure data integrity is maintained for data systems and forms documentation (e.g., Integrated Maintenance Data System (IMDS), Aircraft Forms, Vital Records, etc.).

L.4.3.2 . Subfactor 2 (Maintenance Approach)

This subfactor assesses the offeror’s over-arching strategy to execute the aircraft maintenance mission for the 80th Flying Training Wing in the unique operational environment at Sheppard AFB.

L.4.3.2.1. Maintenance.

Provide a detailed approach to perform all functions of the aircraft maintenance organization within the uniquely complex Sheppard AFB operational environment (aging aircraft, largest T-38C fleet in AETC with high utilization rates, demanding ENJJPT syllabus, limited facility space, international training mission, etc.). Specifically, the approach shall include procedures to:

a) Perform T-6A and T-38C servicing, ground handling, launch and recovery, Pre-Flight/Thru-Flight/Basic Post Flight, Hourly Post Flight, Transfer/Acceptance Inspections, Special Inspections, Transient Maintenance service, and accurately documenting maintenance actions.

b) Perform T-6A and T-38C scheduled/unscheduled and phase maintenance requirements. (Include number of proposed phase docks per Mission/Design/Series (MDS))

c) Ensure sufficient mission capable primary and spare aircraft are available to meet daily, weekly, and monthly flying schedules.

d) Perform Time Compliance Technical Orders (TCTOs), Time Change Items (TCIs) and One Time Inspections (OTIs) on aircraft and support equipment.

e) Perform Jet Engine Intermediate Maintenance (JEIM) on J85 and Organizational Maintenance on PT6A engines.

f) Perform back-shop/dispatch maintenance support functions including: avionics, electrical-environmental, fuel systems, aircrew egress systems, hydraulics, Aerospace Ground Equipment (AGE), fabrication shops (media blast, paint, sheet-metal, welding, machine, corrosion control), and Non-Destructive Inspections (NDI).

g) Manage full production cycle to include: planning, scheduling, directing, and controlling all maintenance on primary mission and mission support aircraft.

h) Execute off-station recovery of aircraft in a timely manner while maintaining uninterrupted mission support at Sheppard AFB.

L.4.3.3 Subfactor 3 (Staffing and Workforce Approach)

This sub-factor assesses the offeror’s proposed aircraft maintenance organizational structure and staffing plan.

L.4.3.3.1 Organizational Structure.

Provide an approach that demonstrates a thorough understanding of workforce size, roles, and qualifications required to meet PWS requirements while complying with Collective Bargaining Agreement (CBA) limitations and mandates. The organizational structure documentation must include:

a) A complete Staffing/Skill (job classification)/Shift Summary Matrix provided in Section J, Attachment 7.

b) Descriptions of roles, responsibilities and relationships between each organizational element (e.g. branch, work center, shop).

c) Organizational charts, depicting staffing by function, for the base period and each individual option period.

d) Strategy used to determine cross-utilization of workforce within or between functional areas if cross-utilization is intended.

e) A chart depicting the approach to provide qualified backup capability for “one-deep” positions.

L.4.3.3.2 Manpower Determination.

Provide a CBA compliant manpower determination with supporting rationale. NOTE: Include mathematical calculations used to determine manning assignments for ALL full-time and part-time staff during each performance period. The manpower determination documentation must include:

a) Productive man-year calculations broken down by productive work hours (“on equipment” direct support) and non-productive work hours (i.e. breaks, training, personal hygiene, vacation, etc.) Detail rationale used to project straight time vs internally directed overtime, to include projections for each performance period.

b) Approach used to assign staff to work centers, shift(s), and skills (classification/level).

c) Explanation of all upward or downward manning adjustments planned after full performance begins for each organizational element impacted, to include supporting calculations.

L.5. VOLUME III - PAST PERFORMANCE

L.5.1 General

Submit a Past Performance Volume prepared in accordance with all instructions contained within this Section L. Proposals will be evaluated in accordance with the evaluation criteria in Section M.5.

L.5.2 Format and Specific Content

L.5.2.1 Volume Organization. The Past Performance Volume shall be organized according to the following general outline:

(4) Past Performance Information (PPI)

(5) Organizational Structure Change History

(6) Subcontractor Letters of Consent

(7) Client Authorization Letters

(8) Past Performance Questionnaires (PPQ) L.5.2.2 Summary Page.

