DEPARTMENT OF THE AIR FORCE

AIR EDUCATION AND TRAINING COMMAND

DATE

TO: Contractor References

The Headquarters Air Force Civil Engineer Support Agency and the Air Education Training Command Contracting Squadron is in the process of selecting contractors for award under the Sustainment/Restoration and Modernization task order contract. In general terms, this contract will be a focal point for acquiring a wide range of design and construction operations and services for federal customers worldwide. The length of this contract will be a minimum of 5 years. The Government wants to be confident that the contractors selected will be able to successfully accomplish our requirements.

One of the ways the Government intends to evaluate each Offeror is through the verification of the Offeror's past and present performance on other contracts. For this, we must depend on information received from agencies such as yours, which have had first hand experience with an Offeror.

We have tried to make our questionnaire as quick and simple to complete as possible. We ask that you go to the link provided below to complete and submit our questionnaire as soon as possible, preferably by 21March 2006.

(This letter is automatically generated and sent to the PPI reference upon the Offeror’s submission of the electronic PPI. A link will be provided to the referenced POC to complete the PPQ)

General contract information was provided by the Offeror. Please confirm this information and, in the text box provided, identify any errors or changes you feel should be made. Then, answer the questions that follow. We would greatly appreciate your comments describing why you rated a particular question the way you did. When finished, simply press the “submit” button and you are finished. If you have any questions or difficulty completing the questionnaire, please contact Mr. Ron A. Mortag or Lt Wanda McDonald E-mail: ron.mortag@randolph.af.mil Phone: 210-652-4856 E-mail: wanda.mcdonald@randolph.af.mil Phone: 210-652-2490
