About this file

This is an Anti-Collusion Affidavit form required by a state government entity as part of the bidding process for a contract opportunity. The affidavit serves as a sworn statement in which an authorized agent of the bidding entity certifies compliance with competitive bidding regulations and prohibitions against collusive practices.

The affidavit requires the bidder's authorized representative to swear under oath that the bidder has not engaged in collusion with other bidders to fix prices, refrain from bidding, or otherwise restrain competition. The affiant must also certify that no agreements have been made with any state official or employee regarding contract specifications, pricing, or other terms, and that no payments, gifts, donations, or promises of value have been provided to government officials or employees in connection with obtaining the contract award. The form includes spaces for the state and county identification, the affiant's signature, and notarization by a Notary Public, Clerk, or Judge, with a date field for execution.
