EXHIBIT D

BID REQUIREMENTS

The bid must include a detailed description of the doors, and must account for Bidder’s assistance/consultation related to DOC’s installation of the doors and ensuring that the installation is to specifications. Warranty information must also be provided.

The proposal may be provided on a separate sheet, and must be stated in a clear, concise, and easy to read format.

The cost proposal is a firm offer for a period of 150 days following receipt, and it may be accepted by DOC without further negotiation (except where obviously required by lack of certainty in key terms) at any time within the 150-day period.

Please keep in mind the following in preparing the bid:

1. DOC cannot pay in advance of good or services received.

2. The project involves the replacement of 3 Southern Folger door systems with the same door system.

3. DOC will perform all elements of the installation but will require assistance/consultation with the Bidder to ensure that installation is performed according to manufacturer's specifications.

4. Include detailed product and warranty information.

5. Bidder will be paid upon 50% upon the delivery of the door and the remaining 50% after Bidder’s assistance and consultation on the installation.
