About this file
This is a Past Performance Questionnaire template prepared by the Department of Veterans Affairs Network Contracting Office for contractors submitting proposals to federal contracts. The form instructs contractors to complete a Contractor Information section with company details, point of contact information, reference project specifics, and contract performance data, then distribute the questionnaire to past performance reference contacts who complete a separate Respondent Information section and evaluation criteria.
Reference contacts evaluate contractor performance using a 0-5 scale across multiple categories: Quality Management & Workmanship (six evaluation items covering management experience, personnel qualifications, quality control procedures, deficiency correction, submittal clarity, and problem-solving approaches); Timeliness and Adherence to Schedule (three items addressing delay notifications, submittal timeliness, and schedule adherence); Business Practices/Customer Relationship and Ability to Perform (four items covering initiative, independent work capability, subcontractor management, and change order responsiveness); Compliance & Safety (three items addressing reporting accuracy, federal law compliance, and safety program implementation); Infection Control (one item); and Overall Customer Satisfaction (four items including modification cost reasonableness, cure notice history, and contractor award likelihood). Reference contacts must return completed forms to Contract Specialist Amanda Murphy at Amanda.Murphy5@va.gov by the proposal due date, and must provide written remarks supporting their overall performance rating. This template is specifically designed for construction or service contracts managed by the VA and does not address specific products, services, or pricing terms.
