Section L Attachment L-05 Past Performance Questionnaire - EWOCS The 30 and 45 Space Wings of the Air Force Space Command is in the process of competitively selecting a source for the Eastern Western Operational Communications Services (EWOCS) program. The nation’s largest launch and test range capability resides with the 45th Space Wing (45 SW) based at Patrick Air Force Base (AFB), FL and Cape Canaveral Air Force Station (CCAFS), FL and with the 30th Space Wing (30 SW) based at Vandenberg AFB, CA. These wings comprise the Eastern Range and Western Ranges, (ER and WR, respectively). These ranges provide capabilities for customers to safely launch, track and recover spacelift systems as well as perform operational test missions in support of ballistic missile deployment and aircraft flight tests. This PWS contains the requirements necessary to provide voice, data, and video capabilities to fulfill the missions of the ER and WR, tenants and mission partners. While some communications services performed for various units associated with the ER and WR remain on other contracts or are performed by government employees, EWOCS is intended to consolidate as much as reasonably possible and legally allowable. Describe in general terms the nature of the effort.

One of the considerations in proposal evaluation is the verification of the offeror’s past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.

Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 10 calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to kenyatta.harper@us.af.mil and rudy.mehu@us.af.mil or by hand and faxed to (321) 494-0557 (Attention: 45 CONS/PKDC - Ms. Kenyatta Harper). Your completed questionnaire will become a part of the official Source Selection records.

E EXCEPTIONAL – Performance meets contractual requirements and exceeds many requirements to the Government’s benefit. The contractual performance being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Client Authorization Letter NOTE TO OFFERORS: This procurement could be similar to commercial supplies/services. Therefore, to assist the Government’s Past Performance Team in assessing your present and past performance on relevant commercial contracts, the following letter must be sent to your points of contact (POCs) for those commercial efforts that you identify to us in your past performance volume. Should you propose to use major subcontractors and/or joint venture members, you must obtain a similar client authorization letter from each entity. HOWEVER, it is your sole responsibility, as the offeror, to then send out these authorization letters with the Present/Past Performance Questionnaire to your own POCs and to those of your major subcontractors and/or joint venture members and include a copy in your past performance volume.

As you know past performance has become an element of increased emphasis in the AF’s acquisitions. They are requesting that clients of companies who submit proposals in response to their RFP for the EWOCS Program be contacted, and that their participation in the validation process be requested. Then, either include these sentences: We, therefore, respectfully request and hereby authorize you to complete the attached Questionnaire with regards to work we have performed for you, and submit it either electronically, by fax or by mail. If sending electronically, the following applies:

Exchanges of source selection information between government and questionnaire respondents will be controlled by the Contracting Officer. Email may be used to transmit such information only if the email can be sent and encrypted, and must include “Source Selection Information – See FAR 2.101 and 3.104” in the subject line of the email. In order to facilitate the sending and receiving of encrypted emails, a questionnaire respondent must use MS Outlook email configured to support encryption, or, a different email product that is S/MIME compatible and configured to support encryption. If you intend to submit your completed questionnaire via an encrypted email, you will need to contact the Buyer/Contracting Officer indicated below, prior to that first submittal, in order to exchange certificates used for encryption. The offeror shall send out the Present/Past Performance Questionnaire to each of the offeror’s, major subcontractors’, teaming contractors, and/or joint venture members’ Points of Contact (POCs). The responsibility to send out the Present/Past Performance Questionnaires rests solely with the offeror - i.e., it shall not be delegated to any subcontractors and/or joint venture partners. The completed Present/Past Performance Questionnaire shall be submitted directly to the Government not later than [Offeror to fill-in closing date]. Each of the offeror’s POCs may submit its completed Present/Past Performance Questionnaire either electronically, by fax or by mail. If sending electronically, the following applies:

NOTE TO OFFERORS: Past performance information concerning major subcontractors, teaming contractors, and/or joint venture members cannot be disclosed to a private party without the major subcontractor's or joint venture member’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing major subcontractor/joint venture partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Past Performance Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the joint venture members/major subcontractors/teaming contractors identified in your proposal.
