About this file

This file is Exhibit E which provides instructions for submitting Past Performance Questionnaires (PPQ) for a water sampling services contract at Marine Corps Air Ground Combat Center. The document specifies that if a completed CPARS evaluation is available, it must be submitted with the proposal and a PPQ should not be submitted. If no CPARS exists, contractors must submit the NAVFAC Form PPQ for each referenced project, ensuring accurate contact information is provided. PPQs should be submitted with the proposal, but if unable to obtain them before the closing date, contractors should submit the first page with contract and client details. PPQs may be submitted directly to the Government POC Ernesto Pangan if requested by the client.

The associated contract opportunity is a 100% small business set-aside IDIQ contract for water sampling services at MCAGCC Twentynine Palms, with a base year plus four one-year options and one six-month option (66 months total). Key requirements include 24/7 emergency service availability, 24-hour routine response time, 4-hour emergency response time, and ELAP accreditation. The site visit is scheduled for December 10, 2024, at 9:00 AM, RFIs are due December 13, 2024, at 2:00 PM, and proposals are due December 19, 2024, at 2:00 PM. Award will be based on Lowest Price Technically Acceptable criteria.

EXHIBIT “E”

1. The NAVFAC Form PPQ shall be utilized for all evaluations that require a Past Performance Questionnaire (PPQ).

2. Solicitation Submittal Requirements: IF A COMPLETED CPARS EVALUATION IS AVAILABLE, IT SHALL BE SUBMITTED WITH THE PROPOSAL. IF THERE IS NOT A COMPLETED CPARS EVALUATION, the Past Performance Questionnaire (PPQ) included in the solicitation is provided for the offeror or its team members to submit to the client for each project the offeror includes in its proposal for Factor (insert applicable factor number, usually Factor 1, and insert factor title, usually Corporate Experience). AN OFFEROR SHALL NOT SUBMIT A PPQ WHEN A COMPLETED CPARS IS AVAILABLE.

IF A CPARS EVALUATION IS NOT AVAILABLE, ensure correct phone numbers and email addresses are provided for the client point of contact. Completed PPQs should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment C), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government's point of contact, Ernesto T. Pangan, via email at ernesto.t.pangan.civ@us.navy.mil_ prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs or CPARS previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.
