SUBJECT: Past Performance Questionnaire for Contract Requirement FA2517-17-R-7003

1. We are currently responding to the request for proposal, FA2517-17-R-7003, issued by 21ST Contracting Squadron in Colorado Springs for the Modeling, Simulation, and Analysis (MS&A) for Space and Cyberspace Capabilities (MSCC) Support contract.

2. A major consideration in proposal evaluation is verification of past performance on contracts which reflect our ability to perform on the proposed effort. This acquisition requires we provide a past performance questionnaire to you since I believe you have first-hand experience with our performance on the contract, identified in the subject line.

3. It is extremely important to us that you please take a few moments to complete sections 3 and 4 of the attached questionnaire. Once completed, the detailed information contained in the questionnaire is considered Source Selection Sensitive and will NOT be released to us, the Offeror. Return responses directly to the contracting office as soon as possible, but not later than TBD by encrypted e-mail or by mail as follows:

Satisfactory (S)
Indicates the contractor’s performance meets contractual requirements and the goals and objectives of the PWS. The area of evaluation experienced some minor problems for which: either, 1) corrective action was taken and appears to be satisfactory, or 2) corrective action was identified that will probably correct the problems.
Marginal (M)
Performance does not meet some contractual requirements. The element being assessed reflects serious problems that were difficult to correct. Corrective action was either not identified, appeared only marginally effective, or was not fully implemented.
Unsatisfactory (U)
Performance does not meet contractual requirements and recovery is not likely in a timely manner. Contractor's corrective actions to date are ineffective.
