About this file
This file is an ECHO Project Annual Report Template issued by Volusia County, Florida's Resource Stewardship Division, establishing compliance and reporting requirements for facilities operating under the ECHO program. The template requires annual submissions by January 15 for the preceding calendar year and mandates that facility operators provide comprehensive documentation including facility identification, usage statistics, current fee schedules and hours of operation, maintenance activities completed and planned, and authorized certification that operations comply with all ECHO program requirements. The report must capture metrics such as total annual attendance, number of events held, practice sessions, unique participants, Volusia County resident participation percentages, youth participation under age 18, and days open to the public.
The template serves as a standardized compliance mechanism for public facilities managed under Volusia County's ECHO initiative, requiring operators to document operational performance, facility maintenance, and adherence to program standards through certified attestation. Completed reports must be submitted to the Volusia County Resource Stewardship Division, ECHO Program office located at 123 W. Indiana Avenue, Room 302, DeLand, Florida 32720, or via email to echo@volusia.org. The template does not specify funding sources, pricing structures, or contract terms within its content; rather, it functions as an annual accountability instrument requiring operators to maintain transparent records of facility usage, maintenance stewardship, and community participation metrics.
