About this file

This is an Exhibit A template for documenting relevant prior project experience and joint experience with major subcontractors as part of a proposal submission to the federal government.

The exhibit consists of two parts. Part 1 requires offerors to submit no more than two (2) prior project experience forms if submitting as a prime contractor, or one (1) form if submitting as a subcontractor, with a maximum of three (3) total forms from the entire team. Each Part 1 form is limited to four pages and must describe a single project completed within five years of the solicitation issue date. Offerors must provide comprehensive project details including company name, contract number, contract type, original and current dollar values, project square footage, completion dates, and detailed descriptions of work performed directly by the firm and its employees. Offerors must explain the relevance of prior work to the solicited project and identify major subcontractors with indication of whether they will be used on the new project. Part 1 also requires submission of points of contact from the owner or government agency for reference verification purposes. Part 2, completed only by prime contractors, requires identification of major subcontractors and teaming partners to be utilized on the solicited project, with a minimum requirement for HVAC Systems subcontractor information and optional inclusion of one additional specialty trade. For each subcontractor listed in Part 2, offerors must document up to two previous or current joint projects performed within the past five years, including project details, contract completion dates, project size, final dollar values, subcontractor cost percentages, and scope descriptions. All subcontractors and teaming partners named in Part 2 must submit a completed Subcontractor Consent and Reference Form (Exhibit B).