Describe the role of the offeror and each subcontractor, teaming partner, and/or joint venture partner for whom the offeror is required to provide PPI in accordance with L.5.2.3. The Table of Contents and Summary Page(s) shall not exceed five (5) pages combined.

L.5.2.3 Past Performance Information (PPI).

Submit information on no more than four (4) total recent contracts the offeror considers most relevant in demonstrating their ability to perform the proposed effort. Two (2) of the past performance references may be for subcontractor, teaming partner, and/or joint venture partners, that will perform major or critical aspects of the requirement. “Recent” is defined as ongoing contracts or those completed within three (3) years of the date of issuance of this solicitation, that includes at least six months of documented performance during the three-year period. Include rationale supporting the offeror’s assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining relevancy, reference Section M.5. The following may be provided as part of the PPI: quality and satisfaction ratings for contracts completed in the past three (3) years, e.g. letters, metrics, customer surveys, independent surveys, etc., which demonstrate customer satisfaction with overall job performance and service quality for relevant contracts. The PPI for each contract, including letters, metrics, customer surveys, independent surveys, etc., is limited to five (5) pages total per contract.

The Government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the offeror and other sources such as other Federal Government offices and commercial sources, to assess past performance. Ensure the Point of Contact provided in the PPI is available and knowledgeable of the service provided under that contract.

L.5.2.3.1 Specific Content.

The PPI for each contract or subcontract shall include the following:

(a) Contracting Agency/Customer

(b) Contract Number (for subcontracts, provide the prime contract number and subcontract number)

(c) Contract Type

(d) Total Contract Dollar Value per Year and/or Total Amount

(e) Date of Award

(f) Period of Performance

(g) Narrative addressing how performance of the subject contract is relevant to the requirements of this solicitation.

(h) Description of the role of the offeror and each subcontractor.

(i) Contracting Officer’s Name or Commercial Customer Point of Contact, Telephone Number and E-mail address

The offeror is required to identify and explain which aspects of the past/current contracts are relevant to the proposed effort and how they relate. This may include a discussion of steps taken by the offeror to resolve problems encountered on prior contracts, as well as past efforts to identify and manage program risks. NOTE: Merely encountering problems does not automatically result in to a “Limited” or “No Confidence” rating, since the problems encountered may have occurred on a more complex program, or an offeror may have subsequently demonstrated the ability to successfully overcome the problems. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems.

L.5.2.4 Organizational Structure Change History.

Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized divisions, business groups, subsidiary companies, etc. In many cases, the changes have taken place during performance of relevant past efforts or between the conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company that may have occurred during the past three years. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, indicate how the changes impact relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources. There is no page limit applied to Organizational Structure Change History; however, the offeror is requested to limit the number of pages to the minimum required to clearly articulate the pertinent information.

L.5.2.5 Subcontractor Letters of Consent.

Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Proposals must include a Consent Letter (Section J, Attachment 11) executed by each subcontractor, teaming partner, and/or joint venture partner, that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.

L.5.2.6 Client Authorization Letters.

If applicable, offerors must also include Client Authorization Letters (Section J, Attachment 12) for each identified effort performed for a commercial customer, authorizing release to the Government of requested information on the offeror’s performance.

L.5.2.7. Past Performance Questionnaires (PPQ).

The Government will evaluate the quality and extent of the offeror's performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the offeror and other sources such as other Federal Government offices and commercial sources, to assess past performance. Offerors are requested to complete the information in Sections 1, 2 and 3 of the PPQ (Section J, Attachment 15) and submit to POC(s) the offeror previously provided recent and relevant service. Ensure the appropriate POC is available and knowledgeable of the service provided under that contract. The POC shall submit completed questionnaires directly to the Government representatives identified in the PPQ, no later than the proposal due date identified in Table 1.2. above. Once submitted, the information contained therein shall be considered sensitive and shall not be released to the offeror. Include the completed Sections 1, 2, and 3 of the PPQ in the Past Performance proposal volumes.

L.6. VOLUME IV – SMALL BUSINESS PARTICIPATION

L.6.1. Volume Organization

No Table of Contents or Summary Pages are required.

L.6.2. Provide a narrative detailing the corporate commitment and specific steps taken to identify sources and provide subcontracting opportunities for small businesses.
