About this file

This document includes an experience questionnaire template and details of a related federal contract opportunity. The experience questionnaire is a template for offerors to list up to three similar projects performed in the past five years, including project details, owner information, value, dates, and performance remarks.

The related federal contract opportunity is a small business set-aside solicitation issued by the Department of the Interior National Park Service Northeast Region. It is to provide additional equipment, supervision and staff to monitor, test and maintain fire and intrusion alarms at Fort McHenry National Monument & Historic Site and Hampton National Historic Site. The resulting contract will be a firm-fixed price base year plus four option years. A site visit is scheduled for January 23, 2024 and questions are due by January 26, 2024 to the contract specialist.

Text version

Experience Questionnaire READ CAREFULLY: The purpose of this form is for the offeror to list projects performed during the past five (5) years that are similar to the work described in this solicitation. Provide a minimum of three projects. Use this form to address your familiarity with the Statement of Work and with the evaluation criteria described in the solicitation. Include Area Codes with all telephone numbers. Fill in the information by typing or printing legibly.

It is especially important that the offeror disclose instances in which their experience may be considered by others to be less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror’s experience.
